This could have contributed to the occurrence of back pain in this group. The aim of the study was to assess back pain in a group of teachers before and after the introduction of distance learning due to the COVID-19 pandemic.</p><p><strong>Material and methods: </strong>The study was based upon an online diagnostic survey and included 361 teachers and university lecturers from 16 provinces of Poland. In order to assess the intensity of pain, a Numerical Rating Scale (NRS) was used. Statistical analysis was performed using IBM SPSS Statistics 26 and Microsoft Excel 2021. The level of statistical significance was adopted on the level p < 0.05.</p><p><strong>Results: </strong>Among the surveyed teachers, the mean degree of declared cervical and lumbar spine pain increased significantly after the COVID-19 pandemic (p < 0.001). The mean value of the declared degree of cervical pain increased from 2.49±2.81 before the pandemic to 5.44±1.83 during the pandemic. Before the pandemic, the mean low back pain intensity was 2.81±2.73 on the NRS, while after the introduction of remote working, it was 5.53±2.20.</p><p><strong>Conclusions: </strong>There is a relationship between the occurrence of back pain and the change in the nature of work in the group of teachers in connection with the declaration of the COVID-19 pandemic. There has been a statistically significant increase in teachers' back pain intensity since the introduction of distance learning. Depending on the number of hours spent at the computer, the average degree of the declared back pain in the study group increased statistically significantly. There is a need to set the goals of physioprophylaxis and introduce education in the field of ergonomics of computer work in a group of teachers. The National Labor Inspector clearly indicated that refraining from work does not apply to an employee whose employee's duty is to save lives or property. The aim of the article is to analyze the premises of art. 210 of the Labor Code in the context of medical law and professional ethics and to provide the doctrine with an incentive to research on the difficult issue.</p><p><strong>Material and methods: </strong>It uses the method of analyzing the current provisions of labor law and medical law. The jurisprudence of the Supreme Court and the views of the doctrine were analyzed.</p><p><strong>Results: </strong>The employee duty of a doctor and a nurse is always the obligation to \"rescue\" and \"within\" this obligation, medical personnel, unlike \"all employees,\" do not have the relevant right to refrain. In the context of the rules of practicing the medical profession providing for an exception, i.e., the doctor's failure to take or withdraw from treatment of a patient for important reasons, in a situation where there is no urgent case.</p><p><strong>Conclusions: </strong>The starting point is, therefore, the distinction between the provisions of the Labor Code of a general character legis generalis and the provisions of the Act on the profession of doctor and dentist as specific provisions legis specialis. An employed doctor cannot agree to practice in conditions that expose patients to harm. Refraining from work by a doctor as an employee by referring is subject to limitations. The aim of this article is to present, based on the research search (publications from the last year 2020 and from the beginning of the current year 2021), the general overview of the situation during the COVID-19 pandemic with regard to patients and dentist as well as supporting personal protection. Authors identified 7 questions related to this topic. They address among others such issues as identification of the COVID-19 patients coming to the dental practice; protection of both the dentist and the patient from infection and a need for guidelines on the scope of emergency dental services during the pandemic. Opinions on the subject, expressed by the experts in the field, including general guidelines were also taken into consideration. In general, based on the literature overview, the COVID-19 pandemic has completely transformed the healthcare system, including dentistry. The identification of the COVID-19 patients coming to the dental practice has become a necessity (including taking epidemiological survey, temperature measurement). Currently, since \"the group zero\" in significant part has completed the full vaccination process, the health personnel (including dentists) might feel safer. The efficacy of the vaccines is high and provides a certain sense of security. {"title":"Workload, job satisfaction and occupational stress in Polish midwives before and during the COVID-19 pandemic.","authors":"Arkadiusz Mirosław Jasiński, Romuald Derbis, Radosław Walczak","doi":"10.13075/mp.5893.01149","DOIUrl":"https://doi.org/10.13075/mp.5893.01149","url":null,"abstract":"<p><strong>Background: </strong>The aim of this study was to describe, explain, and compare the correlations between workload, job satisfaction, and occupational stress levels in Polish midwives working before and during the COVID-19 pandemic.