It habitually prompts cognitive, physiological, and emotional reactions. Depression, anxiety, and stress are considered emotional states usually found in preoperative surgical patients. Objective: To find out the frequency and association of depression, anxiety, and stress with socio-demographic variables among preoperative patients. Methods: This cross-sectional analytical study of 258 preoperative patients was conducted at Dow University Hospital and Dr. Ruth Pfau Civil Hospital, Karachi from August 01, 2019, to July 31, 2020. Ethical approval was obtained before conducting the study. Depression, Anxiety, and Stress Scale-42 tool was used for the data collection. The data was analyzed by using SPSS version 21. Mean and frequency analyses were performed for demographic characteristics. Moreover, a chi-square test was performed to check the association of socio-demographic characteristics of study participants with depression, anxiety, and stress. Results: The majority of the participants (72.5%) in this study were male and 59.7% were between the ages of 20-35 years. The finding of this study revealed that most of the patients had moderate depression (43.02%), extreme anxiety (54.26%), and moderate stress 57.13%. Significant associations were found for depression with gender and monthly income, with a p-value of 0.035 and 0.003 respectively. Gender (p=0.021) and level of education (p=0.013) were significantly associated with stress levels. Conclusion: This study concluded that patients before surgeries had different levels of depression, anxiety, and stress. Age is the only variable that showed a significant association with stress and depression. Some diabetic patients present with complaints of epigastric pain, vomiting, nausea, or postprandial fullness, however only a few have severe symptoms. Objective: To determine the frequency of dyspepsia among T2DM patients. Methods: This cross-sectional study was performed in Ziauddin Hospital Kemari Branch, Karachi, Pakistan after acquiring ethical approval from the hospital ethics committee. Short-Form Leeds Dyspepsia Questionnaire (SF-LDQ) was used to identify the presence of dyspepsia. Based on the sum of the frequency of symptoms, a cut-off value of 4 and above was used for the presence of dyspepsia. Results: A total of 368 patients were included in the study with a median age of 68.6 ± 14.2 years. The majority were females (62.8%). The mean duration of diabetes was 15 ± 10.4 years. The frequency of indigestion, heartburn, regurgitation, and nausea was 35.3%, 30.7%, 31.5%, and 11.4% respectively. Out of 368, 168 (45.7%) patients were found to have dyspepsia. The risk of dyspepsia was significantly lower among younger age, male gender, and patients having controlled glycemic status, patients doing physical activity twice a week, taking all three meals on time, and always having their breakfast early. The odds of having dyspepsia were higher among those who are currently and formerly addicted to alcohol, current and former smokers, having comorbidity, consuming <8 glasses of water per day, and those who were always or sometimes sleeping within two hours of having dinner. {"title":"Managing Hepatic Encephalopathy - Combination of Lactulose and Rifaximin Versus Lactulose Monotherapy: A Systematic Review and Meta-analysis","authors":"","doi":"10.37184/lnjpc.2707-3521.6.15","DOIUrl":"https://doi.org/10.37184/lnjpc.2707-3521.6.15","url":null,"abstract":"Background: Hepatic Encephalopathy (HE), a neuropsychiatric complication of hepatic failure, is currently managed with lactulose as a first-line treatment followed by other adjuncts if needed. In this meta-analysis, we determined the effect of lactulose and rifaximin combination in terms of efficacy and mortality reduction compared to lactulose alone. Materials and Methods: We searched databases (PubMed, BioMed Central, and Cochrane-Central) until July 2022 for original studies inspecting the effects of Rifaximin and Lactulose (combination therapy) vs. lactulose as a monotherapy in the treatment of HE on outcomes of clinical efficacy, hospital stay length, HE recurrence, drugs’ side effects and mortality. Data was analyzed via Review Manager (version 5.4.1) and OpenMetaAnalyst. Relative risks (RR) and weighted mean differences (WMD) with 95% confidence intervals were calculated. Results: Fifteen studies with 4327 patients were included. Pooled analysis showed combination therapy to be associated with a significantly lower mortality rate in patients having HE when