There is strong evidence for a potential role of the eNOS gene. The aim of this case control study was to investigate the possible role of genetic variants of the endothelial Nitric Oxide Synthase (eNOS) gene(VNTR) in the pathogenesis of nephropathy in patients with diabetes mellitus. The study included 196 subjects (145 T2DM and 51 normal control) T2DM were classified into 2 groups: 97 diagnosed with DN, and 48 diabetic without nephropathy and hypertension as +ve control. randomly selected was conducted to assess the association of SNP eNOSintron 4a/b VNTR polymorphism gene with diabetic nephropathy in Iraqi population. Blood samples from subjects and controls were analyzed to investigate the eNOS genotypesNo significant differences were found in the frequency of eNOS genotypes between diabetic patients (either in group 1 or group 2) and controls (p >0.05). Also, no significant differences were found in the frequency of eNOS genotypes between group 1 and group 2 (p >0.05).

Acute kidney injury occurs after cisplatin chemotherapy in approximately 30% of patients where severe and lifelong nephrotoxicity can result regardless of the use of powerful hydration with normal saline. In spite of scientific efforts to find relatively less toxic but equally effective substitutes, cisplatin continues to be widely involved in chemotherapy as first line antitumor agent. Apoptosis and necrosis, due to DNA damage response pathways, oxidative stress in association with severe inflammatory response and caspase activation, are the major sources of acute kidney injury due to cisplatin. L-carnitine with their antioxidant, anti-inflammatory and to some extent antiapoptotic effects may have ameliorative effect on nephrotoxicity. \u0000Aim: To assess the effects of L-carnitine in protection from cisplatin–induced nephrotoxicity in cancer patients. \u0000Patients and methods:28 patients were participated in the study and successfully completed their treatment cycles. They were randomized into two groups (N=14 in each). In group I, patients received six cycles of cisplatin based regimen with 21 days-intervals. In group II, patients received L-carnitine(500 mg oral tablet twice daily) plus cisplatin based regimen. GFR (Modification of Diet in renal Diseases formula) was measured at base line and 21 days after 1, 2, 4 and 6 cycles but IL-18 was measured at base line and 1 day after 1, 2, 4 and 6 cycles of cisplatin based regimen. \u0000Results:In patients of group I, cisplatin-based regimen caused significant (P<0.05) increment in serum creatinine and urea levels, highly significant (P<0.01) increment in serum urinary IL-18 levels, and significant (P<0.05) decrement in GFR in comparison to base line levels. \u0000Conclusions:Each of L-carnitine significantly ameliorated nephrotoxicity in patients that received cisplatin.

Cerebral vasomotor reactivity (VMR), one of the most accurate markers of cerebral hemodynamics, has been shown to be impaired in subjects with carotid artery steno-occlusive disease and associated with an increased risk of ischemic events. Transcranial Doppler ultrasonography )TCD( study have been used to investigate cerebral vasoreactivity to identify patients with increased risk and to better plan the treatment. Objective: The aim of study was to assess cerebral vasoreactivity among type2 diabetes mellitus patients by using transcranial Doppler ultrasonography. Patients and method: sixty five persons (45 type 2 diabetic patients and 20 normal control) (32 male,33 female) with mean age (46.95 ± 7.459) enrolled in this study. The study groups are collected randomly from Al Najaf center for Diabetic and endocrine in Al Sader teaching hospital during the time between January 2016 to June 2016. TCD examination was performed in Middle Euphrates Neuroscience Center for all stuied group to assess cerebral vasoreactivity in middle cerebral artery by breath holing test, hyperventilation test (HV), Transient hyperemic test (THRR). (Statistical Package of Social Sciences) SPSS version 23 was used for the statistical analysis. Results : There was highly significant change of breath holding index BHI{ between control group and diabetic group} (p.value<0.05) and significant change of HV between control group and diabetic group . Also statistical significant change in THRR in relation to glycemic control(p.value< 0.05). Conclusion : There was significant changes in TCD indices that assessed cerebral vasoreactivity in breath holding index, ,hyperventilation in type2 diabetic patients compared with control subjects. Effect of glycemic control on cerebral vasoreactivity in diabetic patients shows evaluation of CVR in diabetic goup in relation to glycemic control. There is no significant statistical association in BHI,HV between two groups of glycemic control but show statistical significant change in THRR (p.value< 0.05).

For this reason we study risk factors that associated with this disease in Thi Qar province patients. Such as mutations of breast cancer susceptibility genes 1 and 2 (BRCA1 and BRCA2), clinicopathological parameter ( age, family history and tumor site) . Eighty five blood samples were taken from patients who attended Al-Hussain teaching hospital and oncology unit in Al-Habboby hospital during the period (from August /2014 to April /2017) , fifty blood samples were collected from healthy women as a control. Blood samples were using in genetic study. For histopathological analysis fifty tissue samples were collected from fifty patients with breast cancer who were undergoing surgical resection (mastectomy) to prepare paraffin embedded blocks which have been used for histopathological diagnosis. The present study results revealed that a highest incidence of breast cancer occur in the age group between 40-49 years of age (41%),there is a significant difference among patients age groups (P ≤ 0.01). Family history of breast cancer indicates a strong association with risk of developing breast cancer. The present study showed that family history positive in 31 cases (36.47%), and 54 cases (63.52%) had negative family history of breast cancer. Also the results of present study showed that the 34 cases (41.5%) had first birth at age (20-29years), 18 cases (23.37%) had their first birth in age groups (15-19 years) and (30-39 years), and 7 for the molecular detection of BRCA1/2 gene mutation in patients and control groups. In 85 cases of breast cancer, detection of BRCA1/2 gene mutation by multiplex PCR done for the serum revealed that; (185 del AG) mutation in BRCA1 gene was detected in 6 patients (7.05%), and (5382 ins C) mutation in the same gene was detected in 2 patients (2.35%). Regarding (6174 del T) mutation in BRCA2 gene was detected in 3 patients (3.52%). In the control patients gene mutations of any types didn’t detected.
