It was decided to perform a CT-scan of the thorax that revealed of a rib involvement in the right hemithorax (Fig 1). We evaluated the yield of core needle biopsy in patients that the was diagnosed lymphoproliferative disorder. Investigation abdominal and pelvic CT scan and blood tests were found no other lesions. The patient underwent resection with reconstruction mesh and cement and muscle flap (Fig 2). One of its most common complications is pulmonary artery hypertension (PAH). It has been demonstrated that the development of PAH is correlated with decreased quality of life and survival. Different medications have been proposed for the treatment of PAH, among which one can name statins. However, the same as other medications for the treatment of PAH, statins are still providing controversial results across different study settings. Moreover, the effect of statins on PAH in COPD patients has not been widely investigated so far. According to the present review, most of the available clinical trials demonstrated that the utilization of statins, including pravastatin, atorvastatin, and rosuvastatin may have beneficial effects on PAH in COPD patients. The second arterial conduit to LITA is usually RITA or radial artery. We sought to compare outcomes when either RITA or radial artery is exclusively used as a Y composite graft to LITA for total arterial revascularization. Material and methods: We retrospectively analyzed 231 patients who underwent CABG in the period from 2010 to 2014. RITA was used in 178 patients (RITA group) and radial artery was used in 53 patients (radial group).Results: Radial was used more frequently in female patients and in diabetic patients. Radial group had comparable number of distal anastomoses and lesser operative time to RTIA group. Early postoperative outcomes (low cardiac output syndrome, post-operative myocardial infarction, use of intra-aortic balloon pump, post-operative stroke, re-explorations, incidence of deep sternal wound infection and death) were all comparable in both the groups. Late deaths and need for repeat revascularization were also similar in both the groups for up to 4 years after surgery. Only the incidence of major acute cardiac and cerebrovascular events (MACCE) was more in the radial group. Conclusion: Radial artery has comparable short- and mid-term outcomes to RITA when used as a second arterial conduit in CABG. Attaran","doi":"10.22038/JCTM.2019.34044.1173","DOIUrl":"https://doi.org/10.22038/JCTM.2019.34044.1173","url":null,"abstract":"Introduction: The forced expiratory flow at 25 and 75% of the pulmonary volume/forced vital capacity ratio (FEF25-75/FVC) as a spirometry parameter has been successful in the early diagnosis of chronic obstructive pulmonary disease (COPD) and the methacholine challenge test for assessing airway responsiveness.To determine the accuracy of FEF25-75/FVC for the classification of spirometry lung disease. Materials and Methods: Eighty subjects with clinical diagnosis of COPD and idiopathic pulmonary fibrosis (IPF) were entered into this case-control study. Forty normal volunteers in the control group with a PC20 of more than 8 mg/dl were also enrolled in this study. Spirometry, lung volumes, and diffusing capacity (DLCO) were measured for all the subjects by the body plethysmograph. Final diagnosis of COPD and IPF was confirmed according to patient's history, pulmonary function test, computed tomography of the lungs, and histopathology (in IPF subjects). The FEF25-75/FVC ratio was determined in each group, and test accuracy was compared with lung volumes and DLCO as the gold standard. Results: FEF25-75/FVC was able to divide the subjects into four categories and its agreement with the clinical diagnosis (kappa= 0.486) was more than the ratio of forced expiratory volume in one second per forced vital capacity (FEV1/FVC) and residual volume (RV). Accuracy assessment showed that FEF25-75/FVC had the highest likelihood ratio (133) followed by FEV1/FVC. Mid-expiratory flow parameters including FEF25-75 and FEF25-75/FVC displayed the highest sensitivity, positive predicted value, negative predicted value, and accuracy. Despite advancements in management of hypertension and access to medical care, incidence of HHD is an alarmingly increasing through time. However, information on determinants of HHD studies is limited in Ethiopia. We assessed determinants of HHD among adult hypertensive patients in Gondar University Referral Hospital, North Ethiopia. Materials and Methods: A case control study was conducted from April 1-26, 2018. Cases were adult hypertensive outpatients with cardiac complications, diagnosed within the last two years and were on follow up and care in Gondar university referral hospital during study period. Controls were adult hypertensive outpatients without history of any of the cardiac complications, diagnosed within the last two years and were on follow up and care in Gondar university referral hospital during study period. A total of 159 participants 53 cases and 106 controls were selected using simple random sampling. Data were collected using checklist and interviewer administered structured questionnaire. Multivariate logistic regression analysis was used to identify predictors of hypertensive heart disease and p-value of 0.05 was used to decide for statistical significance.Result: Most of the cases 70.9% and the controls 54.5% were in the age group of ≥60 years. Hypertensive patients who had family history of cardiovascular disease ((AOR) (Adjusted odds ratio) =4.7, 95% CI: 1.8-11.9), sedentary life style (AOR = 3.2, 95% CI: 1.3-7.8), uncontrolled blood pressure (AOR=4 95% CI: 1.8-9.0), and duration of hypertension ≥10 years (AOR=3, 95% CI: 1.1-8.7) were more likely to develop hypertensive heart disease than their counterparts. Conclusion: Multiple factors predicted HHD.
