The patient started to experience parasomnia such as nightmares, raves, difficulty to sleep and awakened from sleep (for one month from the beginning of wound treatment and debridement). The patient also had two re-experiencing symptoms: nightmare and emotional distress (anxiety, anger) after a traumatic stimulus. From his temperament scale and character assessment, there was dysregulation disorder. From the PEDSQL parent's reports, there was also decreased quality of life. \u0000Conclusion: First-line therapy of PTSD for the pediatric population is supportive psychotherapy with trauma-focused cognitive behavioral therapy (TF-CBT) that results in a remarkable improvement in child's psychiatric outcome.

One way to reduce mortality is by blocking the progression of the disease. Transforming Growth Factor-β1 (TGF-β1) is a marker of progression from CKD through its fibrosis pathway. One mechanism for interfering with the TGF-β1 signaling pathway is by giving vitamin D3(cholecalciferol). This study aims to find out that supplementation of vitamin D3 can increase plasma 25(OH)D levels and reduce TGF-β1 levels in children with CKD. \u0000Methods: The study was approved by the hospital's ethical committee, designed as a randomized clinical trial, double-blind, pre and post-test control group, which involved 30 subjects diagnosed as CKD stage 1-3. Subjects were divided into two groups; the vitamin D3-treated group and the placebo group. TGF-β1 and vitamin D plasma level was measured by ELISA method. \u0000Results: Vitamin D levels increased by 9.35 compared to the placebo group. The magnitude of the increase from the Wilcoxon statistical test results obtained p=0.001 (p<0.05), so there was a significant increase in vitamin D levels after supplementing vitamin D3. In the group given vitamin D3 supplementation, it was found that TGF-β1 levels had decreased by 108.64. The reduction from the Wilcoxon statistical test results obtained p=0.001 (p<0.05), so there was a significant decrease after vitamin D3 supplementation. Based on the Spearman correlation test, there is a meaningful relationship between increased vitamin D levels with a reduction of TGF-β1 levels. The correlation coefficient is -0.753, which is harmful. The correlation coefficient is in a substantial range. \u0000Conclusion: We concluded that vitamin D3 could increase 25(OH)D plasma level and decrease TGF-β1 levels significantly in children with stage 1 until 3 of CKD.

However, some develop critical conditions and require mechanical ventilation in the Pediatric Intensive Care Unit (PICU). Various modalities are recommended for mechanical ventilation, such as High Flow Nasal Cannula (HFNC), Continuous Positive Airway Pressure (CPAP), or invasive ventilation with intubation. This study aims to describe the clinical feature, ventilation modalities usage, and the outcome of children with critical COVID-19. \u0000Methods: This is a retrospective study in COVID-19 children with respiratory distress who were treated in the COVID-19 isolation PICU room of Saiful Anwar General Hospital for one year. The data was gained from the medical record and analyzed descriptively. Data were analyzed using Ms. Excel for Windows. \u0000Results: A total of 51 children with COVID-19 were admitted to Saiful Anwar general Hospital in one year period, with 12 of them in critical condition and 6 children require mechanical ventilation. The main signs developed were fever and dyspnea. Invasive mechanical ventilation applicated in 5 patients, and only 1 patient received Non-Invasive Ventilation (NIV). The mean of PEEP used in invasive ventilation is 7-9 cmH2O, lower than ESPNIC’s recommendation of 8-10 cmH2O. Congenital hypothyroidism must be diagnosed immediately because delay in therapy may cause irreversible neurological deficits, such as mental retardation in children. However, this condition can be prevented by screening in newborns so that congenital hypothyroidism can be diagnosed and treated early, and can result in better neurological development. This study aims to determine the congenital hypothyroidism ratio in Malang from March to November 2020. Methods: The descriptive analysis method was used in this research. The subjects were neonates aged 0 – 28 days with a gestational age of 36 weeks and were involving the Health Office in both the Regency and the City of Malang. The data about identity, gender, age at screening, and gestational age were collected. A laboratory screening test was performed by taking a blood sample from the heel or vein for a TSH test. If the result was positive with a TSH value > 20 U/mL, the subject would be recalled for having a confirmation test and referred to a pediatric endocrinologist. Results: There were 2024 (88.34%) subjects were successfully screened from a total of 2291 subjects. The recall rate for congenital hypothyroidism was 1:2024 (0.04%). Conclusion: The ratio of congenital hypothyroidism cases in Malang from March to November 2020 was higher than the global ratio, which was 1:3000. The high rate of congenital hypothyroidism in Malang indicated the need for congenital hypothyroidism screening in newborns.
