{"title":"Masticatory function improvement with the use of mandibular single-implant overdentures in edentulous subjects: a systematic literature review.","authors":"Mayara A Pinheiro,&nbsp;Ingrid A Meira,&nbsp;Marcela B Magno,&nbsp;Lucianne C Maia,&nbsp;Renata C Rodrigues Garcia","doi":"10.23736/S0026-4970.20.04327-7","DOIUrl":"https://doi.org/10.23736/S0026-4970.20.04327-7","url":null,"abstract":"<p><strong>Introduction: </strong>In this systematic review, several masticatory function parameters assessed during mandibular single-implant overdenture (SIO) use were compared to pre- SIO placement values in edentulous patients with aim of contributing to a consensus regarding denture treatment options.</p><p><strong>Evidence acquisition: </strong>This study was registered a priori in the PROSPERO database (CRD42018106567). Two independent reviewers carried out electronic searches in eight databases, without language or time frame limitations, to collate clinical studies comparing masticatory function of edentulous patients before versus after SIO installation with the implant placed on the mandibular midline. Risk of bias was assessed with a before-and-after tool and evidence certainty level was evaluated with the Grading of Recommendations Assessment, Development and Evaluation program.</p><p><strong>Evidence synthesis: </strong>Eleven studies were included in this review (1 prospective, 3 crossover trials, 4 randomized clinical trials, 2 paired clinical trials, and 1 pilot). Enrolled patients were mostly over 60 years old; all patients were using conventional complete dentures (CD) prior to SIO installation. Masticatory performance, masticatory efficiency, bite force, and muscle activity were improved after the SIO placement compared to during mandibular CD use. Mandibular movement and masticatory ability data were inconclusive. Most of the studies had low risk of bias, but all had very low certainly level ratings due to methodological heterogeneity.</p><p><strong>Conclusions: </strong>Placement of SIO improves masticatory function, as reflected mostly by masticatory performance and efficiency data, relative to CD use. They usually are asymptomatic and can be discovered by routine radiographic investigations. This study aims to examine and compare intra- and postoperative consequences associated with use of piezosurgery and conventional rotary instruments for the removal of these oral lesions.</p><p><strong>Methods: </strong>Clinical and histological data of 120 consecutive patients with a mean age of 22 years and 1 month (age range from 19 years and 1 month to 25 years and 3 months) with compound odontomas, diagnosed by biopsy, were assembled. Of a total of 120, 100 oral lesions were placed in the anterior region of the maxilla; of these, 50 were extracted through traditional rotary instruments (GROUP 1) and 50 with piezoelectric surgery (GROUP 2). The remaining 20 oral masses took place in the posterior area of the same bone; of these, 10 were extracted through traditional rotary instruments (GROUP 1a) and 10 with piezosurgery. (GROUP 2b). The piezoelectric cutting device was (Mectron Medical Technology) with MT1-10 insert, pump level 4, vibration level 7. Finally, pain, swelling, and surgical time evaluation were measured 24-48-72 hours after the intervention.</p><p><strong>Results: </strong>As regards the mean time of surgery, in Group (1 and 1a) were 12 minutes, while in Group (2 and 2a) were 18 minutes. Regarding surgery, the osteotomy of the cortical bone was performed in all cases. As regards the measure of bone saved, calculated through a surgery gauge, it was greater of 3.2 mm in Group (2 and 2a) than in Group (1and 1a). The main of facial swelling in both Groups (1 and 1a) was 5.82 mm and 2.23 mm for the Groups (2 and 2a) immediately after surgery.</p><p><strong>Conclusions: </strong>Piezosurgery produced less postoperative discomfort about the swelling. This study aimed to analyze the variables associated with the extracellular matrix in oral pleomorphic adenomas.</p><p><strong>Methods: </strong>This cross-sectional study comparatively evaluated 21 cases of pleomorphic adenomas and 21 sections of regular oral mucosa from the files of an oral pathology department. The tissue sections were subjected to immunohistochemical study with a monoclonal antibody specific to CD31 protein to assess vascular density and were also stained with Sirius red to evaluate the levels of collagen. The numbers of positively-stained cells were enumerated in specific fields through morphometric study, with subsequent statistical analysis.