{"title":"Impact of coronavirus disease 2019 (COVID-19) on Ear, Nose and Throat (ENT) surgery waitlists in regional centres","authors":"Samantha Jaensch, E. Tweddle","doi":"10.21037/ajo-22-1","DOIUrl":"https://doi.org/10.21037/ajo-22-1","url":null,"abstract":"Background: Severe acute respiratory syndrome-related coronavirus 2 (SARS-CoV-2) [coronavirus disease 2019 (COVID-19)] impacted many aspects of healthcare in Australia, particularly already strained elective surgery waiting lists in regional areas. Elective operating was suspended throughout New South Wales (NSW) and Victoria (Vic) despite cases being predominantly located in metropolitan centres. The aim of this study was to quantify the effect of COVID-19 on patients' time spent on Ear, Nose and Throat (ENT) elective surgery waiting lists and the likelihood of breaching recommended waiting times. Methods: Elective ENT operating data was collected from Albury Wodonga Health (AWH) and Dubbo Health Service (DHS) between March 2019 and February 2021. Data was analysed based on the time period (March 2019-February 2020 = pre-COVID, March 2020-February 2021 = COVID) and priority category (1, 2 or 3 based on clinical urgency). Results: A total of 2,915 operations were recorded across the two sites during the study period. There were statistically significant increases in both the length of time spent on waiting lists as well as the likelihood of breaching recommended waiting times following the arrival of COVID-19. This was true for both sites as well as across all priority categories. Conclusions: This study demonstrated a significant increase on the length of time patients spent on elective surgery waiting lists as well as the likelihood of these patients breaching recommended surgery waiting times following COVID-19 pandemic. The shutdown of elective surgery due to the pandemic has had an unnecessarily large impact on regional areas where case numbers were initially low and local services are already stretched. During future waves, a region-specific response involving elective surgery shutdown is recommended, rather than a state-wide or nationwide approach. © 2022 Australian Journal of Otolaryngology. Its rate is variable reported in the literature. We aim to describe the incidence of dysphagia in laryngomalacia, the effect of interventions on this, and the period it persists in these infants. Methods: A prospective cohort from August 2017 to May 2018 of patients with laryngomalacia referred to the tertiary paediatric otolaryngology swallow clinic to a single surgeon were analysed for demographics, dysphagia risk factors, swallow function, and management. Results: There were 20 patients, with a mean age at diagnosis of 2.28 months. Gender was equally distributed in each group. The primary presentation was stridor (n=13, 65%), followed by both stridor and dysphagia (n=5, 25%), and dysphagia alone (n=2, 10%). Nine (45%) had supraglottoplasty. Pre-operative diet modification was instituted in 4 (44.44%). In the immediate post-operative period, this increased to 7 (77.78%) patients: 1 level 1 and 6 (30%) level 2. The median dysphagia time was 191.5 days. Oral aversion developed in 1/3. In the conservative group, diet modification was instituted in 4 (36.36%) patients: 3 level 1 and 1 level 2. The median dysphagia time for this population was 69 days. Conclusions: Dysphagia is an important co-morbidity that often co-exist with laryngomalacia. It persists long after the airway symptoms of laryngomalacia have resolved, especially in those who had supraglottoplasty. It is important to recognise this early to allow for appropriate management and follow up.","PeriodicalId":165885,"journal":{"name":"Australian Journal of Otolaryngology","volume":"5 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2022-06-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"133557862","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

We present a single-institution experience of successful continued management of HNC patients during the pandemic. Methods: A retrospective audit was conducted comparing operative time, anaesthetic time, case volume and type (aerosol generating vs. non aerosol generating) between equivalent time periods from 2019 and 2020 during the peak of the pandemic in Australia, from a single high-volume, tertiary, academic head and neck centre. Furthermore, we document the multi-level changes instituted for each department involved in the patient cancer journey. Results: The average operative time for aerosol generating procedures (AGPs) was 85 minutes (IQR 455) in March 2019 compared with 180 minutes in March 2020 (IQR 366). Case volume and case type was similar between the two years. With appropriate patient screening, perioperative planning and judicious use of personal protective equipment (PPE), higher-risk AGPs were able to continue essentially as normal. Conclusions: HNC requires timely investigation and treatment, even in the midst of a pandemic, which can be achieved at a dedicated, COVID-free oncology centre utilising clear, rapid communication and a multidisciplinary approach. © Australian Journal of Otolaryngology.
