{"title":"An evidence-based case of Psoriasis managed with keynote prescribing Apis mellifica – A case report","authors":"Yashasvi Shakdvipiya, Yogyata Kashyap","doi":"10.25259/jish_44_2022","DOIUrl":"https://doi.org/10.25259/jish_44_2022","url":null,"abstract":"Psoriasis is an autoimmune disorder that is chronic and inflammatory in nature. It is clinically characterised by erythematous, sharply demarcated papules and rounded plaques covered by silvery micaceous scales. Homoeopathic medicines have been found to be useful in such cases. This report highlights a case of psoriasis in a 37-year-old male patient with complaints of psoriatic patches on almost the entire body since 18 years. The patient was treated using keynote prescribing. The remedy prescribed was Apis mellifica 30C, which covered the patient’s symptoms, including burning, redness, sensitivity to heat, and urinary complaints. Photographic records show marked improvement in all skin lesions. This case report suggests that Homoeopathic medicine can help to treat such autoimmune disorders; conventional medicine has a limited success rate in treating such conditions. A female patient of 35 years of age presented with complaints of irregular, profuse menstrual flow with dull, aching pain in the whole abdomen during menses, having previously been diagnosed as ‘Uterine fibroid’ for one year. Homoeopathic medicine was prescribed on the basis of the totality of symptoms which restored the patient’s health. Initially, Calcarea carbonica 200C was given, and later, the potency was increased to 1M. Rhus Toxicodendron 30C was prescribed during the course of treatment for viral fever. The assessment of significant improvement was done using UF symptoms and a health-related quality of life questionnaire every three months. The Modified Naranjo Criteria for Homoeopathy (MONARCH)-causal attribution inventory was also used to evaluate the response of homoeopathic medication and further clarify the importance of individualised treatment in homoeopathy. It also presents with mental and/or physical co-morbidities/disabilities. This manuscript describes the importance of an individualised multidisciplinary integrated approach, including homoeopathy, for the holistic management of epileptic children through two illustrative cases. The team included homoeopaths, paediatricians, paediatric neurologists, physiotherapists, and psychologists in both cases. We aimed to demonstrate the scope of the homoeopathic individualised approach for managing epilepsy and its role in improving the quality of life of patients and caretakers. Both cases involved detailed case-taking, including homoeopathic and clinical perspectives. A multidisciplinary integrated approach enabled a comprehensive clinical diagnosis. An individualised approach was used to form a totality and perform repertorisation to select the most appropriate remedy and potency. Follow-up criteria enabled us to measure the progress. The homoeopathic individualised approach resulted in considerable relief in seizures and a reduction in allopathic medication available only in 1st case. and relief in seizures in both the cases. The functioning of the patient and their quality of life improved considerably in both cases as well. Homoeopathic individualised approach, along with the integrated system, helps in the relief/cure of epileptic episodes, enables a reduction in the use of antiepileptic drugs, and improves associated co-morbidities. Different authors have tried to define symptomatology using their perspectives. However, as medical science evolved over time, so did the HMM. One needs to understand the changes and philosophy incorporated when writing the HMM. Dr. Cyrus Maxwell Boger, one of the stalwarts of homoeopathy, created Boger’s synoptic key (BSK) after integrating numerous concepts and his experiences. One needs to review the HMM before the first edition of the BSK to understand the concepts used by Dr. Boger to construct the BSK. Case-based learning (CBL), flipped classroom (FC), and group discussion (GD) methods have been found useful in delivering the process of integration in the classroom. A team of teachers (one from each six PG departments) and a coordinator conducted the sessions. The following steps were delineated: Identifying a suitable case, preparing assignments for individual FC sessions, individual feedback, and GD. The teaching was conducted over eight sessions of 90 minutes each. Learning from each session was consolidated in the form of PowerPoint presentations at the end of the session. Students were asked to write their experience of learning from each session, which the teaching team conducted. Combining the methods of CBL, FC, and GD in the backdrop of adult learning principles helped to deliver a well-rounded experience of practice in the classroom. This improved classroom engagement and developed critical thinking and problem-solving abilities in students. The method also helped to deliver deeper learning as students were actively involved throughout the process of learning. Implementing an integrated curriculum is challenging. A team of dedicated teachers with the common aim of delivering excellence in education willing to undertake the arduous journey with the students is essential. Minute planning and execution requires clarity of various clinical and homoeopathic concepts and their application. HTN in an adult is said to be present when clinical systolic and diastolic blood pressure exceed 140 mm and 90 mm of hg, respectively. An estimated 1.28 billion people aged 30–79 years have HTN worldwide, most (two-thirds) living in low- and middle-income countries (World Health Organization). Homoeopathy can be useful in managing HTN and its complications, for which a proper knowledge of treatment modalities available in homoeopathy is essential. An intensive search of articles, publications, and writings, both online and offline, dealing with the management of essential HTN through homoeopathy was conducted between November 2021 to January 2022. Databases such as Google Scholar, PubMed, ScienceDirect, and various other published books, literature, and anecdotes in homoeopathy were explored, collecting data on the role of homoeopathy in essential HTN. Emphasis was on words such as homoeopathy, HTN, and blood pressure. Management of essential HTN through homoeopathy can be broadly divided into two categories. First, drugs or medicines given in homoeopathic literature, such as Materia Medicas and Repertories, whose choice of administration in HTN depends on the classical methods of prescription, and secondly, drugs or medicines obtained from research outcomes on HTN. In homoeopathic therapeutics, many medicines are available and used in cases of essential HTN. Recently, research activities targeted toward managing HTN through homoeopathy have increased, and data have been generated.
