The operation material (soft tissues of the face and scalp) was investigated in 20 patients for studying of morphological characteristics of venous dysplasia. In the histological investigation frame the material was fixed and processed in accordance to conventional method, the tissue slices were colored with hematoxilin and eosin. In 7 observations immuno–histochemical investigation with CD34 antibodies (clone QBEND10) and S100 (polyclonal) was applied. \u0000Results. Saliutin","doi":"10.26779/2522-1396.2022.11-12.05","DOIUrl":"https://doi.org/10.26779/2522-1396.2022.11-12.05","url":null,"abstract":"Objective.To determine the expediency of the spleen–preserving operations performance for its injury due to gun–shot thoraco–abdominal and abdominal woundings. \u0000Materials and methods. Of 51wounded persons with the gun–shot penetrating abdominal woundings with splenic injuries 8 (15.7%) were operated on the II level of the medical help delivery, while on the ІІІ level – 43 (84.3%). Of 43 wounded persons, operated on the ІІІ level of the medical help delivery, in 35 (81.4%) splenectomy was performed, while in 8 (18.6%) – the organ–preserving operations. All the splenic injuries have been characterized as a shrapnel woundings. The splenic damage degree was estimated in accordance to Moore, taking into account its structure. In 5 (62.5%) wounded persons the open laparotomy organ–preserving operations were performed, and while using video–laparoscopic system– in 3 (37.5%). \u0000Results. Ultrasonographic investigation in accordance to the FAST–protocol was done in all the wounded persons, what have permitted to determine the blood abdominal outpouring volume as well as the splenic injury presence. The affection volume and further tactics were determined using video–laparoscopy. The Stage I splenic affection in accordance to Moore was diagnosed in 4 (50%) wounded persons, the Stage ІІ – in 3 (37.5%), and the Stage ІІІ – in 1 (12.5%). In all the patients, in whom organ–preserving interventions were performed, the surgical help volume consisted of coagulation, using the radiofrequency therapy apparatus MEDTRONICAEX and the hemostatic sponge application with drainage of the left subdiaphragmatic space, using silicone tube. \u0000Conclusion. Degtyarenko","doi":"10.26779/2522-1396.2022.11-12.23","DOIUrl":"https://doi.org/10.26779/2522-1396.2022.11-12.23","url":null,"abstract":"Objective.To improve the diagnostic biopsy of sentinel lymph nodes in patients, suffering mammary gland cancer, using fluorescent lymphography, and to reduce the quantity of postoperative complications. \u0000Materials and methods. In 2016 – 2021yrs period on the base of Odessa Regional Clinical Hospital 400 patients, suffering mammary gland cancer Stages Т1–Т3N0М0, were operated on with application of fluorescent lymphography, using two dyers – the patent blue and indocyanine green. All the patients were distributed into two groups. In Group I (a control one) in 200 patients the sentinel lymph nodes biopsy was performed. The dyer patent blue was applied for the lymph nodes coloring.In Group II (the main) in 200 women–patients the sentinel lymph nodes biopsy, using the patent blue and fluorescent dyer indocyanine green, was performed. Both dyers were injected subdermally along the external areolar edge in the dose 2 ml of the dyer solution. \u0000Results. Total 5–year survival after axillary lymphodissection and after the sentinel lymph node biopsy have constituted 91and 92%, accordingly, while the recurrence–free 5–year survival – 82.2 and 83.9%, accordingly. Regional recurrence in sentinel lymph nodes on the affected mammary gland side was diagnosed in 1.1% of the women–patients. In 58% of the women–patients the sentinel lymph nodes were clear, and that’s why a subsequent lymphodissection was not indicated to perform. Metastatic lymph nodes were revealed in 42% of the women–patients. \u0000Conclusion. Retrospective analysis of the data obtained was conducted in 1159 injured persons, who were traumatized as a consequence of a traffic accident in megapolis. \u0000Results.There was established, that as a result of traffic accident in the megapolis environment, the injured persons most frequently obtained trauma of the head – 68.31%, of thoracic cage – 57.07%, and of the extremities – 53.99%. The majority of the injured persons survived have had the trauma of the head (67.13%), of the thoracic cage (55.74%) and extremities (53.63%), while mostly rare – pelvic trauma (15.70%). Among the injured persons, who died, the head injuries was noted in 94.0%, thoracic trauma – in 86.0%, and abdominal injuries – in 76.0%. \u0000Conclusion.Trauma as a consequence of traffic accident in the megapolis environment constitutes a multicomponent injury (2.4 damages in every injured person). {"title":"Experience of the trauma operative treatment of the lower extremities main vessels in the work injuries","authors":"P. Nikulnikov, V. Pichka, O. The structure of combined osseous–vascular work injury of the lower extremities was studied, and the results of its operative treatment analyzed retrospectively. \u0000Materials and methods. The data concerning 147 patients with combined osseous–vascular work trauma of the lower extremities were analyzed. All the patients managed were from 19 to 48 yrs old, and their average age constituted 30.2 yrs old. The mine trauma with injuries of femoral and/or the shin bones and a. poplitea prevailed – in 129 (87.8%) and 116 (78.9%) observations, accordingly. There were performed 92 two–staged interventions in a common operative team with traumatologists. \u0000Results. Positive result of operative treatment was noted in 73 (79.3%) patients. The time span between the trauma moment and the operation beginning in this group of the patients have constituted from 2 to 5 h, while the degree of an acute arterial ischemia in accordance to V. S. Savelyev (1973) were signed as IIa–IIb. The operation duration have constituted from 3 to 4 h. All the patients have had a combined shock Degree II. The preoperative blood loss volume has been ranged from 1 to 2.5 l. The a. poplitea injury have had prevailed and occurred in 43 (58.9%) patients. In 6 (6.5%) patients after reconstructive intervention the extremity amputation was performed. \u0000Conclusion.
