Regions around the world are still at high-risk, mostly in populations with a high prevalence of Helicobacter pylori infection or a carcinogenic favorable diet. Gastric cardia cancer incidence is on the rise in some areas. Great steps were made in the last decades in understanding the pathogenesis of gastric cancer and its risk factors. Host genetic polymorphisms play a quintessential role in disease outcome. Helicobacter pylori eradication and endoscopic surveillance are the most effective options to further decrease gastric cancer incidence. Surgery is required for a curative treatment in most cases. We present the first report of a recurrent submandibular fistula in a patient treated with sunitinib. A 68-year-old man was referred to our clinic for a cutaneous fistula situated on the right side of his lower jaw. The patient had been diagnosed with bone metastases from a renal carcinoma 2 years ago and had received a single 4 mg dose of zoledronic acid and subsequent treatment with sunitinib 37.5 mg once daily for the past 4 weeks. The patient was treated surgically by a perilesional incision and primary closure with sutures, advised on meticulous oral hygiene and was kept on an antimicrobial agent in the form of clindamycin. After reinitiating his oncological treatment with sunitinib the cutaneous fistula re-appeared and bone abnormalities were also detected on his X-ray. After 18 months a contralateral cutaneous fistula was observed along with a spontaneous avulsion of the patient's left molar. Epigenetic alterations in oral and maxillofacial area cancers are urgently needed to be investigated, as the observed changes might have crucial diagnostic value for personalized medicine.</p><p><strong>Methods: </strong>Our study aimed to identify the most frequently hypermethylated tumor suppressor gene promoters in OSCC, followed by correlation analysis with the patients' survival. We evaluated the methylation status of the promoters in a panel of 22 tumor suppressor genes in Romanian (n=9) and Bulgarian (n=12) patient groups suffering from oral and maxillofacial area cancers. The extracted DNA was further digested through EpiTect Methyl II PCR Array System containing methylation-sensitive and methylation-dependent restriction enzymes, followed by specific amplification of the products obtained by qPCR and data analysis using the online platform provided by the producer.</p><p><strong>Results: </strong>Different methylation patterns were observed in the tumor suppressor genes' promoters. Among them, the methylation profile of Cccnd2, Chd1, Cdh13, Cdkn1c, Neurog1, Gstp1, and Runx3 genes further correlated with overall survival rates.</p><p><strong>Conclusions: </strong>Our data emphasize that epigenetic alterations are responsible for the clinical heterogeneity of oral and maxillofacial area cancers and significantly impact on patient survival. As more experience has been gained over the past decades with laparoscopic techniques, they are now widely used also for the repair of primary and unilateral inguinal hernias, representing a safe and effective alternative. One of the major concerns of patients undergoing inguinal hernia repair is postoperative pain and socio-professional reintegration.</p><p><strong>Aim of study: </strong>This study started from the hypothesis that the learning curve could influence postoperative pain intensity after laparoscopic inguinal hernioplasty.</p><p><strong>Methods: </strong>A retrospective - comparative study was performed, including a general surgeon's first consecutive cases (n=87) of TAPP (transabdominal preperitoneal procedure) hernioplasty procedures with implantation of self-gripping surgical prosthesis were investigated.</p><p><strong>Results: </strong>The evaluation of clinical and surgical aspects resulted in similar values in case of the studied groups. A reduction in surgical time was observed in case of patients operated after completing the learning curve (p = 0.0005) On the first postoperative day patients complained mostly about persistent and severe type of pain. Average Pain Index calculated with help of Simple Numeric Pain Scale resulted in similar values. Length of analgesic treatment showed no significant differences. The best instrument for this purpose is the microscope, which has proven to enhance quality, longevity and outcome of clinical work. Its use in some dental specialties (such as endodontics) is now well established, but there is also a role for this equipment in other branches of dentistry. To anticipate the contribution of magnification to dentistry in general, recent research and experimental data on the importance of magnification devices will be considered, from the perspectives of optical issues, diagnosing methods and treatment options. It will be shown that, while the microscope is vital to certain specialties (such as endodontics, periodontics, restorative dentistry and prosthodontics), it has little effect on others (orthodontics, for example). Nevertheless, the evolution of the donating activity and of organ transplantation in Romania has been confronted with a certain inconsistency and a lack of resources necessary to the development of the system.