[Pseudoaneurysm Arose from the Side Branch of Vascular Graft after Thoracic Aortic Replacement]. 【胸主动脉置换术后血管移植侧支产生假性动脉瘤】。

He underwent ascending aortic replacement for acute aortic dissection 21 years ago and aortic root replacement 15 years ago. During postoperative follow up, a pseudoaneurysm of the ascending aorta was found. Cause of the pseudoaneurysm was supposed to be injury of the anastomotic site by a sternal wire. Since the pseudoaneurysm was enlarging, operation was indicated. Incising the pseudoaneurysm, we did not find silk threads that were used for ligation of the side branch of the vascular graft, and there was bleeding from the side branch. It is reported that silk threads lose strength in vivo over time. Previously, we had ligated side branches of vascular grafts only with silk threads. Two months after the event, he was referred to our department because the enhanced computed tomography (CT) image revealed a low-density area on the descending aorta, which was suspicious of a mobile plaque. He had no symptoms and there were no other signs suggesting embolization or metastatic tumor in CT image. Enhanced magnetic resonance imaging (MRI) or positron emission tomography (PET) image revealed no enhancement in the lesion and we diagnosed it as mural thrombosis on the descending aorta. We performed thoracic endovascular aortic repair( TEVAR) to compress the intra-aortic thrombus. He had no complication such as thromboembolism in the procedure and also had a good course for two years after the procedure.</p>","PeriodicalId":17841,"journal":{"name":"Kyobu geka. We report the case of a 72-year-old man who underwent coil embolization of bilateral peripheral pulmonary artery aneurysms. Ten years ago, a left upper lobe nodule was incidentally noted on a chest computed tomography (CT). The nodule was suspected as a benign tumor and followed up. As it gradually enlarged, the patient referred to our hospital. Chest CT showed a 2.0-cm nodule in S1+2 of the left lung and a 0.6-cm nodule in S10 of the right lung. Contrast-enhanced CT showed that each nodule had the same contrast enhancement as the pulmonary artery. The left upper lobe nodule and the right lower lobe nodule were connected by a shunt with A1+2b and A10. They were diagnosed as pulmonary artery aneurysms, and coil embolization was performed. Eighteen months later, no enlargement of the pulmonary artery aneurysms or new lesions were observed.</p>","PeriodicalId":17841,"journal":{"name":"Kyobu geka. He was admitted for diagnosis and treatment, and underwent thoracoscopic thymic and thymic tumor removal. The postoperative course was uneventful, and the patient was discharged on the 4th postoperative day. Histopathological examination revealed numerous foreign body giant cells and cholesterol crystals in the thymus tissue, and the patient was diagnosed with cholesterol granuloma. One cause of cholesterol granuloma is trauma. Trauma was suspected as the cause in this case because the patient had bruised his chest in a traffic accident eight years ago. It is difficult to diagnose cholesterol granuloma from image findings, and surgical resection is necessary for diagnosis and treatment to differentiate it from malignant diseases.</p>","PeriodicalId":17841,"journal":{"name":"Kyobu geka. Eight days after PCI, the patient presented worsening heart failure. Transthoracic echocardiography( TTE) and enhanced computed tomography( CT) revealed ventricular septal rupture( VSR) and left ventricular aneurysm( LVA). After establishing cardiopulmonary bypass, the LVA was longitudinally excised along the left anterior descending artery( LAD). A septal anterior ventricular exclusion( SAVE) procedure was performed using a bovine pericardial patch to exclude the LVA and VSR. The VSR was directly closed with pledgeted sutures. Additionally, coronary artery bypass grafting was performed to the LAD using the left internal thoracic artery. However, reusable ablation devices are preferred for reducing healthcare costs. At our department, the general-purpose cryosurgical unit \"Freeze Stick M\" has been used for maze surgery and intercostal nerve block during left thoracotomy, effectively minimizing postoperative pain.</p><p><strong>Patients and methods: </strong>Between August 2016 and July 2024, 44 patients underwent arrhythmia surgery for AF (36 chronic AF, 8 paroxysmal AF). The mean age was 71.9±7.6 years, with 66% males. Surgical procedures included biatrial maze (25 cases), pulmonary vein isolation (16 cases), left atrial maze (2 cases), and right atrial maze (1 case), all with left atrial appendage closure. Outcomes were compared between the SIM group( Freeze Stick M) and AC group( AtriCure).