When treating periimplantitis, we should attempt to manage this problem via nonsurgical therapies that include addressing all modifiable systemic risk factors and local contributing factors. Hence, the aim of this narrative review was to examine published studies on nonsurgical treatment of periimplantitis and evaluate their effectiveness and limitations.</p><p><strong>Materials and methods: </strong>A literature search was performed in MEDLINE via PubMed database up to December 31, 2017. Current published clinical approaches focused on mechanical debridement, adjunctive antiseptic therapy, adjunctive antibiotic therapy, laser-assisted therapy, and combination approaches were included in this analysis.</p><p><strong>Results: </strong>Nonsurgical therapy of periimplantitis may result in complete healing of the disease and the patient is then placed on a supportive maintenance program. If the disease is not resolved and surgical intervention is not an option, active nonsurgical retreatment may be considered. In many cases where disease is not resolved, surgical therapy or implant removal could be considered.</p><p><strong>Conclusions: </strong>Nonsurgical treatment of periimplantitis usually provides clinical improvements in reducing bleeding tendency and in some cases pocket reduction. {"title":"Surgical Treatment of Periimplantitis With Non-Augmentative Techniques.","authors":"Philip L Keeve,&nbsp;Ki Tae Koo,&nbsp;Ausra Ramanauskaite,&nbsp;Georgios Romanos,&nbsp;Frank Schwarz,&nbsp;Anton Sculean,&nbsp;Fouad Khoury","doi":"10.1097/ID.0000000000000838","DOIUrl":"https://doi.org/10.1097/ID.0000000000000838","url":null,"abstract":"<p><strong>Objectives: </strong>The aim of this review was to systematically screen the literature on surgical non-regenerative treatments of periimplantitis, especially for radiologic and clinical outcomes, and to determine predictable therapeutic options for the clinical management of periimplantitis lesions.</p><p><strong>Material and methods: </strong>The potentially relevant literature was assessed independently by 2 reviewers to identify clinical studies, trials, and case series in humans describing the surgical non-regenerative treatment outcomes of periimplantitis with a follow-up of at least 6 months. MEDLINE, EMBASE, and the Cochrane Library were searched for studies reporting changes in probing depth (PD) and/or bleeding on probing (BOP) and/or radiologic marginal bone-level changes.</p><p><strong>Results: </strong>A total of 10 publications were included: 6 prospective randomized controlled trials, 1 prospective cohort study, 2 retrospective controlled studies, and 1 case series. Clinical parameters can be reduced by surgical non-regenerative treatments. Concerning 3 year follow-ups, BOP and PD values decreased more efficiently after implantoplasty than using systematic administration of antibacterials. Adjunctive local chemical irrigations or diode laser have no long-term effects. The non-regenerative surgical approach in combination with implantoplasty also shows improved radiographic parameters.</p><p><strong>Conclusions: </strong>Surgical non-regenerative treatment of periimplantitis can reduce the amount of inflammation in the short-term follow-up. Using implantoplasty may result in the improvement of clinical and radiographic parameters. Randomized controlled trials, controlled trials, prospective and retrospective cohort studies with a minimum of 20 subjects, having cases with/without exposure to the risk indicator were included. Articles written in another language than English were not included.</p><p><strong>Results: </strong>The 17 articles reviewed indicated the cement residuals at abutments were identified as risk factors for both mucositis and periimplantitis. Among the screw-retained prosthetic reconstructions, prostheses screwed directly to the implants have higher risk to develop periimplant disease. The accessibility and the possibility to perform adequate plaque control around the prosthetic suprastructure decreases the risk for periimplant disease; convex emergence profiles seem to increase it. The crown margins located submucosa may impair the periimplant treatment outcome.</p><p><strong>Conclusion: </strong>Prosthetic reconstruction on implants should be designed in a way to allow accessibility to proper plaque control. Screw-retained suprastructure and crown margins located supra-mucosa should be preferred when possible. Articles were reviewed and summarized if the following criteria were met: published evidence with recommendations on soft-tissue conditions around dental implants.