</p><p><strong>Method: </strong>The aim of this study is to analyze the dynamics of the transplantation activity in Romania between 2000 and 2020. The study was accomplished through the analysis of available data corresponding to the above-mentioned period, which were published in the database of the National Transplant Agency, Eurostat and the Global Observatory on Donation and Transplantation. The data were processed using the MedCalc Statistical Software, version 14.8.1 (MedCalc Software bvba, Ostend, Belgium; http://www.medcalc.org; 2014).</p><p><strong>Results: </strong>The activity of donation and organ transplantation in Romania has been maintained at a low level, with a rate of 3.44 donators pmp and a transplantation rate of 12.55 pmp, as reported for the year 2020. Romania remains at a transplantation rate of under 6.6 pmp, despite the considerable increase in the number of patients on the waiting lists, a fact which describes the picture of a relatively weak system, incapable of providing surgical interventions to cover the minimum needs that emerge within a calendar year.</p><p><strong>Conclusions: </strong>Our study points to the fact that the transplantation system in Romania is confronted with a major deadlock. Romania holds the last-but-one place in the ranking of countries in the European Union on transplant activity. The objective of this review is to determine the anxiety and depression prevalence among healthcare workers during the pandemic period.</p><p><strong>Methods: </strong>We searched systematically the literature in five electronic databases such as PubMed, CINAHL, ScienceDirect, MEDLINE, and Cochrane COVID-19 study register. The last online research was performed in May 2022. We included only cross-sectional studies and performed a meta-analysis of pooled prevalence. Publication bias was assessed with a funnel plot and Egger's and Begg's tests. A random effect was applied and heterogenicity I2 was calculated. Quality assessment of included studies was performed using the Joanna Briggs Institute tool.</p><p><strong>Results: </strong>In this review, we included 14 cross-sectional studies comprising 7780 healthcare workers. Participants were from the whole spectrum of healthcare workers. The pooled prevalence of depression was 33.8% (95% CI: 24.6 - 43.6), heterogenicity I<sup>2</sup>: 98.69%. The pooled prevalence of anxiety was 41.3% (95% CI: 30.2 - 52.9), heterogenicity I<sup>2</sup>: 99.01%.</p><p><strong>Conclusion: </strong>One-third of healthcare workers suffered from depression, and more than one-third suffered from anxiety during the COVID-19 pandemic. A provisional dental prosthesis should protect the abutment from pulpal and gingival aggressions, adapt correctly to keep healthy gingival tissues, be durable, and have a low price. The purpose of this in vitro study was to compare the marginal adaptation of different types of provisional fixed dental prostheses (PFDP), fabricated using 3D printing technology versus the milling (computer-aided manufacturing [CAM]) technique.</p><p><strong>Method: </strong>Two resin teeth (second premolar and second molar) on a typodont were prepared for three-unit provisional fixed dental prostheses. Thirty models were 3D-printed after a digital model was created using an intraoral scanner. Then, 30 provisional fixed dental prostheses (PFDPs) were made from a variety of materials using a digital design of a 3-unit PFDP and STL files delivered to a milling machine and a 3D printer, respectively. Ten PFDP were milled (CAM), and two sets of ten each, were fabricated with 3D printing technology (stereolithography), using two different materials. Several indices including the total psoas area index (TPAI) are proposed for the diagnosis of this condition, but there is no validated cut-off point.Our study aimed to assess the role of TPAI as a marker for sarcopenia and to compare the utility of previously proposed cut-off values for predicting post-operative complications in patients with digestive cancers undergoing surgery.</p><p><strong>Methods: </strong>We retrospectively included all adult patients with digestive cancers admitted to a tertiary center for elective surgery between January and December 2019. Sarcopenia was considered based on TPAI evaluated on abdominal computed tomography (CT) and for analysis we used different cut-off points published by various authors. The primary endpoint was the occurrence of any complications as defined by the Clavien-Dindo classification. The secondary endpoints were fistula development, low- versus high-grade Clavien-Dindo post-operative complications, moderate or severe anemia at discharge, major bleeding, hypoalbuminemia at discharge, and decrease in albumin levels by at least 1g/dL.</p><p><strong>Results: </strong>We included 155 patients with a mean age of 64.78 ± 11.40 years, of which 59.35% were males; 58.06% developed postoperative complications. TPAI evaluated as a continuous variable was not a predictor for the development of post-operative complications neither in the general study sample, nor in the gender subgroups of patients.
