{"title":"Analysis of Factors and Management of Hepatitis B Virus Screening in Mothers and Infants: A Scoping Review","authors":"N. People infected with HBV during infancy or childhood are more likely to suffer chronic infection to cirrhosis of the liver and liver cancer. Early detection and prompt treatment are essential for HBV infection. This study aimed to review the factors and management of hepatitis B virus screening in mothers and infants. Subjects and Method: A scoping review method was conducted in eight stages including (1) Identification of study problems; (2) Determining priority problem and study question; (3) Determining framework; (4) Literature searching; (5) Article selection; (6) Critical appraisal; (7) Data extraction; and (8) Mapping. The search included PubMed, ScienceDirect, Wiley Online Library, and Scopus databases. The inclusion criteria were English/ Indonesian-language and full-text articles (scoping review, meta-analysis, systematic review)/ documents/ reports/ policy brief/ guidelines from WHO/ other organizations published between 2009 and 2019. The data were selected by the PRISMA flow chart. Results: The searched database obtained a total of 27.862 articles. After screening, 27.325 articles were excluded because of unmet the inclusion criteria. After conducting critical appraisal for the remaining 537 articles, only 11 articles were eligible for further review. The selected articles obtained from developing countries (China, South Africa, and Tanzania) and developed countries (Netherlands, Japan, Denmark, Northern Europe, and Canada) with quantitative studies design (cross-sectional, case series, and cohort) met the inclusion criteria. The findings emphasized on four main topics around hepatitis B virus screening in mothers and infants, namely demographic factors, risk factors, post-screening benefit, and challenges in screening uptake. Conclusion: Early detection of HBV infection with prenatal screening reduce the HBV prenatal transmission, especially from infected pregnancy. Screening plays an important role in the administration of universal infant HBV vaccination and postexposure prophylaxis with hepatitis B immune globulin (HBIG) at birth. Therefore, PLWHA must take antiretroviral (ARV) to maintain their immunity. This study aimed to determine the relationship between the level of adherence to taking ARV fixed-dose combination (FDC) drugs and CD4 levels of HIV patients. Subjects and Method: This was a cross-sectional study conducted at Pengayoman Cipinang Hospital, Indonesia, in 2018. Total of 91 HIV patient over 17 years of age, had or had received FDC ARV therapy for at least 1 year, and did not experience drug-induced hepatitis were enrolled in this study. The dependent variable was CD4 level. The independent variable was level of adherence to taking ARV fixed-dose combination (FDC). The data were taken from the Voluntary Counseling and Testing Poli Pengayoman Cipinang Hospital. This study used secondary data from the Overview of HIV Care and ARV Therapy. The data were analyzed using Chi-square. Results: A total of 65.93% HIV patients had a good level of medication adherence and 79.12% had an increase of CD4 levels. There was a significant relationship between adherence to taking FDC ARV drugs and CD4 levels (OR = 6.50; 95% CI = 2.15 to 19.62; p<0.001), and it was statistically significant. Conclusion: There is a significant relationship between the level of adherence to taking FDC ARV drugs and CD4 levels. Therefore, patients must receive education and support to improve adherence to taking ARV drugs. {"title":"Determinants of Injury Incidents in the Productive Aged Groups in Central Sulawesi","authors":"Sea Septiana, I. In 2018, the proportion of injuries ranked 12th place in morbidity in Indonesia with the highest cases in Central Sulawesi Province (13.8%). This study aimed to determine of injury incidents in the productive aged groups in Central Sulawesi. Subjects and Method: This was a cross-sectional design conducted in Central Sulawesi from October 2019 to January 2020. A total of 1,323 reproductive-aged household members was selected by random sampling method. The dependent variable was the incident of injury. The independent variables were age, gender, level of education, employment status, and nutritional status. The data were collected from secondary data of basic health research data in 2018 with household dan individual questionnaires. Bivariate analysis was performed by chi-square method. Results: The bivariate analysis showed that the incidence of injury among the productive aged groups increased with age (OR= 1.50; 95% CI= 1.10 to 2.05; p= 0.021), gender (OR= 1.29; 95% CI= 1.03 to 1.62; p=0.030), level of education (OR= 2.01; 95% CI= 1.14 to 3.56; p= 0.010), employment status (OR= 1.86; 95% CI= 1.19 to 2.90; p= 0.005), and nutritional status (OR= 0.77; 95% CI= 0.55 to 1.07; p= 0.033). Conclusion: The incidence of injury among the productive aged groups increases with age, gender, level of education, employment status, and nutritional status. Therefore, it is necessary to increase the capacity of CBIA Facilitators to activate the GKSO cadres despite the busyness. In addition, it is important to enable the GKSO cadre and CBIA facilitator to bring about changes in the quality of pharmaceutical services in the community. The purpose of this study is to increase the capacity of CBIA Facilitators in providing information to the community in Tanjung Gunung Village, Central Bangka Regency, Bangka Belitung Islands Province. Subjects and Method: This was an experimental study using prestest-posttest only control group design. The study was conducted in Tanjung Gunung Village, Central Bangka Regency, Bangka Belitung Islands Province, form March-July 2019. Total of 3 pharmaceutical personnel and 24 community respondents Were selected in this study. The independent variable was capacity of CBIA Facilitators. The dependent variable was the use of antibiotic. The dara were analyzed decriptively. Results: The CBIA facilitators capacity through Agent of Change (AoC) affected the changing of the antibiotics use among