</p><p><strong>Results: </strong>An evaluation of various soft-tissue parameters, including the need of keratinized mucosa, periimplant mucosal height and phenotype, midfacial tissue level, and papillary fill, was performed based on the currently available evidence.</p><p><strong>Comments: </strong>The need of keratinized mucosa is the parameter investigated the most. A trend favors a need of a wide band of nonmobile keratinized mucosa is seen with the benefit of less incidence of periimplant mucositis. In addition, the influence of the mucosal height and tissue phenotype on periimplant tissue health remains inconclusive. Although other soft-tissue parameters, including papillary fill and midfacial tissue level, are not yet proven to be related to periimplantitis, they play a crucial role to achieve successful esthetics.</p><p><strong>Conclusion: </strong>A limited amount of evidence was identified to correlate periimplant soft-tissue parameters with periimplantitis. {"title":"Maxillary Sinus Augmentation With Calcium Phosphate Double-Coated Anorganic Bovine Bone: Comparative Multicenter Randomized Clinical Trial With Histological and Radiographic Evaluation.","authors":"Kang-Mi Pang,&nbsp;Jeong-Keun Lee,&nbsp;Seong-Ho Choi,&nbsp;Young-Kyun Kim,&nbsp;Bong-Ju Kim,&nbsp;Jong-Ho Lee","doi":"10.1097/ID.0000000000000844","DOIUrl":"https://doi.org/10.1097/ID.0000000000000844","url":null,"abstract":"<p><strong>Purpose: </strong>The purpose of this study was to evaluate the clinical, histological and radiographic outcomes of the calcium phosphate double-coated anorganic bovine bone (InduCera; Oscotec Inc., Seongnam, Gyeonggi, Republic of Korea) compared with that of anorganic bovine bone (Bio-Oss; Geistlich Biomaterials, Wolhusen, Switzerland) in maxillary sinus augmentation, prospectively.</p><p><strong>Patients and methods: </strong>Twenty-eight sinuses in 25 patients (3 bilateral) with vertical alveolar bone height from 2 to 5 mm were included in this study. The maxillary sinus was augmented with InduCera or Bio-Oss, followed by a healing period of 6 months. Trephine cores were obtained for histomorphometric evaluation with the implant placement. Cone-beam computed tomography (CBCT) scans were taken at 1 week and 6 months after sinus augmentation.</p><p><strong>Results: </strong>The graft sites were healed without any notable complications in both groups. In histomorphometric analysis, the area of the newly formed bone was measured as 21.37 ± 8.87% for InduCera and 23.02 ± 5.88% for Bio-Oss, with no significant differences. Radiographically, no statistically significant differences were calculated between the groups in bone height at baseline, the height of bone augmentation, the final alveolar bone height (alveolar bone height + augmented height) after 6 months in panoramic radiograph. Records and radiographs were retrospectively analyzed. Twenty-four patients including 34 sinus augmentation procedures and 37 implants were evaluated after 10 years. Subjective and professional evaluations of the final periimplant soft tissue, implant crown, and total implant treatment were conducted.</p><p><strong>Results: </strong>The 10-year survival of suprastructures and implants was 84% and 100%, respectively. The patients were highly satisfied with the esthetic and implant treatment outcome expressed by scores >90 on the visual analog scale. The latter included a search for correlations between factors such as support (teeth/mucosa/bone), number of templates, use of fixation pins, jaw, template production, guiding system, and guided implant placement in articles related to guided surgery with stereolithographic static systems. Studies published between 2012 and 2017 were reviewed.</p><p><strong>Results: </strong>From 761 identified articles, a total of 24 articles were reviewed, which included 2767 dental implants. Data from studies analysis had shown a mean deviation of 3.08 degrees in angular position, 1.14 at the entry point, and 1.46 at apex position. Involved deviation factors were related to planning, laboratory, and surgical phases.</p><p><strong>Conclusion: </strong>Guided surgery may have a limited precision as technique, which surgeons need to be aware in the planning process. The patient presented the absence of elements 35 and 36 associated with a vertical defect impairing the adequate dental implant placement without IAN damage. A full-thickness mucoperiosteal flap was raised, and a bone window was made with a piezoelectric device centralized on the IAN canal position. After dissection, the IAN was moved buccally with a sterile elastic strip. Morse cone 4.0 × 1.5-mm implants were inserted while the IAN remained retracted. At the 4-month follow-up, the screw-retained prosthesis was installed guaranteeing the recovering masticatory function.
