F.I.B.M., Thi-Qar","doi":"10.32792/utq/utjmed/11/1/9","DOIUrl":"https://doi.org/10.32792/utq/utjmed/11/1/9","url":null,"abstract":"Objective: To present the advantages of elimination of the residual cavity after lung hydatid cyst removal by capitonnage associated with closure of the bronchial openings and partial pericystectomy via thoracotomy and to compare our study made at Al – Hussein Teaching Hospital in Thi-Qar governorate with those done in other centers and hospitals in Iraq and outside our country. Patients: This is a retrospective study of 100 patients for whom thoracotomy for lung hydatid cyst(s) was carried out over a period of about three years (from 1st of January 2013 till 1st of February 2016). Methods: The case sheets of relevant patients for whom thoracotomy for lung hydatid cyst(s) were carried out were reviewed to collect information like patient's sex, age, nature of surgery, method of obliteration of residual cavity after removal of hydatid cyst(s), etc... Results: All patients were admitted into AlHussein teaching hospital and thoracotomy was done for all of them regardless the age. Most of our patients suffered only from lung hydatid cyst(s) and several patients had also concomitant liver hydatid cyst(s) for which phrenotomy was carried out at the same session of thoracotomy to deal with the liver hydatid cyst(s). This concomitant phrenotomy to deal with the liver hydatid cyst(s) made the patient in no need for another surgery (laparotomy) in the future and the number of incisions and operations. A large number of our patients had no or mild symptoms at time of examination. Higher number of patients had cough and several patients complained from hemoptysis. All patients were exposed to chest Xray, chest CT – scan and abdominal U/S before surgery. During the operation, the hydatid cyst(s) was/were removed and the evacuation technique was most commonly applied and lobectomy was very rarely carried out. After removal of hydatid cyst(s), the residual cavity was mostly obliterated (capitonnage) to prevent collection of blood, exudate and possibly pus inside residual cavity and to seal the bronchial fistulae completely and ensure rapid expansion of lung. Conclusions: Pulmonary hydatid disease is endemic in Iraq and is diagnosed by imaging investigations and can be treated with minimal morbidity and mortality by lung preserving surgery. Elimination of the residual cavity after hydatid cyst removal by capitonnage was most commonly carried out. Patients &methods: Patients from Al Qurna presented to private clinic with epigastric abdominal pain each of them has been screened for H pylori via stool antigen test, after evaluations of other causes of abdominal pain. Data analysis was carried out using SPSS version-15 Results: a total of 254 patients 142 (55.9%) were males and 112 (44.1%) were females were studied. H pylori stool antigen was positive in 69 (27.2 %) and negative in 185 (72.8%) of the studied patients. Gastrointestinal symptoms that were observed included weight loss, loss of appetite (3.6%), heart burn (60.6%), vomiting (28.3%), diarrhea (7.1%), constipation (15.4%) and abdomen distension (24%). In 59.3% of the H. pylori antigen positive cases, the duration of epigastric pain was of >two weeks. Conclusions: logistic regression analysis indicated that epigastric abdominal pain of > two weeks and loss of appetite were two significant predictors of H. Pylori infection. (1) H. pylori plays a vital role in the aetiology of gastritis especially active antral gastritis, and accounts for 90%of duodenal ulcers and 70% gastric ulcers. (2) It also plays a role in the development of gastric mucosa-associated lymphoid tissue (MALT), lymphoma and gastric adenocarcinoma (3) . Thus the prevalence of this microorganism is a determinant of the risk of peptic ulcer and related lesions. In a study reported a prevalence rate of 55.8% among university students. (4) The bacteria can be cultivated from stool, vomitus and saliva in normal individual (5) . Methodology: Analytical, cross-sectional study in form of household survey had been conducted in AlNasiriyah City, from 1 October 2015 to the end of August 2016. A multistage sampling involving 423 individuals aged ≥ 60 years as a representative sample. Results: The overall prevalence of chronic NCDs was 89%. The top five chronic diseases were hypertension (67%), diabetes (31.2%), musculoskeletal diseases (15.4%), heart diseases (11.1%), and cataract (12.8%). Prevalence of mutimorbidity (patient with two or more chronic diseases) was 59%. The study include 50 patients with age range from 4-15 years old of different gender. The thresholds in the involved cases (conductive deafness) were 31dBHL in the ranging between (0-50) dBHL. 500Hz is the frequency mostly affected, 2000Hz most less affected. In bone hearing threshold (sensorineural problem), the frequency 1000Hz is mostly affected, while the least affected was 250Hz. Objectives : An attempt to identify the characteristics of this disease that is considered as quite common problem in our city, to identify the epidemiological, clinical, laboratory features and complications of HSP in our locality and to find the mode of treatments used by pediatricians and trying to uniform the management in a scientific way. Patients and Methods : Sixty five patients with HSP hospitalized in Sulaimaniyah pediatric Teaching Hospital in Kurdistan/ Iraq were included in this retrospective study from 1 st of January 2013 to 31 st of December 2014; they were selected by Case sheets based on the diagnosis of HSP at discharge time. Results: Of 65 patients, 37 (56.9%) were boys and 28 (43.1) were females, with a male to female ratio of 1.3:1. The patients’ ages ranged from (2 to 12) years, with a mean of 6 years. Approximately 61% of cases were presented during winter and spring. Upper respiratory tract infection preceded HSP in (36.9%) of the patients. Aim : To review our experience in surgical management of Congenital Hiatal Hernia in view of published literature . Study design : Retrospective and Comparative. Patients and Method : during a sixteen –year period (2000-2016) in a single thoracic surgery center , Detailed history taking and thorough physical examination were performed . The investigations included chest radiographs , contrast swallow studies , and rigid esophagoscopy mainly . Surgical techniques used to explore and repair hernia through laparotomy and left thoracotomy or both . Results : Twenty-six cases of Congenital Hiatal Hernia were operated, in M:F ratio of (1.16:1) , age range (25 days – 6 years ) Mean age of presentation (1.84 Year) . Chief presenting symptoms werew frequent vomiting (20 patients) , recurrent chest infections . The diagnosis depended primarily on contrast studies . Statistical analysis showed better outcome regarding hospitalization( 7.77 days compared with 9.125 ) and postoperative complication with Nissen procedure . 1 mortality included in this study caused by mediastinitis and septicemia . Conclusion : Early recognition of Congenital Hiatal Hernia is of high importance in reducing postoperative complications and improving outcome . although SAGES guidelines / No. 8 / 2013 stated (Hiatal hernias can effectively be repaired by a transabdominal or transthoracic approach ( ++++, strong ) ) , Nissen fundoplication done to infant age group with excellent outcome , and our experience was better using Nissen fundoplication. The study recommend the need for carrying out a multicenter study on a national level., team management of Congenital Hiatal Hernia .