</p><p><strong>Results: </strong>The 30-day mortality rate was 2.3%. Postoperative survival was 90.0% at 1 year, 81.0% at 3 and 5 years, and 67.5% at 7 years. AF-free rates at discharge were 76.2%( SIM) vs. 73.9%( AC). For chronic AF, AF-free rates at 5 years were 41.9%( SIM) vs. 38.5%( AC)(p=0.168).</p><p><strong>Conclusion: </strong>The general-purpose cryosurgical unit \"Freeze Stick M\" performed similarly to the Atri-Cure device as a lesion-creation device.</p>","PeriodicalId":17841,"journal":{"name":"Kyobu geka. {"title":"[Study of the Characteristics of Lung Cancer Surgery Cases in the Elderly and the Usefulness of the Japanese National Clinical Database Risk Calculator].","authors":"Teruhisa Kawaguchi, Fumiaki Watanabe, Katsutoshi Adachi","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Introduction: </strong>We examined the utility of the National Clinical Database( NCD) risk calculator in assessing the risk of postoperative complications in elderly patients undergoing lung cancer surgery.</p><p><strong>Methods: </strong>We included 360 cases of primary lung cancer surgery involving lobectomy or more resection performed at our department from May 2012 to December 2021. We divided the patients into two groups based on age (75 years and older, or less than 75 years) and retrospectively analyzed the correlation between the four risk scores of the NCD risk calculator, the incidence of postoperative complications, and the NCD risk calculator.</p><p><strong>Results: </strong>In the group aged 75 years and older, there was a significant decrease in respiratory function and significantly higher scores on the NCD risk calculator. When comparing the correlation between the presence of postoperative complications and the \"death or severe complication score,\" the area under the curve( AUC) as a risk scoring system was 0.710.</p><p><strong>Conclusion: </strong>The NCD risk calculator is suggested to be a useful tool for assessing the risk of surgical complications in elderly patients.</p>","PeriodicalId":17841,"journal":{"name":"Kyobu geka. A posterior mediastinal tumor was noted on chest computed tomography (CT). Magnetic resonance imaging (MRI) scan showed a high signal on both T1- and T2-weighted imaging. Lipoma was suspected, but since the border with the vertebral body was partially unclear, video-assisted tumor resection was performed for diagnosis and treatment purposes. Finally, we diagnosed it as non-infiltrating angiolipoma. Angiolipoma is a benign tumor that accounts for 5-17% of all lipomas. Pathologically, they can be divided into two types:infiltrating and non-infiltrating. Most are non-infiltrating, but recurrent cases of the infiltrating type have been reported. Angiolipoma commonly occurs in subcutaneous tissues of the upper extremity and trunk, but we report a rare case that occurred in the posterior mediastinum.</p>","PeriodicalId":17841,"journal":{"name":"Kyobu geka. We present a case of a 48-year-old man with Stanford type A acute aortic dissection complicated by malperfusion of the right common carotid artery. His consciousness was deteriorating preoperatively, and we performed an emergent aortic root replacement and partial aortic arch replacement. He suffered a severe cerebral edema with a brain herniation caused by cerebral infarction after the surgery. Decompressive craniectomy saved his life and improved his neurologic functions, and he has become able to communicate well. It is suggested that the decompressive craniectomy for the severe cerebral infarction after a surgery for type A acute aortic dissection with cerebral malperfusion is a good option for better neurologic outcomes.</p>","PeriodicalId":17841,"journal":{"name":"Kyobu geka. A 39-year-old man presented to his physician for evaluation of fever and pericardial pain and was referred to our department for further management. The patient's condition stabilized after pericardiocentesis;however, the cause of cardiac tamponade remained undetermined on admission. Transesophageal echocardiography and electrocardiography-synchronized contrast-enhanced computed tomography (CT) revealed a suspected source of bleeding in the right atrium, necessitating open chest surgery. Preoperative imaging showed no evidence of a tumor;however, intraoperatively, we observed a suspected right atrial tumor, which was resected, followed by repair using a bovine pericardial patch. Histopathological examination confirmed diagnosis of angiosarcoma. Angiosarcoma is often diagnostically challenging in the early stages, and multiple examinations are important for early diagnosis and treatment.</p>","PeriodicalId":17841,"journal":{"name":"Kyobu geka.
