Rowan","doi":"10.1097/01.OGX.0000151757.63955.95","DOIUrl":"https://doi.org/10.1097/01.OGX.0000151757.63955.95","url":null,"abstract":"OBJECTIVE\u0000To identify clinical factors that might identify women with vulvar lichen sclerosus who are at increased risk of developing squamous cell cacinoma.\u0000\u0000\u0000STUDY DESIGN\u0000A retrospective, case-control study compared 46 women presenting between 1992 and 2000 with clinical and histologic evidence of squamous cell carcinoma of the vulva arising in a background of lichen sclerosus and 213 new referrals with vulvar lichen sclerosus during the same period.\u0000\u0000\u0000RESULTS\u0000The ages of the patients and presence of clinical hyperplasia were the only differences between the 2 groups.\u0000\u0000\u0000CONCLUSION\u0000Women presenting with vulvar cancer arising within a background of lichen sclerosus are significantly older than women presenting with lichen sclerosus. In addition, clinical evidence of squamous hyperplasia is independently associated with vulvar carcinoma. Ovarian volume, antral follicle counts, serum hormone levels and ovarian artery resistivity index values were measured before and 1 year after the sterilization procedure on the third day of the menstrual cycle.\u0000\u0000\u0000RESULTS\u0000Serum hormone levels, mean ovarian volume (MOV) and mean follicle counts (MFC), were not statistically significant before and 1 year after the operation (P > .05). The difference in ovarian artery resistivity index values was not statistically significant before and 1 year after the operation (P > .05). A significant correlation was observed between FSH, MOV and MFC before and after the operation (P < .05). Mazor","doi":"10.1097/01.OGX.0000137615.96233.53","DOIUrl":"https://doi.org/10.1097/01.OGX.0000137615.96233.53","url":null,"abstract":"OBJECTIVE\u0000To determine the rate, obstetric characteristics and perinatal outcome of pregnancies with uterine leiomyomas.\u0000\u0000\u0000STUDY DESIGN\u0000A population-based study comparing all singleton deliveries between the years 1988 and 1999 in women with and without uterine leiomyomas was performed. Patients lacking prenatal care were excluded from the analysis. Multivariable analysis, adjusting for possible confounders, such as maternal age, parity and gestational age, was performed to investigate associations between uterine leiomyomas and selected outcomes.\u0000\u0000\u0000RESULTS\u0000There were 105,909 singleton deliveries with 690 (0.65%) complicated by uterine leiomyomas during the study period. Using a multivariable analysis, the following conditions were significantly associated with uterine leiomyomas: nulliparity (odds ratio [OR]=4.0, 95% confidence interval [CI] 3.3-4.7, P<.001), chronic hypertension (OR=1.9, 95% CI 1.6-2.4, P<.001), hydramnios (OR=1.5, 95% CI 1.2-2.0, P<.001), diabetes mellitus (OR=1.4, 95% CI 1.1-1.7, P=.001) and advanced maternal age (OR=1.2, 95% CI 1.1-1.2, P<.001). Higher rates of perinatal mortality (2.2% vs. 1.2%, OR=1.8, 95% CI 1.1-3.2, P<.001) were found in the uterine leiomyoma group as compared to the control group. While adjusting for maternal age, parity, gestational age and malpresentation, pregnancies with uterine leiomyomas had higher rates of cesarean deliveries (OR=6.7, 95% CI 5.5-8.1, P<.001), placental abruption (OR=2.6, 95% CI 1.6-4.2, P<.001) and preterm deliveries (<36 weeks' gestation, OR=1.4, 95% CI 1.1-1.7, P=.009) as compared to pregnancies without uterine leiomyomas. Wolf","doi":"10.1097/00006254-200406000-00015","DOIUrl":"https://doi.org/10.1097/00006254-200406000-00015","url":null,"abstract":"OBJECTIVE\u0000To determine the rates and types of acute complications occurring within 14 days of large loop excision of the transformation zone of the cervix.\u0000\u0000\u0000STUDY DESIGN\u0000A retrospective chart review was performed for patients who underwent a loop excision procedure between July 1999 and July 2001. Extracted information included age, parity, ethnicity, indication for the procedure, risk factors for cervical intraepithelial neoplasia, histologic classification of the specimen, and type and incidence of complications. Length of follow-up was also recorded. Statistical analysis was performed utilizing Wilcoxon sum analysis.\u0000\u0000\u0000RESULTS\u0000Five hundred fifty-seven charts were re- viewed; 74.8% of the patients were < or = 40 years old. Ethnicity was 59% Hispanic. The most common indication for loop excision was high grade squamous intraepithelial lesions on a Pap smear. Of the patients, 40% used tobacco, and 30% had a sexually transmitted disease. The overall complication rate was 9.7%, with a major complication rate of 0.6% and minor complication rate of 9.1%. Minor complications included 14 cases of abdominal pain and 26 of vaginal bleeding, 7 of which required treatment with Monsel solution or placement of Surgicel (Ethicon, Albuquerque, New Mexico). There were 6 cases of vaginal discharge and 1 of bladder spasm. Major complications included 1 patient with a bowel injury who returned to the operating room, 1 who returned to the operating room for hemorrhage and 1 with prolonged observation for chest pain. There was no association between age, parity, risk factors for cervical dysplasia, indication for procedure or pathology of specimen with acute complications.\u0000\u0000\u0000CONCLUSION\u0000Any surgical procedure may cause acute complications.
