Advanced bilateral nuclear cataract was identified, and cataract surgery was performed when the patient was 7 months old. After surgery, she began </span>amblyopia treatment using extended-wear </span>soft contact lenses<span> (SCLs). When she started school, her corrected distance visual acuity<span> was 20/20 in both eyes. The exotropia<span> was complicated by dissociated horizontal deviation and dissociated vertical deviation. After 10 years of age, the SCLs were chosen to achieve crossed monovision. The binocular visual acuity was 20/28 or better at distance and at near, and the asymmetric dissociated vertical deviation became less apparent. The treatment produced an acceptable appearance when the patient was looking at a far distance (ie, distance of contact with other people). She was seeking vision correction. The options were limited because of the </span>anisometropia<span><span>, corneal neovascularization, and high astigmatism. The manifest refraction was −3.00 +4.00 × 115, giving a corrected distance </span>visual acuity of 20/25. The refraction and corneal condition were followed and remained stable for 2 years; therefore, a toric implantable collamer lens (ICL) was selected. The uncorrected distance visual acuity improved from 20/70 to 20/25</span></span></span><sup>−1</sup> and continued to remain stable more than 18 months postoperatively. In 2015, he had DSAEK in Nagoya, Japan, after examinations showed the presence of </span>bullous keratopathy<span> in the right eye. In August 2017, he was referred to the Baptist Eye Institute in Kyoto, Japan, because the DSAEK graft had failed. The failed graft and the pIOL were extracted, after which lensectomy<span>, IOL implantation, and anterior capsule staining were performed. A fresh DSAEK graft was attached to the posterior corneal surface via an air injection. By 6 months postoperatively, the corrected distance visual acuity had improved from 20/2000 to 20/20. On examination, she was found to have advanced cataracts with a prominent posterior subcapsular component. Because her preoperative blood sugar levels had become elevated while on ziprasidone, the patient discontinued the medication before uneventful cataract surgery in the right eye. Postoperatively, the blood sugar level was improved; simultaneously, she noticed an improvement in vision in not only her operated eye but also her unoperated left eye. Examination showed near-complete resolution of the cataract in the left eye. In this case, a 60-year-old woman presented with progressive decreased </span>visual acuity and increased cloudiness, especially in the morning, in both eyes (left &gt; right). She had goniotomy and </span></span>strabismus surgery<span><span> in 1957 when she was 6 months old. Six months after the triple DMEK procedure was performed in the left eye, the corrected distance visual acuity was 20/40, the </span>intraocular pressure<span> was 14 mm Hg, and the central corneal thickness was 452 μm. Slitlamp biomicroscopy showed a clear cornea with no signs of </span></span></span></span>graft rejection<span>. The patient was diagnosed with advanced keratoconus<span><span> in the right eye; the left eye was normal. He presented with acute corneal hydrops after 8 months. An intracameral perfluoropropane injection was given; </span>penetrating keratoplasty was performed 3 months later. As a result, creation of the anterior surface of the lenticule was not complete. One month later, a cap was created using the flap mode; the flap side cut was manually stopped after the flap cut was made. After laser in situ keratomileusis flap creation, the refractive lenticule was manually removed using microforceps, similar to the uneventful SMILE procedure. Three months before the patient presented to our clinic, the microkeratome cut outward during LASIK flap creation, amputating the partially cut flap across the visual axis. Without a flap cut into the preserved one half of the cornea, complete flap amputation was not considered a safe option. Topography-guided transepithelial PTK was used to regularize the cornea and treat the resulting irregular astigmatism. Clinical examination and anterior segment optical coherence tomography<span> showed a corresponding triple-layered transparent tissue in the anterior chamber<span> and behind the iris plane. Based on the evaluation findings and the evidence available in the literature, a possible diagnosis of spontaneously absorbed lens was made and secondary intraocular lens implantation in the bag was planned. However, the patient declined surgery and was lost to follow-up.
