Angioleiomyoma has been defined as frequently painful, benign subcutaneous or deep dermal tumor composed of mature smooth muscle bundles which are surrounded and interlaced by vascular channels. Only 8.5% cases arise in the head and neck. A 39 years old male presented with swelling right side face and upper neck past five years. The swelling was non tender, immobile and non-compressible. There was lateral pharyngeal wall bulge which was pushing the tonsil medially. The patient underwent total conservative parotidectomy and histopathology (HPE) came out to be angioleiomyoma. Angioleiomyoma is difficult to diagnose correctly from clinical manifestations and imaging alone, and a biopsy is mandatory to make the diagnosis.

Atypical TB symptoms include non-specific symptoms and extrapulmonary TB, and patients with comorbidities may present with atypical TB due to impaired immune function. TB treatment includes antibiotics, which can cause hepatotoxicity, and require close monitoring for signs of liver injury. A 32-year-old female with intellectual developmental disorders was admitted to the hospital with suspected pneumonia and acute respiratory viral infection. Despite initial treatment with antibiotics, her oxygen saturation decreased, and she developed recurrent pneumothorax on the right side and neutrophilic pleuritis. Computed tomography (CT) scans showed increasing infiltrative changes, and bronchoalveolar lavage eventually revealed Mycobacterium tuberculosis DNA, leading to the diagnosis of tuberculosis pneumonia with pleurisy. This case report highlights the challenges of diagnosing TB and the importance of maintaining a high index of suspicion for TB in patients who do not fit the typical profile of the disease. The case also emphasizes the potential for drug-induced liver damage with TB treatment and the need for close monitoring. Early diagnosis and appropriate management are crucial to prevent further complications and improve outcomes in patients with TB.

It is considered to be a relatively rare disease. Most commonly the first symptoms are associated with ocular muscle weakness resulting in ptosis and/or diplopia that may be progressive during the periods of muscle exertion and resolve with rest. However, any skeletal muscle group may be affected leading to the variability of clinical symptoms and potential challenges in diagnostics. We present a case report of a 62-year-old male that initially presented with bulbar symptoms and unintentional weight loss, with atypical findings in electromyography study (the absence of decrement amplification in a combination of spontaneous muscular activity) – suggestive for amyotrophic lateral sclerosis (ALS) diagnosis. After a thorough investigation the diagnosis of ALS was not confirmed but myasthenia gravis was highly suspected and anti-MuSK antibodies came positive. The patient was prescribed Pyridostigmine, Prednisolone and underwent plasmapheresis procedure which led to significant relief of the symptoms.

(Oleaceae) are claimed to possess powerful central nervous system (CNS) depressant activity. Despite these traditional claims, no in-depth scientific study has been performed on the CNS depressant activity of the flowers of J. grandiflorum. Therefore, the present study was aimed at evaluating the CNS depressant activity of an ethanolic extract of the flowers of J. grandiflorum in Swiss albino mice. \u0000Methods: Acute oral toxicity tests were done at doses of 550, 1750, and 5000 mg/kg. The extract was also subjected to phytochemical tests and TLC tests. The CNS depressant activity of the EEJG was evaluated by various models, such as the forced swimming test, tail suspension test, thiopental sodium-induced sleeping time test, locomotor activity test, and muscle co-ordination test, at two different doses (500 and 1000 mg/kg) in Swiss albino mice. Diazepam (1 mg/kg) was used as a standard drug. \u0000Results: In acute oral toxicity studies, the extract was found to be safe up to a dose level of 5000 mg/kg body weight. EEJG at both doses (500 and 1000 mg/kg) showed significantly (p<0.05, p<0.01) increase in immobility time in the forced swimming test and tail suspension test. In the thiopental-induced sleeping time test, EEJG at 1000 mg/kg showed a significant (p<0.05) effect on the onset of action time and also significantly (p<0.01) increase the duration of sleeping time. EEJG at 1000 mg/kg showed a significant (p<0.01) decrease in locomotor activity, and the EEJG at both doses (500 and 1000 mg/kg) showed a significantly (p<0.05, p<0.01) decreased in muscle co-ordination activity when compared to control group. \u0000Conclusions: The present study confirms the significant CNS depressant activity of the ethanolic extract of the flowers of J. grandiflorum, which may be due to flavonoids and steroids present in the extract as phytoconstituents confirmed by TLC.