compared to lactulose alone (RR 0.71 95% CI 0.58-0.88, P=0.002, I2= 68%), and clinical efficacy was also improved in the combination group (RR 1.33, 95%CI 1.19-1.48, P <0.00001, I2= 52%). HE recurrence rate, adverse events, and length of hospital stay did not significantly differ among the two groups (RR= 0.61, 95 % CI= 0.35 to 1.05, P= 0.08, I2= 84%), (RR= 0.92, 95% CI= 0.51 to 1.69, P= 0.80, I2 = 0) and (WMD −1.52, 95% CI −3.22 to 0.18, P=0.08, I2 = 83%) respectively. {"title":"Sensitivity and Specificity of C-Reactive Protein Against Blood Culture in Patients with Neonatal Sepsis - A Hospital-Based Study","authors":"","doi":"10.37184/lnjpc.2707-3521.6.18","DOIUrl":"https://doi.org/10.37184/lnjpc.2707-3521.6.18","url":null,"abstract":"Background: Neonatal septicemia is one of the primary causes of disease and death in low-income countries which makes early diagnosis an important factor to prevent microbial resistance and initiate antibiotic therapy. C-reactive protein is an essential biological indicator that might play an important role in the early detection of neonatal sepsis. Objective: The present study was conducted to determine the sensitivity and specificity of CRP against blood culture in the identification of neonatal sepsis. Methods: This study was carried out on 416 neonates identified with sepsis at the NICU and SICU of the National Institute of Child Health, Karachi, Pakistan for a span of 06 months from April 2022 to September 2022. The first blood sample for the evaluation of CRP and blood culture was drawn at the time of admission. The second sample of CRP and blood culture was drawn 72 hours after the first blood collection. The sensitivity and specificity of CRP were calculated on both occasions. Results: Of the total 416 neonates, 165(39.6 %) patients’ blood culture was positive and in 273 (65.9%) patients CRP was positive. The validation study of CRP in the diagnosis of neonatal septicemia reported sensitivity, specificity, positive predictive value, and negative predictive value to be 87.1%, 45.9%, 52.6%, and 83.7% 72 hours after the admission in comparison to 80.6%, 44.2%, 48.7%, and 77.6% at the time of admission. Multiple factors are responsible for inappropriate use of PPIs including self-medication, less knowledge about PPIs in medical staff, and consumer-oriented advertising.\u0000\u0000Objective\u0000To assess the knowledge and practices about PPI use among Physicians in Tharparkar District of Sindh province, Pakistan.\u0000\u0000Methods\u0000This is a cross Sectional study that was done in the rural hospitals (Basic Health Unit, Rural Health Centre, Talukka, and District Hospital) in Tharparkar District Sindh. The study took place for 6 months from October 13, 2019, to April 12, 2020. All PMC licensed Physicians working in district Tharparkar for at least 6 months were consecutively enrolled. Knowledge questions comprised of questions about PPI and their indication and side effects while practice questions such as indication, route, and duration of action were asked. All participants were classified as having adequate knowledge and adequate practice based on the correct answer to each question. The study included PMC-licensed physicians with one-year internship/ house job experience and Physicians who had worked at district Tharparkar in a rural setting for at least 6 months. Physicians who did not give consent were excluded from the study.\u0000\u0000Results\u0000Of 102 practitioners, the mean age of the participants was 39.57 ±9.14 years. The majority of the practitioners were males (n=82, 80.4%). Most of the practitioners (n=66, 65%) were working in district hospitals, 21 (19.6%) in rural health centers, and 15 (14.7%) in basic health units. Diabetes can give rise to many secondary ailments among which heart failure is the most common. Heart failure brings about confirmatory changes in the myocardium which shifts the metabolism from glucose to fatty acid which further impairs cardiac function. Among the antidiabetic agents, SGLT2 inhibitors such as empagliflozin and dapagliflozin have decreased heart failure exacerbations even in the absence of diabetes. We employed search engines such as Google Scholar, MEDLINE, and Euro PMC for our article search. The keywords such as heart failure, SGLT2 inhibitors, and diabetes were used. Duplicate studies were excluded during our analysis. According to the literature, several theories have been put forward on the mechanisms through which SGLT2 inhibitors can work in heart failure.
