About a year ago an incident involving the unfortunate death of a house officer and the subsequent claims of the hospital being the worst place to practice has once more pushed this issue to the fore resulting in a high-level committee investigating the matter and providing recommendations to ensure the wellbeing of junior doctors. While changes were affected, other similar allegations have surfaced. \u0000These problems however are not unique to Malaysia. They have and continue to be well documented in all healthcare facilities the world over. \u0000As educators and practitioners, we are major stake holders in any process involving the profession and it is important that we begin to think about strategies to address this issue holistically and prevent any further incidents of this nature. The other issue that needs study is if these are isolated incidents or if there are systemic defects in the process that have gone unattended. We all believe that most people in the profession do not subscribe to this behavior, however it does exist and must be acknowledged. \u0000To consider this problem holistically we need to look at the magnitude of the problem and viable solutions for the doctor in training, the trainers, and any other party with a stake in the outcome.

The most common scoring system is the Alvarado scoring system followed by other scoring systems like the RIPASA and AIR systems. Most of the scoring systems are good at ruling out appendicitis or selecting patients who may require further imaging like ultrasound or computerized tomography. On their own most clinical scoring systems are not used to solely diagnose acute appendicitis and we have conducted this review article to look at the common scoring systems that are being used and its sensitivity and specificity.

We report a case of 34-year-old male who presented with a short history of fever, bilateral pedal edema and left sided chest discomfort of 10 days duration. On evaluation he was found to have pericardial effusion. Extensive work up to evaluate routine causes of pericardial effusion was unremarkable. His pericardial fluid showed microfilaria, however there was no microfilariae in peripheral blood smear. He was managed with pericardiocentesis and 04 weeks of Albendazole, diethylcarbamazine to which he responded. In the tropics filarial pericardial effusion should always be considered in cases of pericardial effusion of unknown etiology.

Ear swabs were taken by an otolaryngologist and sent to the Microbiology Department for processing. The specimens were cultured within 30 min of collection and the isolates identified using standard microbiological techniques. Selected virulence properties were characterised.\u0000Results: The most common organism isolated was Staphylococcus aureus 18 (34.6%) followed by Pseudomonas aeruginosa 14 (26.9%). Fungal isolates were recovered from 6 11.5%) of specimens; in 11 (21.15%) of cases no microbe was detected. Age did not significantly affect incidence of CSOM of microbial origin (p = 0.1742) although most cases were in the age 31-40 years group. Male and female patients were almost equally affected (M: F = 1:1.08), DNase followed by biofilm formation were the predominant virulence phenotypes identified while coagulase, followed by haemolysin production, were least common.\u0000 Conclusion: Staphylococcus aureus and Pseudomonas aeruginosa were the most common organisms among CSOM cases and recovered from all age-groups. The etiology of TBN is not clear. The aim of this exploratory study was to examine differences in the content of of bromine (Br), calcium (Ca), chlorine (Cl), iodine (I), potassium (K), magnesium (Mg), manganese (Mn), and sodium (Na), as well as I/Br, I/Ca, I/Cl, I/K, I/Mg, I/Mn, and I/Na content ratios in tissues of normal thyroid and TBN. Thyroid tissue levels of eight chemical elements (ChEs) were prospectively evaluated in 105 apparently healthy persons and in 79 patients with TBN. Measurements were performed using non-destructive instrumental neutron activation analysis with high resolution spectrometry of gamma-radiations from activated short-lived radionuclides. Tissue samples were divided into two portions. One was used for morphological study while the other was intended for ChEs analysis. It was observed that in TBN the Br, Cl, Mn, and Na mass fraction, as well as the I/Br, I/Cl, I/Mn, and I/Na mass fraction ratios were higher whereas mass fractions of Ca and I and also I/K mass fraction ratio were lower than in normal thyroid. These changes can potentially be used as TBN markers. Furthermore, it was found that the levels of Br, Ca, Cl, K, Mg, Mn, and Na contents in the normal and affected thyroid gland were interconnected and depend on the content of I in thyroid tissue. Because I plays a decisive role in the function of the thyroid gland, the data obtained allow us to conclude that, along with I, such ChEs as Br, Ca, Cl, K, Mg, Mn, and Na, if not directly, then indirectly, are involved in the process of thyroid hormone synthesis.

If the problem of dental and oral health in stunted children is not addressed, it will cause malnutrition to get worse and the quality of life for stunted children to get lower.\u0000Methods: This research is an analytic observational study with a cross sectional approach. This study used the Early Childhood Oral Health Impact Scale (ECOHIS) Questionnaire and was conducted on stunted toddlers which would be filled out by the parents of these toddlers in Sumberjambe District, Jember Regency. The sampling technique uses total sampling. Of the 9 villages in Sumberjambe sub-district, 3 villages were taken randomly. Each village takes 1 posyandu with the most participants.\u0000Results: There is a significant relationship between the behavior of maintaining oral and dental health and the quality of life of stunting toddlers.
