This study focuses on the development of a machine learning-based artificial intelligence system for the identification of heart disease. Diagnose, risk stratification, and management are basically a few of the critical thinking-intensive aspects of medical treatment that have been mechanized because of the development of artificial intelligence (AI) and data science, which has reduced the strain on doctors and decreased the possibility of human error. Using the primary clinical outcomes of each CHD (Congenital heart defects) and the right computational algorithms, risk stratification as well as the prediction of treatment results are feasible. Cardiovascular diseases (CVDs) have a significant economic burden for healthcare systems all over the world and are associated with high morbidity and mortality. In the United States, adults had a prevalence of 7.2% for coronary artery disease, which is one disease category included under the CVDs umbrella. In the years 2016–2017, this disease entity damaged the United States economy 360 billion US dollars. The domain of cardiovascular medicine has expanded in new directions because of the development of artificial intelligence (AI) as well as machine learning over the past two decades. It will be highly beneficial to the treatment of this disease if we are able to predict cardiac arrest in the early stages. A quick and effective detection method must be developed in order to decrease the enormous number of deaths caused by cardiovascular diseases. In order to increase the model's ability for predicting heart disease in every individual, an effective approach was utilized to control how it can be used. Ultrasonography revealed hypoechoic lesions in the affected area. Fine needle aspiration cytology (FNAC) indicated the presence of Acid-fast bacilli. Given the presence of multiple matted suppurative lymph nodes, surgical excision was performed, and the child was initiated on anti-tubercular treatment (ATT). After 3-4 weeks, the swelling showed signs of reduction, and the child is currently under regular follow-up. Protection from a viral infection is mainly achieved by virus-neutralizing antibodies which are produced after infection or vaccination. But to what degree these can induce the production of neutralizing antibodies is poorly understood. To determine the development of SARS-CoV-2 IgG antibody production against spike protein after infection and vaccination. A prospective study was conducted from July 2021 to December 2021 at tertiary care hospital, Solapur. Based on the evidence of previous SARS-CoV-2 infection and status of vaccination, a total of 150 participants above 18 years of age were enrolled. A blood sample was collected for the serum to perform an Enzyme-linked immunosorbent assay (ELISA) using the ErbaLisa COVID-19 IgG kit. Chi-square test for goodness of fit was used to analyze the data. In this study, we have assessed the development of protective immunity and it was found that 89.33% (134/150) of participants had a detectable level of SARS-COV-2 IgG antibodies. Also, in the present study the development of antibodies following infection, after a first and second dose of vaccination is compared, and observed that SARS-COV-2 IgG antibodies were detected in 88% of participants following infection, in 84% of participants after the first dose, and in 96% after the second dose. Effective vaccination is an essential solution to decrease - the severity, transmissibility and the emergence of new variants. Still, the immune response or the associated stress may link the vaccine with certain temporary menstrual changes. A Total of 100 women met the inclusion criteria. 23 experienced irregular bleeding following the vaccine. The most predominant menstrual changes were more menstrual bleeding (43%), more menstrual pain (41%), delayed menstruation (38%), fewer days of menstrual bleeding (34.5%), and shorter cycle length (32%). Menstrual cycle change following COVID-19 vaccination appears small and temporary and should not discourage individuals from becoming vaccinated. On neurological examination found weakness in all four limbs with exaggerated deep tendon reflex. Blood tests were within normal limits. He was treated with pulse steroid therapy that is 1gm of methylprednisolone IV for 5 days but no improvement was noted. This is a case of neuromyelitis optica spectrum disorder which shown no improvement with pulse steroid therapy. Hypertension has been identified as a major cause of morbidity and mortality across the world. A 29-year-old female presented with sudden onset painless loss of vision in both eyes since a day. Known case of hypertension for 6 months on irregular medication. On examination vision of the right eye was hand movement with no pinhole improvement, and on left eye it was counting finger 2 meter with no pin hole improvement. Intraocular pressure of both eyes was normal. Anterior segment examination of right eye was normal except relative afferent pupillary defect. Anterior segment of left eye was normal. Fundus examination - Bilateral disc swelling with inferior retinal detachment extending 4 to 7 clock hours with flame shaped haemorrhages and cotton wool spots over blood vessels with AV crossings with dull foveal reflex. OCT showed bilateral exudative detachment of sensory retina. BP -150/100 mmhg. Further investigation found to have acute renal failure, requiring urgent dialysis, and patient was started on dialysis. 3weeks after blood control was acheived ,visual acquity improved to 6/18 BE with exudative retinal detachment in both eyes resolved by its own except with few residual haemorrhages. Accelerated HTN may precipitate bilateral exudative retinal detachment in patient with acute renal failure in young adults. Bilateral exudative retinal detachment is a catostropic event which results in devastating vision loss within very short time span. Timely intervention may save the vision loss and renal failure. Kumar","doi":"10.51131/ijpccr/v3i3.22.48","DOIUrl":"https://doi.org/10.51131/ijpccr/v3i3.22.48","url":null,"abstract":"Rhinocerebral mucormycosis (RCM) is an angioinvasive fungal infection, in which infection of the nasal and sinus walls may secondarily invade the orbit, skull base, and brain. RCM carries high mortality up to 46 % in patients with sinus involvement and 62 % in patients with rhinocerebral involvement. The diagnosis of RCM and assessment of the resection margins are based on histopathology, specific cultures, and/or PCR detection of Mucorales species. It was a prospective, longitudinal study conducted under neurosurgery department in Basaveshwara Medical College and Hospital, Chitradurga, Karnataka during 1st May to 30th June 2021. All patients with radiologically, clinically diagnosed & biopsy proven by samples collected intraoperatively are included in the study. Caldwell luc approach was employed for maxillary sinus debridement, FESS for pansinusitis cases & Craniotomy for cases with intracranial involvement. In this study, we found that age, gender, neutrophil counts, monocyte count, ferritin levels, CT severity, diabetic status and usage of steroids and oxygen had a major role in the pathogenesis of mucormycosis. IHC was done and genetic studies were sent. The surgical treatment of RCM requires early diagnosis & aggressive surgical debridement. The outcome of the aggressive debridement was good. The debridement should be continued until clear resection margins are obtained based on the assessment of microscopical findings of hyphae, fungal cultures, and/or fungal DNA detected by PCR. Prashanth","doi":"10.51131/ijpccr/v3i3.22.40","DOIUrl":"https://doi.org/10.51131/ijpccr/v3i3.22.40","url":null,"abstract":"MCTD is a rare disease with overlapping features of many connective tissue disorder and the presence of positive anti-U1RNP antibodies with anti-ssA (Ro), anti-ssB (La) being negative. We report a case with fever and edema of upper limb and lower limb small joints associated with violaceous discolouration which on further evaluation led to diagnosis of MCTD. A female aged 44 years presented with history of fever of moderate degree with no diurnal variation and subsides on taking medication for 7 days and had also noticed ecchymoses followed by maculopapular rashes (violaceous) associated with tenderness of fingers and toes (more on exposure to cold). On further evaluation led to a diagnosis of MCTD. This case report urges the clinicians to exercise great caution for connective tissue diseases in patient presenting with similar history. This rare case of disease is uncommon in the available literature, especially for this ethnicity or region. This report should be of great interests to all diagnosticians of internal medicine.