</p><p><strong>Material and methods: </strong>This study was cross-lagged in nature and conducted in 2 phases at public health facilities. The first phase took place between March and August 2018 before the current pandemic, and the second in February 2021. In total, 133 midwives working before the COVID-19 pandemic and 92 midwives working during the pandemic were surveyed.</p><p><strong>Results: </strong>Analysis revealed that working during the pandemic, personal SARS-CoV-2 infection and workload were positive predictors of occupational stress. Midwives working during the pandemic experienced significantly higher levels of occupational stress compared to the period before the pandemic. Job satisfaction was found to be a negative predictor of stress in both groups, and to mediate the correlation between workload and occupational stress. The positive influence of job satisfaction on coping with stress was stronger in the group working during the pandemic.</p><p><strong>Conclusions: </strong>Workload increased, and job satisfaction decreased, occupational stress levels in both groups of midwives. The COVID-19 pandemic, directly and indirectly, increased midwives' occupational stress levels, and reinforced the negative correlation between workload and job satisfaction. The results confirm the important role of working conditions in shaping occupational stress levels. Deregulation of the immune system as a result of SARS-CoV-2 infection may lead to the impairment of the normal defense pathway in the SARS-CoV-2 positive patients. This scoping mini-review is aimed to critically appraise the existing evidence concerning the potential link between COVID-19 condition and abnormal manifestations within oral cavity, affecting oral mucosa, salivary glands and sensory elements. COVID-induced oral mucosa manifestations, with various clinical aspects, are likely to occur as coinfections and secondary symptoms, as immune system imbalance is <i>per se</i> a causative factor of secondary manifestations. Oral mucosal lesions related to SARS-CoV-2 infection do not seem to demonstrate gender predilection, with the average reported age around 50 years of age. They include mainly white and erythematous plaques, ulcers, blisters, petechiae. The affected intraoral areas were mainly: tongue, palate, lips, gingiva, and buccal mucosa. The \"aggressive\" therapies utilized to treat COVID-19, including drug interactions may aggravate or initiate OMLs pathologies. Neglected oral hygiene or partial or complete abandonment of oral hygiene during intensive hospitalization constitutes an independent factor promoting a wide range of oral pathologies. In addition, stress factor indirectly impairs functioning of the immune system. Oral mucosa lesions occurring in COVID-19 cases present with a wide variation of non-characteristic features. These intraoral, soft tissues abnormalities seem to be reversible and transient. A potential impact of severe oral pathologies to systemic health resulting from SARS-CoV-2 infection, along with associated poor oral hygiene should not be ignored, predominantly in seriously ill patients hospitalized in intensive care units. The value of IFR depends on the number of COVID-19 deaths in a population in a given period and the number of infected people in this population, usually provided by seropepidemiological studies (anti-SARS-CoV-2 IgG in the case of COVID-19). The objective of our study was to estimate IFR in the course of COVID-19 pandemic in 2020, in the general population of Upper Silesia Metropolitan Area (USMA).</p><p><strong>Material and methods: </strong>The seroepidemiological study was conducted in October-November 2020. Among randomly selected inhabitants of Katowice, Gliwice, and Sosnowiec (N = 1167), the presence of SARS-CoV-2 virus infection was assessed based on a positive IgG test result performed with the ELISA method. Data on deaths due to COVID-19 were obtained from the Registry Offices of each city. The infection fatality rate was calculated using the formula IFR (%) = [number of deaths/number of infected] × 100.</p><p><strong>Results: </strong>Results of our study showed the prevalence of infection at 11.4% (95% CI: 9.5-13.2). In three examined towns, in the period January-November 2020, there was a total of 516 COVID-19 deaths. The resulting crude IFR was 0.65% (95% CI: 0.56-0.78). The IgG test had 88% sensitivity and 99% specificity and these figures were used to adjust IFR. The adjusted IFR value was similar to the crude value: IFR = 0.62% (95% CI: 0.53-0.74).