The patient underwent multiple debridements, resulting in a critical bone defect. Reconstruction of the first ray with 1MTPJ arthrodesis was performed using a vascularized medial femoral condyle free flap (MFC-FF). A novel reconstruction involved MC resection, midclavicle distraction osteoplasty (DO) with iliac crest bone graft, and sternoclavicular (SC) joint reconstruction using a semitendinosus tendon allograft. By 1 year, the patient achieved successful healing and functional recovery.</p><p><strong>Conclusion: </strong>Patients with excessive MC bone loss may require a clavicular lengthening to approximate the MC to the manubrium. A 50-year-old man presented with a nonhealing posterior heel wound and persistent discharge after multiple surgeries, resulting in complete tendon loss. Imaging confirmed osteomyelitis. He underwent staged management with radical debridement and antibiotic-impregnated polymethylmethacrylate beads, followed by removal. No further reconstruction of the Achilles tendon was performed, yet the patient had restored plantarflexion and functional gait.</p><p><strong>Conclusion: </strong>Aggressive debridement and staged local antibiotics are essential. Both wrists ultimately became symptomatic and were treated with proximal row carpectomy. The patient had durable pain relief in both wrists after proximal row carpectomy.</p><p><strong>Conclusion: </strong>Bipartite lunate is a rare developmental anomaly that is usually asymptomatic. Operative intervention directed at treating involved osteoarthritic joints is warranted in the rare symptomatic patient. Acute 3-cm forearm shortening restored tendon tension and reduced defect size, enabling primary tendon reconstruction with pull-in sutures and nerve repair. Early staged free vascularized fibular grafting was performed. Within 6 hours, surgical reduction of the femoral head and internal fixation of both femoral neck and acetabulum were performed using a dynamic hip screw and a plate, respectively. The tumor encased the thoracoabdominal aorta, presenting a significant challenge for en bloc resection. We used a novel anterior transperitoneal and transdiaphragmatic approach. By longitudinally incising the diaphragm along the aortic hiatus, we obtained direct visualization to safely dissect the aorta from the tumor in both thoracic and abdominal cavities. This facilitated a successful 2-stage en bloc spondylectomy and reconstruction with a three-dimensional-printed vertebral body. The patient was found to have a partial quadriceps tendon rupture, lateral and medial-sided soft-tissue injuries, and avulsion of the anterior and posterior cruciate ligaments. Examination showed calcaneal tenderness, and MR imaging revealed a posteroinferior calcaneal lytic lesion suggestive of osteomyelitis. Cultures were negative, but histopathology confirmed Rhinosporidium seeberi. At the meniscal repair surgery following UKA, there was anterior translation of the lateral meniscus with anterior and posterior popliteomeniscal fascicle tears.
