The longterm adverse effects, aside from complications, are known in relation to education, development and employment (2). There are also significant short-term effects on QoL with recurrent acute illness (3). Little data has been published on the quality of life (QoL) of children living with ear disease in Australia. We investigated the utility of the Otitis Media-6 (OM-6) questionnaire (3) among Aboriginal and non-Aboriginal parents/caregivers to assess the QoL experienced by children undergoing surgery for ventilation tube (VT, grommet) insertion to treat recurrent otitis media (OM) and otitis media with effusion (OME). The OM-6 questionnaire is a tool developed in the United States specifically for assessment of the impact of recurrent acute OM and OME (4). The purpose of this article is to confirm the in vitro activity of the PVP-I nasal spray against SARS-CoV-2 and whether that may translate into reduced nasal shedding in vivo. Methods: Two SARS-CoV-2 virus isolates were exposed to 0.5% PVP-nasal spray (Nasodine®) for different times in vitro, with PCR and cell culture used to assess impact on viral infectivity and RNA copies. An open label in vivo single arm pilot study of 14 subjects with positive COVID-19 PCR diagnosis was undertaken. Baseline nasal swabs were collected to quantify SARS-CoV-2 pre-treatment, followed by a single 0.5% PVP nasal spray application (1.12 mL). Nasal swabs were collected at 5, 15, and 60 minutes post-dose to assess immediate and residual impact of treatment. Results: In vitro, the nasal spray reduced infectivity by 3.5 log10 TCID50/mL (99.97%) after 15 seconds exposure and eliminated detectable viral infectivity after 60 seconds;there was no effect on viral RNA detection by PCR. In vivo, culturable virus (VOC beta/B.1.351 variant) was obtained from 6 of 14 PCRconfirmed positive subjects;in these subjects, 5 minutes after the single PVP-I dose, the mean viral titre was reduced by 65% versus baseline and by 79% versus baseline at 60 minutes post-dose. 5 of the 6 subjects (83%), had reduction or cessation of viral shedding at 5 minutes in all 6 subjects, virus titers 60 minutes post-dose were below baseline value. 0.5% PVP-I treatment didn’t interfere with the laboratory diagnosis of COVID-19 via PCR-detection of viral RNA in humans. Conclusions: 0.5% PVP-I nasal spray is rapidly virucidal to SARS-CoV-2 in vitro using exposure times consistent with nasal residence;single in vivo nasal administration reduced infectious viral titers in COVID-19 subjects with culturable virus. A single application of 0.5% PVP-I nasal spray does not interfere with PCR-mediated laboratory diagnosis of COVID-19. We are undertaking a large double blinded randomized controlled trial to confirm if repeated application of 0.5% PVP-I nasal spray over a longer period could be useful in suppressing viral shedding and transmission risk in COVID-positive patients. © Australian Journal of Otolaryngology. Baseline impedance of esophageal mucosa has been shown to reduce with prolonged acid exposure. Mean nocturnal baseline impedance (MNBI) is a novel measure that has shown promise in GERD but has not yet been evaluated in LPR. This study aimed to assess the role of MNBI in LPR and GERD patients. Methods: Off-therapy impedance-pH tracings were blindly reviewed for 187 patients previously prospectively allocated clinical diagnoses of LPR (n=105) or GERD (n=82). Conventional impedance-pH measures and MNBI were analysed for the two groups. Results: MNBI was reduced in both groups of refluxers. MNBI was significantly lower in the distal esophagus in GERD patients compared with LPR (1,679±914 vs. 2,109±863; P=0.001). Similarly, in the proximal esophagus, MNBI was lower in GERD than LPR (2,289±579 vs. 2,541±471; P=0.001). In the pharynx, MNBI was similar between the two groups (2,116±699 vs. 2,133±770; P=0.878). Distal acid exposure time (AET) and the number of distal acid reflux episodes negatively correlated with distal esophageal MNBI (r=−0.195; P=0.007) and (r=−0.330; P<0.001) respectively. Conclusions: Baseline impedance was reduced in both LPR and GERD at both distal and proximal esophageal measurements, and more severely reduced in GERD. Baseline impedance is strongly and inversely related to acid exposure in the esophagus. Pharyngeal MNBI was not reduced or different between groups. Distal and proximal esophageal MNBI may be useful in diagnosis of LPR as well as GERD.","PeriodicalId":165885,"journal":{"name":"Australian Journal of Otolaryngology","volume":"4 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2022-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"124575019","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
