The diplopia started two months prior to the initial visit while the eye started to crossed in. The patient also complained of severe daily headaches along with ringing in her ears. She believes the symptoms started while she was recovering from shingles on her right side of the forehead. Prior MRI imaging was reported to be unremarkable. The patient was referred to neuro-ophthalmology service with possible diagnosis of thyroid eye disease given the red eye and esotropia. Patient had no history of other medical conditions except shingles in trigeminal distribution that occurred about 4-6 weeks before the onset of her symptoms. Central VA was recorded to be unaffected at 20/20 in both eyes with correction. On sensorimotor exam, there was severe horizontal diplopia at the primary, increasing in left, and right gazes due to bilateral VI nerve palsy. Intraocular pressure testing 22mmHg right eye and 19mmHg left eye, indicating ocular hypertension in the right eye. She had corkscrew blood vessels in both eyes. Bouchard, Shaozhen Zhao","doi":"10.33140/jocr.06.04.01","DOIUrl":"https://doi.org/10.33140/jocr.06.04.01","url":null,"abstract":"Purpose: The purpose of this study was to determine how objective and subjective parameters of dry eye disease correlate with objective vision quality changes following femtosecond laser in-situ keratomileusis (FS-LASIK) surgery in low and high myopes. Methods and Materials: This prospective, observational case series recruited patients undergoing bilateral FS-LASIK. Patients were divided into low/moderate and high myopia groups based on pre-operative assessment. Objective and subjective measurements of dry eye, as well as measurements of vision quality were obtained pre-operatively, 1-week, 1 month, and 3 months post-operatively. Thirty patients were included. The low/moderate and high myopia groups each contained 15 patients. Results: Non-invasive keratography tear break up time (NIKBUT) decreased after FS-LASIK in both groups. Objective scatter index (OSI) improved in the L/M myopia group but worsened in the H myopia group post-operatively. Modulation transfer function (MTF) cutoff improved in both groups post-operatively. A negative correlation was identified between the NIKBUT and both the OSI and MTF results. Changes in ocular surface disease index (OSDI) scores support these objective findings. Conclusion: The results of our study provide evidence that ocular surface changes associated with dry eye contribute to reduced subjective vision quality after FS-LASIK. Methods: This case report showed an intense progressive flattening of keratometric parameters in a patient without risk factors such as high maximum keratometry, thin cornea, uncontrolled ocular allergy, collagen, or rheumatologic diseases. Even with significant haze formation and progressive flattening, the patient exhibited progression after a long follow-up period. Results: The patient presented with grade 2 haze associated with progressive flattening of approximately 10 D in keratometric parameters (Kmax, K1, K2, and Kmean) during the first 5 years after surgery. Corneal opacity altered the minimum pachymetry reading on Pentacam to 265 microns. Anterior corneal follow-up optical coherence tomography revealed a thickness of 453 microns. The patient developed low visual acuity while wearing glasses. The scleral lens was fitted, allowing a visual acuity of 20/30, with satisfactory visual quality. Keratometric stability was observed between the fifth and tenth year. In the tenth year, the patient maintained haze, but there was a progression of 1.7 D in the Pentacam comparative map. Conclusions: Long-term monitoring after corneal crosslinking should be performed. Imaging was done, which revealed pineal neoplasm with obstructive hydrocephalus. In ophthalmology, nanoparticles are of special interest as carrier systems for the targeted delivery of drugs, genes or siRNA to the anterior as well as to the posterior segment of the eye. Above that, sustained drug release plays a major role for the treatment of many ophthalmic diseases and nanoparticles have the potential to perform this task without inducing local adverse reactions. Nanoparticles are primarily defined by their size that has to range below 100 nm. All other characteristics are variable. The large variety of nanoparticle types and the fact, that they are easily modifiable allows their adaption to the task set for them.
