In 2021, the nurse labor force shortage has continued to increase with 37.4% of hospitals feeling the squeeze [1]. Healthcare facilities are experiencing an increase in RN position vacancy rates due to the pandemic. The national hospital turnover rate is currently 19.5% and a third of hospitals in the United States are reporting a vacancy rate of greater than 10% [1]. The RN Recruitment Difficulty Index remains elevated at the level of an 89-day period to hire an experienced RN. This finding is evidence that the RN shortage will persist, and RN recruitment will continue to be a challenge for hospitals. Millennials currently represent one quarter of today’s population and will represent 75% of the workforce by the year 2030. A millennial is defined as a person born between the years 1981 and 1996 with a current age of 25 to 40 years old [1]. As different innovations find their way into the healthcare sector, physicians and patients experience some challenges as they try to adopt these innovations. One particular innovation in healthcare has been the use of patient portals to increase self-management of different conditions by patients. This study focused in exploring the barriers in the access of the My Chart portal at Baptist Memorial Health Care Corporation and, to some extent, how to handle this problem. An online survey questionnaire was sent to 22 help care desk agents who have been employed with the organization for more than 90 days and face-to-face interviews were scheduled with 5 of these participants. Data obtained from the 19 respondents who responded to the questions and the five interviewees revealed that the major barrier to patient portal access is the lack of computer or phone skills. Most of the factors such as congenital endowment, emotion, diet, lifestyle and other factors lead to spleen loss, liver loss and suffocation, poor air movement, and the water-salt and sweet things are transformed into phlegm and lipid accumulation. Among them, spleen and liver dysfunction plays an important role in the pathogenesis of this disease. Spleen deficiency and dampness are the constitutional basis of obesity T2DM, and poor airway is an important disease condition. Using the covariance structural equation modeling along with confirmatory factor analysis to measure the theoretical constructs for the data on acute care hospitals in the United States, the authors demonstrate that IT capability, hospital-physician integration, and innovativeness directly affected the variability in standardization. Methods: A recently discharged patient presented to the community pharmacy for a Transitions of Care (TOC) service. Medication refill histories from the pharmacy and insurance company were gathered, and the patient’s discharge paperwork was requested from the hospital to identify any medication discrepancies and medication-related problems (e.g., medication non-adherence, duplicate therapy, drug-drug interactions). After obtaining this information, the pharmacist and an Advanced Pharmacy Practice Experience (APPE) student attempted to perform a medication reconciliation and identity medication-related problems to create a Personal Medication List (PML) and Medication Action Plan (MAP). Results: The community pharmacist and an APPE student were unable to effectively implement TOC in a community pharmacy-based setting due to several barriers in the healthcare system including misuse of the Health Insurance Portability and Accountability Act (HIPAA) and delayed response or nonresponse from Healthcare Providers (HCPs). As a result, the patient has been readmitted twice since the initial hospitalization. Conclusions: The patient continues to be at high risk for medication-related errors, health complications, and hospital readmissions due to unresolved medical issues and medication non-adherence. Kaul","doi":"10.29011/2688-6472.000024","DOIUrl":"https://doi.org/10.29011/2688-6472.000024","url":null,"abstract":"This paper provides a critical review of existing procedures and practices, including surveillance methods and systems for identifying patients’ risk of acquiring healthcare associated respiratory infections, in different settings. The burden of healthcare associated respiratory infections afflicting patients and healthcare workers is underappreciated. Infection control measures for TB control have been strictly applied in the United States and have decreased rates of disease acquisition below that of the general population. However, in most other countries healthcare workers are greater risk of acquiring TB and other infections. The principles of these TB and general infection control measures can be applied in resource scarce settings and should reduce the transmission of TB in particular but may provide protection from other respiratory infections as well. As facilities vary widely in the complexity of care they provide, a framework of risk assessment should be used to identify and apply engineering controls, work practice controls, administrative controls, personal protective equipment and finally education. Novel and creative solutions, including use of telemedicine and mHealth solutions should be further explored. As care becomes more complex in resource constrained settings, the necessary infrastructure related to water, energy, medical records, ability to do preventive maintenance and sterilize (reprocess) devices must be put into place so that the benefits of invasive care exceed the risks to the patients. The volume of water available per patient and family in hospitalized patients is a crude indicator of the quality and sophistication of care that can be provided. As we seek improve infection control as much as possible with simple interventions, it is important to realize that once again, limits are reached without a broader system infrastructure strengthening. Safe care does not harm the patient, the provider or the community. Implementation of safe care requires risk assessments and provision of appropriate ventilation and adequate volumes of water. Because the preconditions for a safe environment of care cannot be assumed in resource scarce settings, there may be limitations in the effective application of US infection control “bundles”. Infection control measures will have to be tested prior to implementation. Unlike in the United States, the impact of infection control measures may have to be extrapolated from studies done at sentinel sites. Measures can then be applied to facilities and evaluated with process control measures.
