While psychological stress affects cardiovascular health, the link between specific psychological symptoms and CMD in ANOCA patients remains unclear.</p><p><strong>Aims: </strong>This study aimed to investigate the association of anxiety, depression, and sleep disorder with CMD assessed by invasive coronary physiology in ANOCA patients.</p><p><strong>Methods: </strong>A total of 102 ANOCA patients underwent coronary angiography and invasive coronary function testing with thermodilution-derived index of microcirculatory resistance (IMR) and coronary flow reserve. CMD was defined as coronary flow reserve <2.5 or IMR ≥25. Anxiety, depression, and sleep quality were assessed using the Generalized Anxiety Disorder-7, the Patient Health Questionnaire-9, the Pittsburgh Sleep Quality Index scales, respectively.</p><p><strong>Results: </strong>Among the enrolled 102 ANOCA patients, 50.0% exhibited anxiety symptoms, 30.4% showed depression symptoms, and 67.6% reported sleep disorders. Patients with abnormal IMR had significantly higher prevalence of anxiety (72.9% vs. 29.6%; P <0.001), depression (43.8% vs. 18.5%; P = 0.009), and sleep disorders (81.2% vs. 55.6%; P = 0.006) than those without. Multivariable analysis identified anxiety as an independent predictor of CMD with abnormal IMR (odds ratio, 4.84; 95% confidence interval, 1.98-12.36; P <0.001). For CMD defined by abnormal coronary flow reserve, sleep disorder showed a borderline association. However, comparative real-world data between dual-chamber LPMs and conventional transvenous pacemakers, are lacking to date.</p><p><strong>Aims: </strong>Our aim was to present the first single-center experience with dual-chamber leadless pacemakers and to compare early electrical performance, safety, and 90-day clinical outcomes with contemporary dual-chamber transvenous systems.</p><p><strong>Methods: </strong>This investigator-initiated registry included consecutive patients undergoing dual-chamber pacemaker implantation: 178 dual-chamber transvenous pacemakers (2023-2024) and 27 dual-chamber LPMs (2024-2025). Leadless implantations were performed as two stages due to reimbursement regulations. Baseline characteristics, procedural parameters, electrical performance at implantation and 90 days, and complication rates were analyzed.</p><p><strong>Results: </strong>Patients receiving LPMs had a higher prevalence of prior device infection (22.2% vs. 1.1%; P <0.001), chronic dialysis (22.2% vs. 1.1%; P <0.001), and malignancy (18.5% vs. 3.4%; P = 0.007). Procedural success was achieved in all LPM cases. Despite staged implantation, the 90-day complication rate was low and comparable between patient groups (7.4% vs. 4.5%; P = 0.62). Electrical parameters improved over time in the LPM group, with stable low pacing thresholds at 90 days. However, an estimated battery longevity was shorter in the LPM cohort.</p><p><strong>Conclusions: </strong>In this real-world preliminary comparison, dual-chamber leadless pacing demonstrated promising short-term safety outcomes despite implantation in a clinically complex population, traditionally considered as at elevated risk for transvenous systems. Despite the widespread use of QoL assessments, the role of patient-related factors as determinants of QoL remains not fully elucidated.</p><p><strong>Aims: </strong>This study aims to assess the relative impact of different factors on QoL in HF patients, also its impact on mortality in this group.</p><p><strong>Methods: </strong>The study was based on the national registry - HEart failuRe ObsErvational Study of the Polish Cardiac Society, where QoL was assessed at baseline (Kansas City Cardiomyopathy Questionnaire [KCCQ]-12), and follow-up data on all-cause mortality were obtained. Adjusted linear regression was used to identify factors associated with KCCQ-12.</p><p><strong>Results: </strong>The study included 1397 patients with a mean KCCQ-12 score = 47.92 (28.65-72.92). Higher New York Heart Association class had consistently significantly lower values of all KCCQ-12 domains and KCCQ-12 overall score (85 [69-93] vs. 65 [46-79] vs. 35 [23-49] vs. 21 [10-38]; all P <0.001). Hospitalized patients had significantly lower KCCQ-12 overall score, with a significant difference between patients hospitalized for HF or non-cardiac reasons and patients hospitalized for other cardiac conditions. Apart from visit type, individual factors, cardiovascular conditions, and non-cardiac conditions were independently associated with KCCQ-12. KCCQ-12 also significantly influenced all-cause mortality (hazard ratio per 1 point increase = 0.976; 95% confidence interval, 0.970-0.981; P <0.001).</p><p><strong>Conclusions: </strong>Individual factors with both cardiac and non-cardiac comorbidities are independently associated with QoL in patients with HF. The clinical relevance of off-pump procedures continues to grow in the context of an aging population. However, comparative data between minimally invasive direct coronary artery bypass (MIDCAB) and off-pump coronary artery bypass (OPCAB) via sternotomy in octogenarians remain limited.</p><p><strong>Aims: </strong>This study aimed to retrospective compare the outcomes of MIDCAB and OPCAB in patients aged >80 years with 1 vessel disease, using data from the Polish National Registry of Cardiac Surgery Procedures.</p><p><strong>Methods: </strong>One thousand thirty‑five patients undergoing isolated single-vessel coronary artegy bypass grafting with the left internal thoracic artery between 2006 and 2023 were analyzed. Among them, 296 underwent MIDCAB and 739 underwent OPCAB surgery. Propensity score matching yielded 237 well-balanced patients per group. Baseline characteristics, postoperative outcomes, and early and long-term survival were compared. Follow-up ranged from 0.1 to 12.8 years.</p><p><strong>Results: </strong>No differences were observed in baseline characteristics or early and long-term mortality between the groups (24.9% in the MIDCAB, 32.5% in the OPCAB group, P >0.084). The MIDCAB group showed a shorter hospital stay (11.26 vs. 12.62 days; P = 0.004), a lower incidence of respiratory complications (0.8% vs. 4.2%; P = 0.041), and reduced prolonged mechanical ventilation (16.6% vs. 28.6%; P = 0.008).</p><p><strong>Conclusions: </strong>In octogenarians undergoing isolated left internal thoracic artery to LAD bypass, MIDCAB is as safe and effective as OPCAB.
