We hypothesized that combining these surgical procedures might offer a better quality of life without compromising on surgical outcomes.\u0000\u0000\u0000SUBJECTS & METHODS\u0000A prospective cohort study was conducted at our centre including all adult patients who reported to us for the correction of secondary cleft deformities. All patients with secondary deformities associated with cleft lip palate and alveolus were enrolled in the study. Patients with medical issues who are not fit for general anesthesia, Bilateral Cleft deformities, and patients with gross skeletal deformity warranting surgical correction by Le fort I distraction were excluded from the study. All the patients who were included in the study underwent simultaneous lip revision, rhinoplasty and cleft alveolar bone grafting. The outcome Lip nose profile was assessed using standardised patient photographs with Ascher Mcdade scale and Aesthetic outcome index by 2 blinded maxillofacial surgeons and the bone grafting was assessed using Berglands grading system.\u0000\u0000\u0000RESULTS\u000028 patients were enrolled in the study, 10 patients were excluded based on the exclusion criteria. Thus 18 patients planned for synchronous correction of unilateral secondary cleft deformities were included in the study The Ascher Mcdade score improved from 15 (13- 16), to 4 ( 4- 5) in the post op. All the patients were totally satisfied with the surgical outcome. The aesthetic index showed excellent outcomes in all the lip-nose procedures, except for one case which showed a good outcome. The Bergland scale showed normal bone height in all cases of alveolar bone grafting, except for one case, which showed less than 3/4th fill of bone. Twelve teeth in each group was treated etch rinse, self-etch or selective-etch (n=8) protocols with respective adhesive system. Bucco-palatal cusp measurements were performed before and after preparation, and 180s after curing and then subjected to 5000 thermocyclws. After immersed in 0.2% basic fuchsin dye for 24h, cervical micro-leakage was evaluated. The data was analyzed by One-way ANOVA and post-hoc Tukey's honest Test (p=0.05).\u0000\u0000\u0000RESULTS\u0000All groups and bonding protocols revealed no significant differences in total cuspal deflection measurement and micro-leakage scores. After preparation, a significant increase was found in bucco-palatal width of the teeth compared to baseline (p=0.002). One of the techniques most used in this type of surgery is the sagittal osteotomy of the mandible. This technique offers many advantages, but within its disadvantages is the inferior alveolar nerve paresthesia. There are various treatments that aim to recovery of the nerve bundle, and one of them is low intensity laser treatment. The aim of this study was to evaluate the effectiveness of low intensity laser therapy in the recovery of neurosensorial tissues after mandibular sagittal osteotomy during orthognathic surgery.</p><p><strong>Methods: </strong>Twelve patients submitted to surgery, using mandibular sagittal osteotomy, were treated unilaterally with low intensity infrared (808 nm, GaAIAs active medium) laser, following the inferior alveolar nerve path. The other part of the mandible was treated by placebo. The parameters used were 100 mW of power, irradiancy of 3.6 W/cm2, 2.8J energy per point, an energy density of 100 J/cm2, 28 seconds at each point with a distance of 1.0 cm between points, two sessions per week with a minimum of 10 sessions, starting 48 hours after surgery. Mechanical evaluation was performed in first, fourth, seventh and tenth session.</p><p><strong>Results: </strong>Significant improvement on the treated side was observed. Immunohistochemistry with anti-CD15 antibody and antifactor VIII antibody; and toluidine blue special stain were employed for the detection of eosinophils, microvessels, and mast cells, respectively.</p><p><strong>Results: </strong>The mean numbers of eosinophils, mast cells, and microvessels per high power field in group 1 and group 2 were 15.37±11.86 and 12.62±14.30, 6.00±4.84 and 4.51±4.51, 13.96±6.25 and 6.62±2.05, respectively. Eosinophil density had a positive correlation with both mast cell and microvessel density. The records of 2195 subjects were analyzed. The data records of subjects with palatally displaced canine (PDC) were compared with the data of randomly selected subjects without PDC. Descriptive statistics, logistic regression model and χ2 test were used.</p><p><strong>Results: </strong>One hundred and one subjects presented at least one PDC; 106 subjects without PDC were randomly selected. From the logistic model, it was found that only gender and persistence of the deciduous canine were significantly associated with PDC. The Chi-square test showed a correlation between the presence of upper lateral incisor anomalies and PDC, otherwise, considering the unilateral impaction, there was no statistically significant difference in the relationship between canine impaction and the presence of the lateral incisor anomalies, on the same or opposite side of the impaction.