</p><p><strong>Results: </strong>Higher numbers of CD31-positive cells were observed in tumor cells than in cells in the regular oral mucosa (P=0.004), which were always located around blood vessels. In tumor lesions, the correlation between the collagen area and vascular density was discrete between these two variables (r=0.046), although the association was not statistically significant.</p><p><strong>Conclusions: </strong>The results suggested that the increased expression of CD31+ in cells associated with capillaries may be related to tumor neoangiogenesis. The opinion of these future dentists would ultimately will decide the endurance of REPs as routine procedures in endodontic practice. The aim of this study was to perform a survey to identify the knowledge, attitudes, and practices of undergraduate students about regenerative endodontic procedures (REPs).</p><p><strong>Methods: </strong>The questionnaire was obtained after cross-cultural adaptation of a questionnaire previously applied in USA and was applied to two hundred forty-eight undergraduates. Data were statistically analyzed.</p><p><strong>Results: </strong>Most of the students (82.9%) agreed that regenerative therapy should be incorporated to dentistry and 87.5% of them believed that stem cells banks would be useful for the tissue regeneration. Most participants (58.1%) would like to obtain an internship/tutoring that addresses REPs and 80.8% of participants think that the major obstacle to a patient accepting a REP was the expected high cost of the treatment. The freshmen students were more optimistic about offering stem cell treatments to their patients (P≤0.05).</p><p><strong>Conclusions: </strong>The undergraduates were very optimistic about the future of REPs, stem cell banking, and tissue engineering. Soft tissue calcifications found in the mandibular angle area were recorded according to gender, age, and bilaterally. Patient medical records were reviewed for atherogenic risk factors including hypertension, diabetes, dyslipidemia, chronic renal disease, menopause, drug usage and any other systemic condition. Data were analyzed with the χ<sup>2</sup> test and Fisher's Exact test; P<0.05 was considered statistically significant.</p><p><strong>Results: </strong>Among the 814 patients, a total of 74 (9.1%) calcified carotid artery atheromas (CCAA) were identified, of which 34 (4.17%), 40 (3.19%), and 14 (1.7%) were right, left, and bilateral, respectively. Orthopantomographic examination revealed triticeous cartilage (7.3%) in the cervical bifurcation region, asymptomatic styloid process elongation 192 (23.5%), tonsilloliths 27 (3.3%), sialoliths 37 (4.5%), calcified lymph nodes 46 (5.7%), impacted roots 109 (13.4%), and foreign radiopaque materials 23 (%2.8). There was a significant age and gender difference between type II diabetes and CCAA. A positive correlation was found between styloid process elongation and temporomandibular joint disorders and antidepressant use.</p><p><strong>Conclusions: </strong>Soft tissue radiopacities can be a validated risk indicator of cardiovascular events or misdiagnosed pain. Friction between brackets and mucosa is a kind of chronic trauma that may cause buccal mucosal proliferation and ulceration and is one of the most common complaints of patients during orthodontic treatment. The aim of this study was to evaluate the changes of the lower lip epithelial cells after the removal of orthodontic fixed appliances.</p><p><strong>Methods: </strong>Cells of lower lip oral mucosa of 31 orthodontic patients were collected by exfoliative cytology in three times: immediately (T0), 30(T1) and 60 (T2) days after debonding. Nuclear (NA) and cytoplasmic (CA) areas, NA/CA ratio, cell morphology and cellularity of smears were evaluated using cytomorphometric and cytomorphologic methods. Friedman test followed by the Wilcoxon test was used to compare the NA, CA and NA/CA values in three different times. Cell morphology and cellularity were analyzed by the Chi-square test. All statistical tests were performed with SPSS version 20.</p><p><strong>Results: </strong>The NA and CA values increased significantly (P<0.001) after the removal of friction and irritation of orthodontic appliances in T1 and T2 and reached their highest level in T2. The NA/CA values did not change significantly (P=0.25). Class I smears, according to Papanicolaou classification, were observed in T0, T1, and T2. Superficial epithelial cells were predominant cell types in the lower incisor region in all times without statistically significant difference in three intervals.