It predominantly affects the aortic valve followed by the mitral valve. It can be silent on presentation or can present with complications like devastating systemic embolization, myocardial infarction, and death. We describe a rare case of a young man hospitalized for the investigation of cerebrovascular events. On transesophageal echocardiography (TEE), a PFE was identified and resected successfully through a minimally invasive surgery. We analyzed the impact of sequential ITA grafts on late survival after BITA used on the left side. Methods: From 1989 to 2014, 1644 patients underwent BITA surgery without any additional arterial graft. The revascularization of the left side was optimized with a sequential ITA graft in 824 patients. The revascularization of the right side was performed with an associated vein graft in 599 patients. Propensity score was calculated by logistic regression model and patients were matched 1 to 1 leading to two groups of 334 matched patients. The primary end point was overall mortality from any cause. Results: The population was not homogenous: Greater the arterial revascularization, lower the risk profile. The 30-day mortality was 1.2% without influence of the surgical technique performed. The mean postoperative follow-up was 12.4 ± 6.7 years and 95% complete. Late mortality was significantly influenced by age, heart failure, 3-vessel disease, LV ejection fraction, number of arterial anastomoses and sequential ITA. The significant difference in patients’ survival observed at 20-years in favour of sequential ITA in unmatched groups was confirmed in matched groups. In multivariable Cox model analysis, the use of sequential ITA remained predictor of long-term survival in matched groups. Conclusions: These results confirm that higher the number of ITA anastomoses, better the long-term survival. The new method Flotrac/VigileoTM system calculates cardiac output (CO) by analyzing arterial pressure waveform and has recently been marketed which have the ability to monitor CO noninvasively. In this study we compared Flotrac/VigileoTM (FV) in patients undergoing valvular heart surgery with pulmonary artery cardiac output measurement. Nineteen patients, who would undergo isolated valve replacement surgery were included into the study. All patients were monitored with PAC and FV monitoring systems. Cardiac outputs were measured at four different stages of the operation: After induction of anesthesia (T1), before cannulation (T2), after cardiopulmonary bypass (T3) and after sternal closure (T4). There was no difference between demografic data of patients. CO measurements obtained with the FV system were higher for the T1, and T4 measurements; lower for the T2 and T3 than did those obtained with the PAC. For all measurement stages, good agreements were found between two methods as shown by Bland-Altman statistics. Although many studies are reported for the evaluation of the FV in the operating theatre we could find no studies that have examined this method in isolated valvular heart surgery patients. We compared two different CO measurement methods: FV and PAC in valve replacement surgery patients. The CO values provided by FV were similar to the PAC values that may be considered as gold standart. Extracorporeal membrane oxygenation (ECMO) in cardiac surgery has been established for cardiogenic shock refractory to conventional therapy. Veno-venous ECMO can be a rescue modality for ARDS post cardiac surgery. We present the case of a 73-year-old woman who was referred to our hospital with the diagnosis of acute Stanford type A aortic dissection to receive surgical treatment. She was subjected to successful aortic repair and evolved with severe ARDS, requiring VV-ECMO, which was weaned after 11 days.