</p><p><strong>Conclusions: </strong>The female sex and the persistence of maxillary deciduous canines are associated with PDC. {"title":"Etiology and characteristics of halitosis in patients of a halitosis center in Northern Italy.","authors":"Federica Romano,&nbsp;Enrico Pigella,&nbsp;Nicoletta Guzzi,&nbsp;Valeria Manavella,&nbsp;Lorena Campanelli,&nbsp;Mario Aimetti","doi":"10.23736/S0026-4970.19.04186-4","DOIUrl":"https://doi.org/10.23736/S0026-4970.19.04186-4","url":null,"abstract":"<p><strong>Background: </strong>As there are few studies in Europe describing characteristics of breath malodor for large groups of patients, this retrospective study was designed to analyse the etiology of halitosis among patients attending a breath malodor center in Northern Italy.</p><p><strong>Methods: </strong>Clinical records of 547 consecutive patients were reviewed and data on self-perceived halitosis, organoleptic scores, volatile sulphur compound (VSC) levels, and oral health condition were extracted and analysed.</p><p><strong>Results: </strong>The prevalence of intra-oral halitosis was 90.7%. In 21 patients no objective signs of breath malodor could be found. Periodontitis and gingivitis were the main cause of bad breath in 33.9% of subjects and in combination with tongue coating in 55.2%. Only eight subjects have tongue coating as the only cause of halitosis. Ear, nose and throat (ENT)/extra-oral causes were found in 5.2% of the patients. VSC concentrations were lower in the psychogenic halitosis group, whereas no statistically significant differences were detected when comparing intra-oral and extra-oral halitosis except for (CH3)2S.</p><p><strong>Conclusions: </strong>Psychogenic halitosis is a rare condition among subjects complaining of suffering from bad breath. The most prevalent cause of halitosis is intra-oral, in particular a combination of tongue coating and periodontal disease. In Brazil, primary care is the preferred gateway to the Unified Health System (Sistema Único de Saúde [SUS]) and has an important role in the coordination of and responsibility for user's care. Using ImageJ software, 5 isometric and isogridded ROI from each root filling area and isometric ROI from the Al stepwedge were calculated.</p><p><strong>Results: </strong>Evaluation of the total of 120 images displayed that PSP system produced significantly higher contrast resolution (P<0.05) in regard to pixel values than the CMOS. The CMOS system was non- responsive to increasing dose (P=0.000). Regarding the EqAl values, no significant difference was determined between groups (P>0.05).</p><p><strong>Conclusions: </strong>The contrast resolution was higher using the PSP system. Guiraldo","doi":"10.23736/S0026-4970.19.04291-2","DOIUrl":"https://doi.org/10.23736/S0026-4970.19.04291-2","url":null,"abstract":"AIM\u0000The objective was to examine the microshear bond strength values of different dual resin cements to computer-aided design (CAD)/computer-aided manufacturing (CAM)-created ceramics containing lithium disilicate, which underwent different surface treatments.\u0000\u0000\u0000METHODS\u0000Two dual resin cements (RelyX ARC and RelyX Ultimate) and three surface treatments of lithium disilicate ceramic blocks (Ceramic Primer (CP; containing silane) plus Adper Scotchbond Multipurpose (ASM) adhesive; CP plus Single Bond Universal (SBU) adhesive; and SBU adhesive alone) were tested. The SBU adhesive includes silane in its formulation. Each sample underwent a microshear test and bond strength values were subjected to analysis of variance (ANOVA) followed by Tukey's test (α = 0.05). Fracture patterns were also observed with light microscopy.\u0000\u0000\u0000RESULTS\u0000RelyX ARC treated with CP + ASM or CP + SBU presented resistance values that were significantly higher than the same treatments for RelyX Ultimate. Conversely, for the blocks treated with SBU alone, the mean resistance value for RelyX Ultimate was higher than that of RelyX ARC. Regarding the different types of treatment for each resin, the mean resistance value of RelyX ARC treated with CP + ASM was significantly highest, followed by treatment with CP + SBU and SBU treatment alone. Meanwhile, RelyX Ultimate treated with CP + ASM or CP + SBU presented resistance values that were significantly higher than SBU treatment alone. Mast cells are found in tissues with various allergic and inflammatory conditions including periodontal disease through the release of cytokines, chemokines, and proteolytic enzymes. Given the findings of some studies on the related role of mast cells, this study was conducted to determine the number of mast cells in aggressive periodontitis.</p><p><strong>Methods: </strong>This study was conducted on 30 participants divided into two groups visiting the Faculty of Dentistry of University of Medical Sciences. A total of 15 participants with aggressive periodontitis and 15 healthy participants were investigated. For counting mast cells, toluidine blue staining was used. The percentage of mast cells and demographic factors were evaluated in the two groups and the results were analyzed in SPSS using descriptive statistics and independent t-tests.
