Non-union can be of the following types, hypertrophic, oligotrophic, atrophic, and septic. In view of a non-union, exchange nailing is preferred and so routinely performed for non-unions and fractures of the tibia. Case Presentation: Here, we present a case of an implant failure from an atraumatic event in a case of atrophic non-union of the tibia, which was treated by exchange nailing and bone grafting. A 22-year-old male, with a history of the left closed tibia fibula diaphysis fracture 1 year ago, underwent exchange nailing with bone grating after having an atrophic non-union with implant breakage. Conclusion: Implant failure and non-union are caused due to a wide variety of factors. Recent advancements in spine surgery include ultrasonic bone scalpel, which cut bone accurately and precisely. It is a unique surgical device which offers a controlled osteotomy which slices the hard bone while the soft tissues remain largely unaffected. The major benefits of using this modern instrument are the soft-tissue sparing, controlled cutting, reduced bleeding, and thereby increasing the effectiveness. Our symposium in the 2022 WIROC issue, the brainchild of Dr. Vishall Kundnani and team, features a special on spine pathology. It has contributions from stalwarts of spinal surgery all over India in the form of case reports, review articles, original articles, imaging and diagnostic tests, meta-analysis, arthroscopy-assisted spinal surgery, and current trends in spinal surgery. It provides insight into the challenges associated with the diagnosis and management of various spinal ailments, which can cause our patients significant distress. Our four guest editorials include heart-warming messages from Dr. S. Gawhale, Bombay Orthopedic Society President, Dr. Kushall Kundnani, Dr. K. Badani, and Dr. Kshitij Chaudhary. We are fortunate that Dr. Bhaskar Anand, winner of Indian Society of Hand Surgery Award for lifetime achievement, has contributed his rich experience in dealing with CLUB hand. We feature a highly informative review article on management of the 1st time shoulder dislocators as well as an inspiring research article on newer concepts in the 1st time patellar dislocators. An original article summarizing the technique of Arthroscopic Latarjet management of shoulder instability is particular enlightening as it simplifies a difficult surgery in a succinct manner. The landscape of joint replacement surgery is ever changing with newer techniques of uncemented knee joint replacement receiving positive reviews in meta-analyses. Therefore, we have also featured an article on cementless knee replacement, which is an eye opener to the progress of science and technology. An article on the severity of violence of irate patient relatives toward doctors is frightening, but thought provoking. It guides the reader on how to take various preventive measures when faced with aggression and violence. The role of ultrasonography of the hip reveals how hip pathologies can be identified in their infancy, especially in the neonatal setting. Finally, a potpourri of articles in trauma of the foot and ankle is both stimulating and informative. {"title":"Clinical outcome of Patella stability after fixation of Osteochondral fracture in Acute Primary Traumatic Patella Dislocation without MPFL Repair or Reconstruction","authors":"Mukesh S. Surgical treatment is reserved for osteochondral fractures (OCF), medial patello femoral ligament (MPFL) avulsion/tear, compound injuries, and any underlying bony abnormalities requiring correction. MPFL repair (at avulsed site) or reconstruction along with underlying bony correction in PTPD is controversial. The purpose of this study is to evaluate patella stability after fixation of OCF in PTPD without performing MPFL repair or reconstruction, even in the presence of any underlying bony abnormalities. Material and Methods: This is a retrospective study of eight patients who had PTPD with OCF along with MPFL injury who presented between 2016 and 2019. Pre-operative X-rays and MRI were done to assess the status of MPFL, identify the presence of OCF and to calculate Insall-Salvati index, Tibial Tuberosity-Trochlear groove distance and Trochlear dysplasia. All patients underwent surgery by open approach and OCF fixation by bioabsorbable pins or suture material. MPFL was neither repaired at avulsed site nor reconstructed and even no bony corrections were done. Results: Average size of OCF fragments is 15*7 mm and all involving medial facet of patella. Lysholm score improved significantly from 32.8 to 94.8 and Kujala score from 49.1 to 96 at the end of 6 months. None of the patient had patella instability till latest follow-up, average follow-up period is 4.5 years (3–6 years). Clinically, apprehension test was negative in all cases with full ROM. Post-operative imaging including X-ray and MRI showed complete healing of the OCF along with complete healing/regeneration of MPFL with same pre-existing bony abnormalities if present preoperatively. Conclusion: This study shows excellent patella stability and full knee function along with complete union of OCF and complete healing/regeneration of MPFL in PTPD even in the presence of underlying bony abnormality. Single, one and half, double limb spica cast may be applied depending on the indication. The position of hip immobilization is also dependent on the underlying pathology for which the spica is being applied. The inguinal region and knee are potential weak spots in a spica and these should reinforce during spica application. Potential complications include plaster sores, breakage, avascular necrosis of femoral head (in developmental dysplasia of hip), neurovascular compromise, and superior mesenteric artery syndrome (very rare). They usually present as painless swelling near the joint and can be complicated by mechanical irritation, compression or injury of adjacent structures, fracture, malignant transformation, and post-operative recurrence. Locking of the knee refers to flexion of the knee without complete extension and passive extension is limited resulting in significant pain. Excision is a successful form of the treatment for symptomatic osteochondromas. Case Report: A case of osteochondroma of the proximal tibia illustrates extra-articular cause of locked knee secondary to the incarceration of pes anserinus tendons by the lesion. It’s true incidence and etiology in unknown but, recent evidence supports overuse injury as an underlying etiology. It can present as either unilateral or bilateral heel pain in active growing children or adolescents who had a history of new-onset sports activity. Diagnosis mainly relies on a thorough clinical examination with a positive squeeze test being confirmatory. Radiologic imaging may be unnecessary but can help rule out or avoid missing other problematic conditions. Treatment options are mainly conservative and include rest, medications, therapy, or orthosis. Educating parents and coaches is pivotal for the prevention of these self-limiting conditions in young athletes. We’ve We’ have seen a significant decrease in the number of patients. The study’s study’s goal was to determine the extent to which the epidemic has affected Indian orthopaedic practice. Methods: An online survey of currently practicing Indian orthopaedic doctors was done. The study enlisted the help of 500 orthopaedic surgeons. A statistical analysis was performed to determine the relationship between the demographic profile of study participants and other orthopaedic practice characteristics. Results: Maximum participants belonged to the age group of 30-–40 years (39.8%) and only 13.6% belonged to the age group of 51-–60 years. Approximately, 85.8% participants were married. Out of all, 86% participants were consultants and 14% were residents. Most of the participants (35.2%) have 5-–10 years of practice experience. Most of the participants were working in charity hospitals (31.4%) and very few of them (4.8%) were working in government hospitals. A substantial proportion of doctors are in peril as they are victims of violence by their patients or relatives, which often is unreported. Methodology:A structured study questionnaire was designed and prepared in the form of “Google Forms.” Results: Student doctors tackling such abuse are known to develop psychological stress at most times; and come about with high functioning depression, fear, and post-traumatic stress disorders at such a preliminary stage of their practice. From this study, we explored that how doctors from being genuinely praised for their altruistic work to having faced social stigma and abuse. Conclusion: We intent to fortify doctors to tackle this emerging issue for the safety of physicians. Drawing inference from the literature and graphical analysis, a sustainable way to alleviate duress on doctors would be ameliorating public health-care services and thus the quality of life.
