Awareness of the importance of nutrition in the management of cirrhosis is critical to help improve clinical outcomes in this frequently frail patient population. Managing malnutrition before liver transplantation while on the waiting list and, metabolic disturbances in post-surgery are still a challenge. The purpose of this review is to support an interdisciplinary nutrition approach of these patients. Cirrhotic patients are frequently malnourished before liver transplantation and this is associated with a poor prognosis. Although the relation between nutritional status versus survival, successful operation and recovery after liver transplantation is well established, prevalence of malnutrition before the operation is still very high. The diagnosis of the nutritional status is the first step to address the adequate nutritional therapy. {"title":"Situación de los receptores de trasplante de órganos sólidos con COVID-19 en México","authors":"José Salvador Aburto-Morales, José André Madrigal-Bustamante, Josefina Romero-Méndez, Cinthya Ayerim Lucio-García","doi":"10.35366/94514","DOIUrl":"https://doi.org/10.35366/94514","url":null,"abstract":"Introduction: Transplanted patients who receive immunosuppressants are at a higher risk of getting COVID-19 infection, of presenting complications in its evolution and the concomitant probability of rejection of the transplanted organ, besides the difficulties for its prevention, prompt detection, monitoring and control. It is relevant to identify the status and evolution of transplant recipients who acquired COVID-19 disease in the Mexican case. Material and methods: It is a cross-sectional study, for which information was collected between April 1 and May 25, 2020 in Mexico. A descriptive analysis of simple and relative frequencies as ratios, proportions and rates was performed from primary sources of information, to identify the general characteristics of the recipients of transplantation with COVID-19. Results: 28 confirmed cases of COVID-19 were reported in transplant recipients. Of these, the majority occurred in men (57.14%), in recipients from Mexico City (78.57%), in facilities from the Ministry of Health (46.43%) and corresponded to kidney transplantation (92.86%). The most affected age group was 45-64 years. Of the total cases, eight died, it has been estimated a case fatality of 28.6%. Conclusions: The data may suggest that COVID-19 disease in solid organ transplant recipients could have a more severe course compared to the general Mexican population. One of the factors influencing the large difference between supply/demand is the shortage of organs secondary to family refusal. Objective: Analyzing family refusals, establishing the reasons why family refusal is given to the donation of organs and tissues for transplant purposes, in the North Zone General Hospital «Bicentennial of Independence». Material and methods: This study is qualitative, descriptive, observational and retrospective. Carried out at the North Zone General Hospital «Bicentennial of Independence», considering the family refusal to organ donation during the period from March 2019 to March 2020. Results: The sample consisted of 54 cases of potential donors in which family negative was obtained for donation. The reasons that prevailed in family refusals to the application for organ donation were: the belief that the body should be completely buried (21.29%), the family member did not express his wish in life (17.59%), the family member informed him in life that did not want to donate (14.81%), belief that it was not the desire of the family member to donate (9.25%), the desire not to make decisions at that time (7.4%), not to reach a family agreement (5.55%). Conclusion: The results obtained show the reasons why the relatives of potential donors refuse to access the donation. Despite efforts made by the staff in charge, denial of organ donation is a current problem.
