In Germany, the Medical Informatics Initiative (MII) and the Network University Medicine (NUM) are of central importance for this transformation. This review article outlines these structures and highlights their future role in enabling privacy-preserving federated multi-center analyses with interoperable data structures harmonized between site-specific IT infrastructures. A mean follow-up time of 58 months (13-176), overall and progression free survival (OS, PFS) were registered. A volume of interest for the primary tumor was defined on PET and CT images. Parameters SUV<sub>max</sub>, PET-solidity, PET-circularity, and CT-volume were analyzed. To evaluate the prognostic value of each parameter for OS, a minimum p-value approach was used to define cutoff values, survival analysis, and log-rank tests were performed, including subgroup analysis for combinations of parameters.</p><p><strong>Results: </strong>Mean OS was 58±28 months. Poor OS was associated with a tumor CT-volume >14.3 cm<sup>3</sup> (p=0.02, HR=7.0, CI 2.7-17.7), higher SUV<sub>max</sub> values >12.2 (p=0.003; HR=3.0, CI 1.3-6.7) and PET-solidity >0.919 (p=0.004; HR=3.0, CI 1.0-8.9). Combined parameter analysis revealed worse prognosis in larger volume/high SUV<sub>max</sub> tumors compared to larger volume/lower SUV<sub>max</sub> (p=0.028; HR=2.5, CI 1.1-5.5), high PET-solidity/low volume (p=0.01; HR=2.4, CI 0.8-6.6) and low SUV<sub>max</sub>/high PET-solidity (p=0.02, HR=4.0, CI 0.8-19.0).</p><p><strong>Conclusion: </strong>Even in this group of low-risk NSCLC patients, we identified a subgroup with a significantly worse prognosis by combining morphologic-metabolic biomarkers from [18F]FDG-PET/CT. The combination of SUV<sub>max</sub> and CT-volume performed best. Das Programm soll jüngere Vertreterinnen sowie Vertreter aller durch die Nuklearmedizin vereinigten Berufsgruppen ansprechen und ist auf Soft Skills fokussiert, welche bei Fachkongressen nicht adäquat berücksichtigt werden können. Schwerpunkte der diesjährigen „Young DGN Autumn School“, welche am 17. und 18. November 2023 in Frankfurt am Main stattfindet, werden Stressmanagement und interkulturelle Kommunikation sein. Darüber hinaus gibt das Format dem nuklearmedizinischen Nachwuchs Gelegenheit zur Vernetzung. Weitere Informationen sind der „Young DGN“-Online-Präsenz zu entnehmen und Bewerbungen per E-Mail an youngdgn@nuklearmedizin.de zu richten. Die Kursgebühren sowie Hotelkosten der ausgewählten Kandidatinnen und Kandidaten werden durch ein Stipendium gedeckt. Results of the last survey 2018 are set in squared brackets.</p><p><strong>Results: </strong>MPS data from a total of 133,057 [145,930] patients (-8.8%) with 131,868 [143,707] stress and 106,546 [121,899] rest MPS were analysed. A comparison with official data revealed that 54% all MPS were recorded. From 2018 to 2021, official data showed a every year an increase in MPS numbers. On average, 610 [502] MPS patients (+22%) were examined in each department. 74% [69%] of the responders reported an increase or no changes in their MPS patient numbers. Ambulatory care cardiologists represented as always, the mayor referral group (68% [69%]). For the first time, pharmacological stress was more frequently applied than ergometry (42% [51]). Regadenoson was mostly used. The use of the different protocols remained nearly unchanged. Two-day protocols were predominantly applied (49% [48%]). A shift from multi-headed cameras (58% [72%]) to SPECT-CT systems (24% [17%]) was found. Attenuation correction was performed in 33% [26%] of all MPS. 88% [86%] of all stress, 88% [87%] of all rest and 87% [83%] of all stress and rest MPS were acquired as gated SPECT. 72% [67%] of all departments performed scoring by default. The number of departments without scoring decreased to 13% [16%].</p><p><strong>Conclusions: </strong>The MPS Study 2021 shows that the long-term positive development of MPS imaging in Germany is continuing. The COVID-19 pandemia did not change this trend.
