The initial distinction is based on the red cell size: anemias are classified as microcytic, normocytic, or macrocytic. The presence of abnormally shaped erythrocytes (poikilocytes) may suggest a specific disease or cause. A problem with the morphologic approach is that the morphologic changes in early anemia may be subtle and easy to miss. A second problem is that one morphologic abnormality may have several possible causes. Methods: A sample of 100 participants were included in the study. Informed consent was obtained. Venous blood samples were obtained and serum prepared, the hematology analyzer (Sysmex 3000) used to measure the hematology parameters, Cobas e114 used to estimate the serum ferritin. Results: The results showed significant differences between the mean level of male and female in the following parameters RBC’s count, and RBC’s indices MCV, MCH and PLTs count p. value( <0.05) and significant statistical deferens in RBC count , MCV and MCH among study group p .value (0.001 , 0.002 , 0.02) respectively. Platelets count show statistical significant differences among study group p. value (0.03). The correlation studies showed there are significant correlation between age and Hb concentration p. value (0.01), and positive correlation between weight and Hb concentration p. value (0.01). In current study there were Significant association between Age , RBCs, Hemoglobin , HCT, MCH ,MCHC and Platelets Count among patients with liver disease (P. value = 0.00, 0.001, 0.013, 0.000, 0.00,0.000, 0.000) respectively .The mean of hematological parameters, WBCs mean was 8.3 ±3.3 (×109 /l), RBCs 4.5 ± 0.5 (×1012/l) and the mean of Hb was 9.7±1.4 g/l which was low than normal, PLTs Count mean was 284.5 ±98.9 (×109 /l) while the mean was serum ferritin 13.4 ±7.1 mg/l in pregnant women with anemia. Table (7). In this study there was significant statistical difference between RBCs count and age (p. value 0.040) and statistical insignificant different between (WBCs, Hb, PCV, MCV, MCH, MCHC, PLTs count and Serum ferritin among pregnant women (p. value: 0.14, 0.68, 0.52, 0.18, 0.33, 0.7, 0.23 and 0.97) respectively. Positive statistical correlation was observed between serum ferritin and Hb, also positive correlation was observed between serum ferritin and PCV. In the current study significant positive correlation was observed between first and second trimester in Hb concentration P.value (0.000) and between second and third trimester P.value (0.000), Negative correlation between serum ferritin and different stage of trimester P.value ≥ 0.05. and significant statistical difference in Hb and serum ferritin level among patients use iron supplement P.value (0.009 and 0.000) respectively. Identifying the risk factors for acute myocardial infarction in this group of people is necessary for risk factor modification and developing cost-effective secondary prevention strategies as young. The aim of this study was to determine the prevalence, clinical background and in-hospital outcome of AMI among young (age ≤45 years) adults and its socioeconomic burden. Results: All Acute myocardial infarction patients during the period from 2016-2019 were divided into two groups: young adults (age≤45) and older adults (age>45). Age data were available for 3081 patients admitted with acute myocardial infarction. Out of these 593 (19%) patients were young adults with mean age of 39±6.2 whilst 2488 (80.7%) were older adults with mean age of 60±9. Young adult Patients were more of male gender (92% vs 82%, p<0.001) more smoker (47% vs 30 %, p<0.001) and had more prevalence of obesity (BMI ≥30 34% vs 27%, p<0.001) but were less diabetics (43% vs 57%, p<0.001) and less hypertensive (35% vs 58 %, p<0.001).Young adult patients had higher level of LDL (120±47 vs. 112.9±41.6, P=0.02), total cholesterol (189.2±54.4 vs. 173.9±47.7, P<0.001) and triglycerides (157.7±104.4 vs. 126.6±91, P<0.001).Young adult patients had more extensive thrombus and frequently required thrombus aspiration (16 % vs. 11%, p=0.003) but less common left main disease (0.9% vs 4%, p<0.001) and 3 vessels disease (8% vs 18%, p<0.001). Young adult patients had less deterioration of left ventricular function (EF 42.4±10.4 vs. 41.1±10.6, P=0.04). There was highly significant negative correlation between left ventricular ejection fraction (LVEF) and age (P<0.001) but positive correlation between age and length of in hospital stay (p=0.02).In-hospital complications including pulmonary edema, cardiogenic shock, cardiac arrest and mortality were similar in the two groups. Age, female gender and diabetes were found to be the independent predictors for in-hospital mortality among our patients (P=0.003, 0.05 and 0.05 respectively) Conclusion: Young adult patients presented with acute myocardial infarction are more frequently smokers, obese and dyslipidaemic. These patients also have more thrombus burden. These results underscores the importance of smoking cessation, weight reduction programs and Health education for public especially of this age. The operation was carried out with the remodeling of the right ventricle, using a swine bicuspid pulmonary prosthesis. Twenty-six years later, the patient is in excellent clinical condition, CF I (NYHA), with normal peripheral oxygen saturation. Recent invasive and not invasive imaging show: absence of intracardiac shunt and growing of the right ventricle outlet tract and discrete double lesion of the pulmonary valve. The pulmonary flow directed uniformly for both lungs. In selected cases, the long- term prognosis of patients with truncus arteriosus, undergoing early surgical repair, avoiding the use of valved conduit, makes for an excellent evolution, without new interventions. The aim of this study is to evaluate the importance of ambient temperature and dehydration, indicated by plasma osmolarity on the clinical outcomes of cardiac patients during Hajj season in 2017. Methods: We included all patients referred to tertiary center with acute coronary syndrome during Hajj period of 2017. Plasma osmolarity was calculated using concentrations of sodium, plasma glucose, and blood urea nitrogen at admission. Patients were stratified by groups (G) of admission osmolarity, clinical outcome was compared. The primary endpoints were in-hospital mortality, length of stay, Cardiac complications (heart failure, re-infarction, arrhythmia, shock and thrombus formation), left ventricular function and readmission