{"title":"QRS波の増高と減高","authors":"Kotaro Takayasu, Koichi Inoue","doi":"10.5105/jse.42.237","DOIUrl":"https://doi.org/10.5105/jse.42.237","url":null,"abstract":"QRS波の振幅に影響を与える要素として,誘導電極の位置のほかに,1心起電力の大きさ,2心臓周囲組織, 3 internal cancellation効果などが挙げられる. 心起電力は,心臓が電流を発生させる能力の大きさを示す.発生する起電力が大きければ,QRS波の振幅は 大きくなる.正常では右心室より左心室の壁が厚く,心筋量も多く,起電力も大きいため,主に左心室の伝導 を反映している.心肥大においては,心筋量が増加するため心起電力が増加しうることから,QRS振幅増大の 原因となりうる.また,心筋細胞の減少,虚血や低酸素,心筋線維化,電解質異常(低 Na血症,高 K血症), 低体温,特定の薬物中毒(抗不整脈薬,ジギタリス,向精神薬など)では心起電力が減少するため,これらの病 態において QRS波の振幅は減高する. Abstract QRS波形は心室の心筋の脱分極の総和により形成されるが,その振幅の増高・減高は心電図でも重要な所 見の一つである.振幅は,1心起電力の大きさの異常,2心臓周囲組織の導体としての伝導率の異常,3 Internal cancellation効果(極性が反対の電位同士が打ち消し合って電位が低く記録される現象)の減弱, などで変化する.振幅が増高する疾患としては心肥大が最も多く,不良な予後と関連するため,振幅の増 高・減高は臨床上も重要である.ほかにも,後壁心筋梗塞や脚ブロック,顕性 WPW症候群などで QRS 波が増高しうる.減高に関しては疾患特異性が低いが,その他の随伴する心電図異常である程度の診断は 可能である.振幅の異常は特異度が高く,見逃しがないように注意したい.","PeriodicalId":14543,"journal":{"name":"Japanese Journal of Electrocardiology","volume":"42 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2022-12-23","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"73760980","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
