The aim of the present study was to measure the levels of (lead and cadmium) in infertile male and correlate these levels with male fertility. After free informed consent, the present study included 70 infertile couples (without female infertility factor) attending the infertility clinics (Dermatology and Venereology Department; Al-Azhar Faculty of Medicine; New Damietta); who were well-known to be exposed to lead and/or cadmium confirmed by measuring blood lead and cadmium levels, and another 30 healthy fertile males without evidence of environmental exposure were included as controls. All patients underwent history taking, clinical and andrological examination and laboratory investigations (complete blood count, blood glucose level, renal and liver function tests and hormonal profile (serum FSH, LH, estrogen and testosterone) with measurement of lead and cadmium levels using graphite furnace atomic absorption spectrophotometry. Finally, a sample of semen was obtained for semen analysis. The percentage of poisoning is low, but the mortality is high and no effective antidote available. The objective of this study was to find out predictors of mortality for patients with acute aluminium phosphide poisoning. All patients with acute aluminium phosphide poisoning admitted to Poisoning Control Center, Ain Shams University Hospital between the years 2015 to 2017 were prospectively studied and compared between survival and non survivor patients. Data collected include demographic data, clinical manifestations, laboratory parameters, ECG and treatment offered to the patients. A total 31 patients were enrolled comprising 20 males and 11 females, 84% were suicide and mortality rate was 35%. Shock and cardiac arrhythmia were observed in 52% and39% respectively, while 26% presented with coma. Abnormal blood sugar and metabolic acidosis were found in 19% and 45% respectively. Fifty two percent of the patients needed inotropic therapy and 32% received N-acetylcysteine (NAC). Risk factors increasing mortality were found as shock, tachycardia, coma, metabolic acidosis, hyperglycemia, cardiac arrhythmia and the need for inotropic drug therapy. The study concluded that aluminum phosphide is a low-cost highly-toxic rodenticide. The circulatory collapse, metabolic acidosis and cardiac arrhythmia are the major causes of death. The role of NAC must be reassessed in larger scale. However, the actual incidence and related factors are not well-studied. The purpose of this work was to investigate the relationship between the incidence of seizures and plasma tramadol level in patients with tramadol-inducing seizures. In addition, we reported the pattern of tramadol overdoses in selected patients. The study was conducted from 1st January 2017 to 28th February 2018. It involved 102 cases who recruited from toxicological units of AlAzhar University Hospital (New Damietta). After free informed consent to contribute in this study and within one hour after arrival to a toxicological unit, 10 mL of blood were separated and the serum was used for extraction of tramadol residues by using a highperformance liquid chromatography (HPLC) and other blood samples were stored at _ 8oC for the following assays to all patients; fasting blood sugar, hemoglobin levels, serum urea, serum creatinine and liver enzymes. Then clinical examination to all patients included vital signs with a special attention to central nervous system (CNS) manifestations, gastrointestinal (GIT) manifestation. The frequency of seizures (27.4%) with acute tramadol poisoning in humans is not dose dependent and seizures occurred with therapeutic doses and there were no relation between blood tramadol level and seizures. We concluded that risk of seizures increase in cases took other drugs with tramadol as alcohol and heroin.