</p><p><strong>Conclusions: </strong>Orthodontic appliances increase keratinization of lip mucosa which is considered an adaptive reaction to the mechanical stimulus, but after removing of the appliances cellular activity rapidly increases. The development CAOT offers solutions to overcome many of limitations in adult orthodontic treatment, such as risk of hyalinization, root resorption, periodontal problems, lack of bone growth and a long therapy time due to slower tooth movement compared to adolescents. The purpose of this systematic review was to identify if CAOT is also useful for expanding narrow arches in adult orthodontic patients.\u0000\u0000\u0000MATERIALS AND METHODS\u0000An electronic research on PubMed until June 2020 was made with the combined search terms \"corticotomy expansion\", \"corticotomy orthodontic expansion\" and \"corticotomy orthodontic\".\u0000\u0000\u0000RESULTS\u0000Six studies were included in this systematic review. The authors found discordant results in literature, however it seems that CAOT can have better results than conventional expansion, but it cannot be considered as an alternative therapy for severe posterior cross-bite. CAOT have a greater predictability if done with bone grafting.\u0000\u0000\u0000CONCLUSIONS\u0000The evidence of corticotomy as a help to palatal expansion in adult orthodontic treatment has not been well described in literature and only few published reports are available. Many studies confirm that this condition is almost frequent in general population; the prevalence reported is between 2.7% and 11.3%. This condition shows a multifactorial etiology: genetic factors, hereditary factors, environmental factors, local or general infective processes, radiations, drugs and traumas. The aim of this observational study was to evaluate the prevalence of tooth agenesis in a sample of orthodontic healthy patients considering each Angle malocclusion.\u0000\u0000\u0000METHODS\u0000900 patients from a University clinic and three private practices were analyzed through intra and extra oral photographs, orthodontic study casts, panoramic radiographs and lateral cephalograms. The following inclusion criteria were applied: non-syndromic patients, without previous orthodontic, prosthetic treatment or extractions. Patients' clinical data consisted in general and dental anamnesis, intra and extra oral photographs, orthodontic study casts, digital panoramic radiographs and lateral cephalograms. The whole sample was divided into four groups (A-B-C-D) in relation with Angle's malocclusion and dental agenesis registered.\u0000\u0000\u0000RESULTS\u0000The whole sample presented a mean prevalence of the condition of 8.80%. Class II/2 is the most affected malocclusion with 20.37% of the patients affected by the condition, followed by class III (11.19%), class II/2 (8.05%) and class I (6.62%). The Chi-square test demonstrated a statistically significant different distribution of the condition between the four groups (p-value = 0.0059). The four groups presented also different patterns of missing teeth.\u0000\u0000\u0000CONCLUSIONS\u0000We found a significantly different distribution of the condition in the different malocclusions with class II/2 presenting the higher prevalence of tooth agenesis (20.37%). This study aims to eveluate the heating and osteotomy speed in bone blocks of ox's shins, to report the best way of its use in the clinical practice.\u0000\u0000\u0000METHODS\u0000A bone blocks had the dimensions as follow: 20mm lenght, 10 mm width, and 5 mm wide. It was evaluated 5 different groups: Group LM - Low speed and medium pressure; Group HM - High speed and medium pressure; Group HH - High speed and High pressure; Group LH - Low speed and high pressure; Group LL - Low speed and low pressure. The heating increasement was measured with a thermal viewer and the osteotomy was timed when the cut depth reached 5 mm and the whole block detached itself. One-way ANOVA and Tukey tests were adopted to analyze the data and the level of significance was set at a p-value of 0.05.\u0000\u0000\u0000RESULTS\u0000The pressure and speed of the tip, works directly in the generated temperature during osteotomy. Each group was then divided into three subgroups (n=20) according to the polymerization mode (A; low-intensity, B; high-intensity, and C; soft-start). Subgroups were then divided into two further groups according to the polymerization time (I; 10 s and II; 20 s). DC of light-cured resin cement beneath different kinds of ceramics was tested using FTIR spectroscopy. Results were compared to a control group cured without overlying ceramic.\u0000\u0000\u0000RESULTS\u0000While the type of ceramic and mode of polymerization showed a significant effect on the DC of resin cement, polymerization duration did not. Vita Mark II group showed significantly the highest DC of resin cement, followed by Vita Enamic and Cerasmart.
