Vasoplegic syndrome, reported between 9 and 44%, is characterized by severe hypotension,severe decrease in systemic vascular resistance, decreased arteriolar reactivity, need for increased volume, and decreased response to norepinephrine, despite normal cardiac outflow.1,2 Vasoplegia is associated with high mortality and morbidity, especially in pediatric patients. Prolonged use of ACE inhibitor, calcium channel blockers, use of amiodarone and heparin, low cardiac reserve (EF <35%), symptomatic congestive heart failure and diabetes mellitus are among perioperative risk factors in adults.3 On the other hand, preand post-CPB hematocrit, pre-bypass mean arterial pressure (MAP), surgery type, duration of bypass, use of antifibrinolytic and/or vasopressor before bypass, hypothermia and needed dose of vasopressin for terminating bypass have been found intraoperative risk factors in pediatric cases.4","PeriodicalId":14916,"journal":{"name":"Journal of Anesthesia & Critical Care: Open Access","volume":"29 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2020-05-21","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"79446045","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Developing a retractable scar in the anterior neck which progressed, so it was scheduled by plastic surgery service for scar release plus the application of anterolateral free thigh flap. Anesthetic management was given with general anesthetic with intubation in an awake patient with flexible fibroscopy, based on fentanyl and intravenous propanidid, once vocal cords are crossed with the fibroscope endotracheal tube is introduced and balanced general anesthesia is provided. Surgical anterolateral free thigh flap procedure ends, endotracheal tube is removed, without complications. In the late postoperative period, the patient presents favorable evolution with adequate flexion and cervical extension with the use of the flap.","PeriodicalId":14916,"journal":{"name":"Journal of Anesthesia & Critical Care: Open Access","volume":"20 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2020-05-05","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"80707330","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Materials and Methods: After securing Institutional Review Board approval, the study was registered via ClinicalTrials.gov (NCT 0307996). The Siriraj Anesthesia Quality Assessment Questionnaire was developed and validated under the conceptual/theoretical framework of Maslow’s hierarchy of needs, Hospital Consumer Assessment of Healthcare Providers and System, Patient Satisfaction Questionnaire, Service Quality Questionnaire and Quality of Recover after Anesthesia. A total of 476 qualified responses with obtained consent were included in the study to verify the appropriateness of the tool. Results: The study involved 397 qualified patients (83.4%), aged 47.6±16.6, representing all anesthesia services. Significantly, a binary logistic regression revealed the predicted dependent variables as Process of Care (4.39±0.51) and Physical Support (4.35±0.66) in comparison with a sequence of Accessibility and Convenience (4.33±0.56), Communication and Information (4.00±0.70), Technical Quality (4.46±0.45) Psychological Support (4.57±0.65) and Loyalty (4.52±0.68) in anesthesia quality assessment. Conclusion: Process of Care and Physical Support appeared to be significantly valued features, whereas Communication and Information, Accessibility and Convenience, Technical Quality and Psychological Support were found to be weak points in anesthesia quality assessment.","PeriodicalId":14916,"journal":{"name":"Journal of Anesthesia & Critical Care: Open Access","volume":"1 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2020-02-26","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"88685240","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
