{"title":"Femoroasetabular sıkışma sendromlu hastalarda pelvik bölgenin radyolojik anormallikleri ve klinik sonuçlar","authors":"Tuba Erdem Sultanoğlu, Sarfinaz Ataoğlu, Hasan Baki Altinsoy, Hasan Sultanoğlu","doi":"10.26559/mersinsbd.840586","DOIUrl":"https://doi.org/10.26559/mersinsbd.840586","url":null,"abstract":"Aim : In our study, we aimed to examine the clinical outcomes and radiological changes in the pelvic region in patients with femoroacetabular impingement syndrome; to emphasize the importance of femoroacetabular impingement in the etiology of hip pain and conservatif management. Methods : The sociodemographic and clinical characteristics, abdominal and pelvic computed tomography scans, pelvic magnetic resonance imaging of 104 patients diagnosed with femoroacetabular impingement syndrome were retrospectively analyzed. Results : 41.3% of the patients were female, 58.7% were male and the mean age was 52.78 ± 13.18. 37.5% of the patients are housewives; 29.8% are not working; 28.8% were working and 3.8% were students. The proportion of patients with hip pain was 76%. 31.7% had pain in the right hip; 22.1% had pain in the left hip and 22.1% in both hip regions. Activity modification and medical therapy for 17.3% of the patients; medical treatment and home exercise program in 32.7%; physiotherapy was applied in 26.0%; no treatment was applied in 24.0%. 54.8% of the patients were diagnosed by tomography and 45.2% of them were diagnosed by magnetic resonance imaging. According to radiological results, 80.8% of them had cam; 5.8% had pincer and 13.5% had mixed type of femoroacetabular impingement. In 53.7% of asymptomatic patients and in 61.8% of the patients with pain complaints had additional radiological changes in the pelvic region. Accompanying radiological changes in the pelvic region were mostly subcortical cysts and sclerosis. Conclusion : Femoroacetabular impingement syndrome, which is one of the important factors in the development of hip osteoarthritis, should be considered when investigating the causes of hip pain. Protection of the hip joint and increasing the patient's quality of life should be aimed with early diagnosis and effective treatment. {"title":"Toplum kaynaklı pnömoni tanısı ile yoğun bakıma yatışı yapılan erişkin hastalarda antibiyotik kullanımı ve mortalite oranları","authors":"Havva Kubat, Arzu Pinar, Halime Eker, S. This study was conducted in order to contribute to the creation of effective treatment protocols by determining the profile of patients who can be treated with a diagnosis of community-acquired pneumonia requiring intensive care and who do not respond to treatment. Method: The data of 100 adult patients who were hospitalized in the intensive care unit with the diagnosis of community-acquired pneumonia in Adana City Training and Research Hospital were retrospectively analyzed then recorded. Results: Of the patients participating in the study, 30% were female and 70% were male. The survival rate of the patients was 75.0%, the mean age of the patients who died was 74.4±13.5 years, and the mean age of the patients who recovered was 66.6±18.2 years. It was noted that 95% of the patients had one or more chronic diseases. It was determined that empirical antibiotic treatment was continued in 59% of patients, antibiotics were changed for various reasons in 41%, and the most common reason for changing antibiotics was the increase in serum C-reactive protein level. Growth was detected in sputum and aspirate culture samples in 31% of the patients. Combined antibiotic therapy was started in 81% of the patients, and single antibiotic therapy was started in 19%. The most commonly used empirical antibiotic is cetriaxone+moxifloxacin or levofloxacin combined therapy. Bilateral lung involvement was noted in 78% of the patients, and pleural effusion developed in 39%. A 70% decrease was found in serum C-reactive protein levels and a 67% decrease in serum white blood cell levels measured for three consecutive days. Mortality was found to be significantly lower in patients with decreased serum C-reactive protein and white blood cell levels (p<0.001, p=0.001, respectively). Conclusion: In this study, the relationship between clinical and laboratory features and mortality in patients diagnosed with community-acquired pneumonia was investigated.
