Despite the potential benefits, endovascular stent grafting may elicit an unexpected systemic inflammatory response, called postimplantation syndrome (PIS). The main features of PIS include fever, elevated C-reactive protein levels, leukocytosis and/or coagulation disturbances, perigraft air on abdominal computed tomography, and no evidence of infection. The main management of PIS is supportive care. Antibiotics have no clinical benefit. Patients with maintenance hemodialysis are prone to opportunistic infections because of altered immunity, and frequent exposures to health-care associated infections. Herein, we report a case of candida arthritis of right shoulder with preceding fungemia in patients with maintenance hemodialysis. The diagnosis is based on the isolation of Candida Tropicalis from blood and synovial fluids of the shoulder joint. The patient has received intravenous fluconazole and arthroscopic surgical debridement. We then changed the fluconazole into amphotericin B due to the abnormal signs in the liver function tests, although the fluconazole successfully controlled fungemia and arthritis. In Korea, no studies have compared these elderly patients with younger patients. Methods: This study is based on patients who were diagnosed as having rib fracture at the Emergency Department of the Seoul Medical Center from March 2013 to April 2011. The medical records and radiological examinations of 192 cases were reviewed. Two groups were created-older than 65 years and younger than 64 years. Comparisons were made between the two groups including place of injury, time of injury, associated injuries, and the final result in the Emergency Department. Results: The final patient sample size was 192. The elderly group had 142 patients with the average age being 48.77± 9.70 years and 75.4% males. The young group had 50 patients with the average age at 75.90±7.21 years and 25% males. The most common place of injury was road (p=0.007) in the young group and home (p=0.002) in the elderly group. The most common mechanisms of injury were slipping (47.4%), traffic accidents (18.2%), falling (14.1%), and assault (9.4%). Falling was more prevalent in the young group than in the elderly group (p=0.011). Rib fractures occurred at night (p<0.001), on Saturdays, on Sundays, and in the winter in the young group and in the morning(p =0.002), on Mondays, on Fridays, and in the winter in the elderly group. Associated injuries, in descending order, were chest, limb, head, and spine. This study was aimed to investigate the etiologies and underlying diseases of leg edema in elderly patients. Methods: We retrospectively reviewed medical records of 247 patients aged over 65, who visited an Emergency Department of a tertiary hospital due to leg edema from January 2010 to December 2012. Results: A total of 226 patients with complete medical records were included. The most common cause of leg edema in elderly patients was renal failure (42 cases, 18.6%), followed by heart failure (37 cases, 16.4%), and deep vein thrombosis (32 cases, 14.2%). However, the etiologies were not established in 66 cases (29.2%). Patients with leg edema caused by renal and heart failure had renal (40 cases, 95.2%) and cardiovascular diseases (29 cases, 78.4%), respectively, while others had diabetes mellitus and trauma. Patients with leg edema caused by deep vein thrombosis had underlying conditions such as cancer (13 cases, 40.6%), trauma, surgery within 1 year, and diabetes mellitus. Overall, chronic bilateral edema (120 cases, 53.1%) was most commonly observed form of leg edema in elderly patients. Deep vein thrombosis, cellulitis, and lymphedema usually caused unilateral edema, whereas systemic diseases such as renal failure, heart failure, and liver cirrhosis caused bilateral edema. Conclusion: Leg edema in elderly patients is usually caused by systemic diseases such as renal and heart failure closely related to underlying diseases. With the increase of older population, physicians should know the clinical significance of multimorbidity in managing older patients. Multimorbidity focuses on the findings of multiple diseases in a single patient and interactions between diseases and a patient. It has been reported that the prevalence of multimorbidity is over 50% in elderly people and co-occurrence of certain chronic diseases is often observed in patients with multimorbidity. Multimorbidity is significantly associated with higher mortality, increased disability, impaired quality of life, and increased medical costs. Unfortunately, current guidelines do not address enough for older patients with multimorbidity. Simple and fragmented approach for multiple medical conditions in older patients may lead to many clinical problems including polypharmacy, iatrogenic events, and increased medical cost. Modern geriatrics was born with the coinage of the word ‘geriatrics’ by Ignatz Leo Nascher. If Nascher was the father of geriatrics, Marjory Warren was its mother. The American Geriatrics Society was organized in 1942 and Journal of the American Geriatrics Society was published in 1946. The Japan Gerontological Society started in 1959, consisting of the Japanese Geriatric Society and the Japan Socio-Gerontological Society. The Korean Geriatrics Society was inaugurated on October 3rd in 1968 and the Korean Geriatrics Association was established for financial replenishment of research and academic activities in 2008. In 2002, 1st Joint symposium between Korean Geriatrics Society and Japanese Geriatrics Society was held. Herein, we present a case of adenosquamous carcinoma of the transverse colon with hepatic metastasis. The patient is a 72-year-old woman who is presented with a chief of lower abdominal pains. Her colonoscopy has indicated an ulcerofungating mass about 4 cm × 3 cm in size in the distal transverse colon, and the biopsy confirmed her diagnosis of adenosquamous carcinoma. An abdominopelvic computed tomography shows an circumferential enhancing mass on her distal transverse colon with three hepatic metastatic nodules. Both colo-ovarian fistula and tuboovarian abscess formation, as complications of diverticulitis, are rare. Here, we report a case of colonic diverticulitis which ultimately resulted in colo-ovarian fistula and tuboovarian abscess formation. A 76-year-old female was presented with lower abdominal pain, diarrhea and intermittent hematochezia for about 1 month. According to the abdominal computed tomography, tuboovarian abscess was shown to adhere to the inflamed sigmoid diverticulum. Sigmoidoscopy revealed pus flowing from the opening of the diverticulum at the sigmoid colon. Hence, the patient underwent oophorosalpingectomy and low anterior resection. Thereafter, she was treated with bowel rest and broad spectrum antibiotics, and finally recovered. Although rare, colonic diverticulitis may result in colo-ovarian fistula causing tuboovarian abscess formation in postmenopausal patients. Methods: Patients with complaints of subjective memory impairment were categorized into subjective memory impairment (SMI, n=42) and mild cognitive impairment (MCI, n=42) based on their scores in the Korean version of MiniMental State Examination, Activities of Daily Living (ADL), and Clinical Dementia Rating and the neurologist’s diagnosis and evaluation. The neurocognitive function and depression rating between the SMI and the MCI groups were compared using the Seoul Neuropsychological Screening Battery and the Geriatric Depression Scale. Results: The SMI group showed normal concentration, visual-spatial construction ability, and depression and deficits in the processing of memory consolidation through the delay test of verbal learning. The MCI group also showed no deficits in concentration but showed impairments in delayed recall and recognition test of verbal learning, visual memory test, and frontal lobe/executive functions. Conclusion: The MCI group showed more distinctive neurocognitive deficits in delayed recall and recognition test of verbal learning; encoding, storage, and retrieval of visual memory; and naming ability compared to the SMI group.