rate. Result: Total of 300 patients were identified with mean age 56.2 ±12.1, 84% males and 97(32%) were pilgrims. They were exposed to average heat index 61.9 ±10.6° C. Significantly longer admissions were found in the group of higher osmolarity (G2) (≥295 mos/L) as compared to patients with normal osmolarity in G1 [6.7 ±14.9 VS 4.0 ±4.5, P=0.045]. Total in-hospital death rate was 4.3% (13). Using Binary regression analysis; osmolarity Group [p=0.009], Pilrgrim [P=0.005], Heat index [ P=0.005], were independent predictor of inhospital mortality, while Heat index is the only independent predictor for MACE [P=0.001]. Conclusion: Plasma osmolarity and heat index significantly affect cardiac patient’s outcome. This public health crisis is nowhere in the world greater, than in the most advanced nation, the United States of America. As of this writing, there are over 4 million reported cases of infection, and145,000 deaths. In some southern states, test positive rates are as high as 25%. Nation’s top infectious disease expert, Dr Anthony Fauci has been on the news, constantly advocating, social distancing, facial coverage, hand washing, and other public health best practices. On the other hand, our expert from the Midwest, Professor Michael Osterholm of the University of Minnesota, initially advised us to hide from the virus,-“if the virus cannot find you, you will not get the disease,” Just this morning, he was on the national TV heralding the bad news, -the coronavirus will never go away, -“like HIV, will stay with us forever.” These days, no news is good news, and no one seems to agree on any news, related to the coronavirus; infection, transmission, disease progress, immunity or the severity. Individuals with pre-existing conditions, such as hypertension, excess weight, obesity, type-2 diabetes and vascular diseases, are at a higher risk to experience severity of this disease, than healthy individuals. We report a rare case of tirofiban-induced severe thrombocytopenia and its management. Case Presentation: We present a 51-year-old male who underwent coronary angioplasty with drug eluting balloon for exertional angina. The platelet count dropped from 230 x 109 /L (before angiogram) to 3.8 x 109 /L within 24 hours of intravenous tirofiban infusion without active bleeding. We transfused 4 units of platelets, continued aspirin and clopidogrel and monitored in coronary care unit. Gradually the platelet count improved with no evidence of bleeding, ischemic or thrombotic events. Africa has one of the largest prevalence of diabetes mellitus in the world. However, the medical, economic, social and emotional barriers against optimal care of patients with diabetes are very prominent in sub-Saharan Africa. Therefore, poor glycemic control and suboptimal control of other risk factors are not uncommon. These increase the risk of developing cardiovascular complications of diabetes, the endpoint of which is often heart failure. The care for heart failure patients in sub-saharan Africa has been documented to be significantly low when compared with Asia or Southern America. The direct and indirect costs are unbearable and access to care is often compromised. So, when a patient with diabetes develops heart failure in sub-Saharan Africa, the burden is further magnified out of proportion to other developing regions in the world. This review article aims to highlight the peculiarities of the burden of care for heart failure patients with diabetes so that concerted efforts can be directed at addressing the challenges. Aiash","doi":"10.47363/JCRRR/2020(1)118","DOIUrl":"https://doi.org/10.47363/JCRRR/2020(1)118","url":null,"abstract":"Purpose of the state-of-the-art review: Left ventricular (LV) global longitudinal strain (GLS) is recently recognized as a more sensitive measure of LV myocardial systolic function compared with LV ejection fraction (LVEF). In addition, left ventricular GLS , myocardial performance index (MPI) and maximum rate of LV pressure rise during isovolumetric contraction (LV dP/dtmax) are more reproducible than traditional assessment of LV systolic function by two dimensional echocardiography (2DE) LVEF. These underutilized techniques can detect preclinical myocardial dysfunction in patients who are at risk of LV failure in valvular-induced heart disease . Current guidelines for diagnosis and treatment of valvular heart disease (VHD) include LVEF as one of the parameters to take into consideration in the clinical decision-making. However, a large body of evidence has shown that left ventricular GLS, MPI and LV dP/dtmax have been classically considered as a sensitive marker of LV contractility and inotropic state. In turn GLS and myocardial performance may be a better prognosticator than LVEF in aortic and mitral valve heart diseases. This timely state-of-the-art review, appraised the evidence and role of GLS, MPI and dP/dT as clinical tools in patients with aortic and mitral valve disease. Recent findings: Left ventricular GLS has been shown to be prognostic in low-flow, low-gradient severe aortic stenosis with preserved LVEF. The role of left ventricular GLS, Tei index (MPI) and maximum rate of LV pressure rise (LV dP/dtmax) in patients with aortic regurgitation and mitral valve diseases (regurgitation and stenosis) is less well established. Summary: Echocardiography is considered the primary non-invasive imaging tool for valvular heart disease assessment and the cornerstone method in diagnosing and evaluating the morphology and severity of aortic and mitral valve diseases. Currently, diagnostic-cardiac catheterization is no more recommended except in very rare cases when echocardiographic image quality is suboptimal, non-diagnostic and when the results of 2DE are discrepant with clinical data. Once clinical decision-making is based on the 2DE and three dimensional echocardiographic in assessment of the severity of mitral and aortic valve diseases, it is crucial that standards should be adopted to maintain accuracy and consistency across echocardiographic laboratories. This illustrative review article assesses left ventricular systolic function (LVEF) employing two and/or three dimensional echocardiography in comparison to GLS, MPI and LV dP/dtmax, especially applied for aortic valve (AV) and mitral valve (MV) diseases. It is noteworthy that this document only provides echocardiographic standards rather than making recommendations for clinical management.
