A pharmacoinvasive strategy involving immediate fibrinolysis followed by transfer to a PCI center to perform angiography (and PCI of infarct-related arteries) within 2 and 24 hours after successful fibrinolysis offers immediate reperfusion and better outcomes in patients in whom primary PCI could not be achieved in a timely manner without an increase in bleeding complications. Dogs are a major source of rabies infection in human beings. Rabies is 100% fatal but largely preventable if proper and timely Post-Exposure Prophylaxis (PEP) is availed in the form of immediate wound wash with soap and water and injections of vaccine and rabies immunoglobulins, the first week being the most important. Here we describe four cases of PEP failure despite complete prophylaxis with vaccine and Rabies Immunoglobulins (RIG) over the past year in Himachal Pradesh. All failures were due to suspected rabid dog bites on the face, especially on cheeks, lips, or eyes where complete infiltration of RIG may not have been possible leading to PEP failure. Method: The place of study was Lok Nayak Hospital located in New Delhi. The type of study was prospective observational. The incorporation of 112 subjects was done for the study on the basis of inclusion criteria. Informed consent was obtained from the patients after which they underwent rigorous history taking, general and systemic examination and relevant investigations. Interpretation and compilation were done with MS Excel (R) Office 365, GraphPad Prism 8.4.2 and Statistical Package for Social Sciences version 25. Seroprevalence was estimated by measuring the COVID-19 antibodies in these patients’ serum with an ELISA kit. Results: Seroprevalence in the current study came out to be 13.39%, which was significantly lower as compared to the overall pooled seroprevalence of India. This was due to better knowledge, awareness and care taken by these patients towards the disease during the pandemic times. COVID-19 antibody index was in low ranges in patients with haemophilia (PWH) and further exploration and subsequent seroprevalence studies are needed to monitor the antibody index. Conclusion: Many factors contribute to the seroprevalence of COVID-19, including demographic profile, occupation, socio-economic status of the country, and personal protective measures taken. {"title":"A Case of Sheehan Syndrome Presenting with Pancytopenia and Dilated Cardiomyopathy: A Rare Combination","authors":"Manisha V Prabhu","doi":"10.24321/2349.7181.202214","DOIUrl":"https://doi.org/10.24321/2349.7181.202214","url":null,"abstract":"Sheehan syndrome is a rare complication of postpartum haemorrhage resulting in pituitary necrosis and panhypopituitarism. We report a case of a 52-year-old female who presented with easy fatigability, asthenia and loss of appetite for 8-10 months. She was found to have alopecia, hypotension, hypoglycaemia, pancytopenia, and a history of failure to lactate after the last childbirth (17 years back) with premature menopause. The hormone profile revealed panhypopituitarism and magnetic resonance imaging of the brain was suggestive of partially empty sella. Her echocardiogram showed dilated cardiomyopathy with an ejection fraction of 35%. A diagnosis of Sheehan syndrome presenting with pancytopenia and dilated cardiomyopathy was made. The patient was started on injectable hydrocortisone and later, on thyroid hormone supplementation after which she improved.
