Pak","doi":"10.26779/2522-1396.2022.11-12.10","DOIUrl":"https://doi.org/10.26779/2522-1396.2022.11-12.10","url":null,"abstract":"Objective.To determine the influence of the genes polymorphism of proinflammatory – interleukin–8 A–251T (rs 4073) and antiinflammatory – interleukin–10 C–592A (rs 1800872) cytokines on course of an acute pancreatitis. \u0000Materials and methods. The genes polymorphism of proinflammatory – interleukin–8 A–251T (rs 4073) and antiinflammatory – interleukin–10 C–592A (rs 1800872) cytokines were studied in 83 patients with an acute pancreatitis, using the method of polymerase chain reaction (PCR). The control group have consisted of 60 healthy persons without an acute pancreatitis in anamnesis. \u0000Results. The rate of the genes polymorphism of interleukin–8 A–251T (rs 4073) and interleukin–10 C–592A (rs 1800872) was determined in oedematious and destructive forms of an acute pancreatitis, as well as in patients operated on. \u0000Conclusion. The acute pancreatitis transition into necrotic form was not connected with the kind of the genes polymorphism of proinflammatory – interleukin–8 A–251T (rs 4073) and antiinflammatory – interleukin–10 C–592A (rs 1800872) cytokines. Operative interventions for an acute pancreatitis complications were performed statistically significantlymore frequently in carriers of the T/T gene genotype of interleukin–8 A–251T (rs 4073). To study the impact of compression on velocity of venous and arterial main blood flow of the lower extremity, as well as cutaneous microcirculation in the back part of the foot in healthy individuals and patients with decompensated forms of varicose disease and postthrombophlebitis syndrome. \u0000Materials and methods. In the investigation 56 individuals took part and divided into three groups: Group I – 20 healthy persons; Group II – 15 patients with varicose disease in decompensated stage; Group III– 21 patients with decompensated stage of postthrombophlebitis syndrome. \u0000In all participants of the investigation the index of ankle–brachial pressure, deep–femoro–popliteal index, the regional perfusion index, transcutaneous partial pressure of oxygen and partial pressure of carbon dioxide, the arterial blood flow velocity in femoral artery and of venous blood flow distally from sapheno–femoral junction were measured before and after application of elastic medical knitwear of various Class of compression or the cuff pressure. \u0000Results. In the Class III compression in patients of Group III the transcutaneously registered indices crossing have occurred between partial pressure of oxygen and carbon dioxide, accompanied by domination of the carbon dioxide partial pressure over the oxygen partial pressure while further enhancement of the compression Class. In patients of Group II this tendency was observed while application of Class IV compression only. At the investigation beginning the values of partial pressure of carbon dioxide registered were higher in the Group III patients, than in the patients of Group II (p=0.0001). \u0000Conclusion. While application of the Class III compression the velocity of the hip venous blood flow, comparing with its initial values, have lowered at average by 78% in patients of Group II and at average in 7.4 times in the patients of |Group III (p=0.0001). To estimate the structure of complicated forms of colorectal cancer and surgical tactics of treatment in patients with this pathology in conditions of general surgery stationary. \u0000Materials and methods. Into the investigation 71 patients were included, treated for complicated forms of colorectal cancer. \u0000Results. Primary radical operative interventions were performed in 48 (67.61%) patients, palliative – 18 (25.35%), and symptomatic – 5 (7.04%). \u0000Conclusion. To improve intraoperative determination of «sentinel» lymph nodes in patients, suffering mammary gland cancer. \u0000Materials and methods. In the 2016 – 2021 yrs period on the base of the Odessa Regional Clinical Hospital 200 patients, suffering mammary gland cancer Stages Т1–Т3N0М0, were operated, using two dyes: the blue patented dye and fluorescent dye indocyanine green. All the patients were distributed into two groups. In Group I in 100 patients the \"sentinel\" lymph nodes biopsy was conducted. The blue patented dye was used for the lymph nodes staining. The same was conducted in 100 patients of Group II with the \"sentinel\" lymph nodes biopsy. For the lymph nodes staining there were applied the blue patented dye and fluorescent dye indocyanine green, which were introduced intravenously into the upper extremity on the side of the affected mammary gland along outflow from the upper extremity to mammary gland. \u0000Results. General five–year survival after axillary lympho–dissection and after biopsy of «sentinel» lymph nodes have constituted 91 and 92%, accordingly, while recurrence–free five–year survival – 82.2 and 83.9% accordingly. Only in 1.1% patient a regional recurrence in «sentinel» lymph nodes on the affected mammary gland side was revealed. In 57% patients the unaffected «sentinel» lymph nodes were diagnosed, that's why further lympho–dissection was accomplished. In 43% women–patients there was revealed metastatic affection of the lymph nodes. Recurrence was registered in 0.2% patients as an isolated metastases in axillary lymph nodes. \u0000Conclusion. To determine the possibility and expediency of application of ferromagnetic instruments for diagnosis and pulling out of the abdominal cavity foreign bodies in the battle gun–shot trauma, including application of miniinvasive technologies. \u0000Materials and methods. In the investigation 154 patients took part, including 110 (71.4%) with application of open laparotomy access, аnd 44 (28.6%) – using laparoscopic technologies. While doing open laparotomy interventions magnet surgical instruments were applied. \u0000Results. The magnet instruments were used in 2.5 times more frequently in open laparotomy accesses, than in the laparoscopic.There was established, that the average duration of search and pulling out of foreign bodies without application of special instruments have constituted (21 ± 2.2) min, while with it – (10 ± 1.2) min only, what is in 2 times lesser (p˂0.05). When special instruments were used, 98% of foreign ferromagnetic bodies were pulled out from abdominal cavity, while without it – 34% only, causing necessity to perform the staged interventions for their pulling out. \u0000Conclusion. Practical introduction of the magnet instruments proposed permits to reveal the majority of the ferromagnetic foreign body quantity with lesser time loss for their search, localization and pulling out, what is very important in conditions of military–field surgery.