</p><p><strong>Conclusions: </strong>The value of IFR estimated for the USMA population was similar to average values obtained in other countries and can be used as the background for monitoring the course and impact of COVID-19 pandemic in the Upper Silesian Industrial Area. The aim of this narrative reviews to evaluate the frequency of occurrence of 3 symptoms often observed in Long-COVID, i.e., chronic fatigue, shortness of breath and cough, and whether comorbidities such as diabetes and arterial hypertension increase the risk of complications after a history of SARS‑ CoV‑2 infection. The method of narrative review was used in this paper. PubMed (May 31, 2021) search was performed to retrieve articles concerning the occurrence of long COVID-19 chronic fatigue, dyspnoea and chronic cough. Studies in which the observation period was <30 days and the average age of subjects exceeded 60 years, as well as studies with no information on the methodology used, in particular without the method of recruiting people for the study, were excluded. Populations with a high frequency of diabetes were defined as the prevalence >10%, and in the case of arterial hypertension >40%. The average frequency of diabetes <10%, hypertension 40%. It can be concluded that in the period of >30 days after discharge from the hospital, in populations with a high incidence of diabetes and hypertension, the incidence of chronic fatigue and cough was higher than in the other analyzed groups. Symptoms of dyspnea were most frequently reported in populations with high rates of diabetes, but at the same time in the average percentage of people with arterial hypertension. Persistent symptoms specific to \"Long-COVID\" can significantly reduce the ability to perform work. In this situation, check-ups performed before returning to work after long-term leave tape on a new dimension. In this situation, authors were particularly concerned with the extent to which job insecurity (both quantitative and qualitative) contributed to the deterioration of workers' well-being.</p><p><strong>Material and methods: </strong>The study was carried out on 382 Polish employees in April and May 2020. The following research tools were used: the <i>Job Insecurity in Pandemic Scale</i> by Chirkowska-Smolak and Czumak and the <i>Depression</i>, <i>Anxiety</i>, <i>Stress Scale</i> (DASS-21) by Levibond and Levibond.</p><p><strong>Results: </strong>The quantitative and qualitative job insecurity were significant predictors of depression and stress, but they did not explain anxiety symptoms. The scope of the explained variance of these negative emotional states by concerns related to work and employment was not large (from 11% to 17.6%). The moderating role of perceived employability was confirmed only in the case of the relationship between qualitative job insecurity and depression, as well as quantitative job insecurity and stress. However, the increase in the explained variance was very small.</p><p><strong>Conclusions: </strong>Uncertainty related to the maintenance of employment and concerns about the deterioration of working conditions due to the COVID-19 pandemic had an impact on emotional disturbances of employees, but they only explained some of the variance of depression and stress and did not affect the perceived level of anxiety. The smaller role of economic stress in the emergence of negative emotional states could be associated with the occurrence of much more serious threats to the health and life of the population in this period. The very low level of unemployment in Poland, which remained at a similar level throughout the pandemic despite the catastrophic forecasts of economists, could also have played an important role. This study aimed to identify PAL changes and examine the age-specific determinants of PAL in younger and older adolescents during the COVID-19 lockdown.</p><p><strong>Material and methods: </strong>This study included 859 high-school students from Bosnia and Herzegovina stratified into 2 age groups: younger (N = 420, 14-16 years of age), and older adolescents (N = 439, 16-18 years of age). Participants were tested over 2 testing waves: before the COVID-19 lockdown (January 2020) and during the COVID-19 lockdown (April 2020). Variables included PAL assessed by the Physical Activity Questionnaire for Adolescents, sports factors, and parental factors.</p><p><strong>Results: </strong>The PAL significantly declined as a result of lockdown measures in the total sample (from 2.76±0.79 to 2.50±0.82). Larger absolute and relative decline of PAL was evidenced in younger adolescents. Sport participation positively influenced PAL before lockdown, with no significant influence during the lockdown. Older adolescents whose mothers were better educated were less likely to be in high risk group with regard to a large decline of PAL as a result of COVID lockdown (OR = 0.50, 95% CI: 0.21-0.84).</p><p><strong>Conclusions: </strong>Results of the study suggest that parental education influences health-related behaviors and that parental education is a protective factor against a decrease in PAL during the COVID-19 pandemic. Main educational agents (i.e., school and parents) should pay more attention to provide children and adolescents adequate information and develop their health literacy, which will hopefully positively impact children's PAL even in challenging situations similar to COVID-19 lockdown.