community. Conclusion: The increasing of CBIA facilitators capacity through Agent of Change (AoC) also increasing the changing of the antibiotics use among community. The implementation of Picture Archiving and Commucating System (PACS) and Radiology Information System (RIS) is one of the efforts to control costs in radiology installations, especially in consumables’ efficiency (BHP). Bukittinggi National Stroke Hospital (RSSN), as one of the vertical hospitals located in the City of Bukittinggi, West Sumatra, has become a precursor to the implementation of PACS and RIS in this province. This study aimed to determine effect of material using ct scan and mri after use of picture archiving and communicating system and radiology information system at radiological installation of Bukittinggi national hospital, West Sumatra. Subjects and Method: This was a descriptive study conducted at Radiological Installation Of Bukittinggi National Hospital, West Sumatra from July 2020. The data were collected by observation and monthly report data. Results: The results of the analysis of the use of PACS and RIS had a significant impact on the cost efficiency of BHP CT scans and MRIs in the RSSN radiology installation reaching 97.9%. Indicators of the quality of nutritional services are the planning of nutritional care in accordance with the service standards for successful nutrition consultation, the accuracy of the diet served, food presentation, taste of the food, and the leftovers of the patient. The minimum amount of food left by patients is less than 20% according to the Hospital Minimum Service Standards. This study aimed to evaluate the relationship between the level of patient satisfaction with food services and the practice of nutritionist consultation as evaluated from the leftovers of patients in the inpatient ward in Hanau Hospital. Subjects and Method: This was a cross-sectional study conducted at the Hanau Hospital. The number of respondents in this study was 67 people. The independent variable was the level of patient satisfaction with food services and nutritionist consulting practice services. The dependent variable was food waste assessed by the Visual Comstock method. Food waste data were obtained using the Visual Comstock method. Assessment of patient satisfaction were using a questionnaire. The practice of consulting nutritionists was assessed by observation using a standard operational checklist sheet for nutrition consultation procedures. The data were analyzed using Rank’s Spearman. Results: From the assessment of nutritionist consultation services during nutrition consultation, it was found that 44.8% patients were in the sufficient category and 43.3% patients were in the good category. 53.7% patients were satisfied with the food services provided, both from the timeliness of serving the food, the taste of food, and the appearance of the waiters, while 15% were dissatisfied. Measurement of food waste showed that the average food waste was 30.2%. Patient satisfaction level was not related to nutritional service on the leftovers of the patient. The practice service of nutritional consultation was not related to patient satisfaction assessed by the leftovers of the patient. Conclusion: Based on the patient’s leftovers, there is no relationship between the level of patient satisfaction with nutrition services. Also, there is no relationship between the nutritional consultation and patient satisfaction. Normal delivery occurs at 37 – 42 weeks of gestation, where the fetus is mature. Labor cannot be separated from labor pain. Several studies have shown that in primitive societies, labor is longer and painful, whereas in advanced societies 7-14% give birth with minimal pain and most (90%) are accompanied by pain and unavoidable pain. The birth ball is a physical therapy ball that helps the delivery of the first stage to help labor progress. A physical therapy ball that helps labor progress and can be used to share positions. One of the movements is to sit on the ball and shake it to comfort and help labor progress. This study aimed to examine the birthing ball therapy on the long-term maternal and labor pain among primigravida mother in private practice midwives, Madiun, East Java. Subjects and Method: This was a cross sectional study conducted in Madiun, East Java. The total of 40 pregnant women were divided into 20 mothers with birthing ball therapy and 20 mothers without birthing ball therapy. The dependent variables were length of the first period of labor and intensity of labor pain. The independent variables were birthing ball therapy. Data were collected using an observation sheet. The data were analyzed using a multiple logistic regression. Results: The use of birth ball was positively associated with the length of the first period of labor (OR = 5.19; 95% CI= 1.06 to 25.4; p = 0.042) and intensity of labor pain (OR = 7.57; 95% CI= 1.53 to 37.3; p = 0.013) and they were statistically significant. Conclusion: The use of birth balls is proven to be effective in shortening stage 1 and reducing pain intensity. Nausea and vomiting are common in the early stages of pregnancy, affecting 70-80% of pregnant mothers. In most women, vomiting begins between 4-7 weeks of pregnancy. Nausea and vomiting are usually mild and self-limiting, but some mothers have a deeper course and cause pregnancy vomiting. This study aimed to analyze the effectiveness of ginger extract consumption on reducing morning sickness in first trimester of pregnant women at Pratama Mariana Clinic Medan, North Sumatra. Subject and Method: This was a quasi experiment study with pretest-posttest control group design conducted in Pratama Mariana Clinic, Medan from July to August 2018. A sample of 15 pregnant women. The dependent variable was emesis gravidarum. The independent variable was extract of ginger. The data were collected by observation and analyzed by wilcoxon test. Result: The extract of ginger is effective to reduce nausea and vomiting in first trimester pregnancy women after ginger extract was given (Mean= 1.93; SD= 0.26) was higher than before (Mean= 1.20; SD= 0.41), and it was statistically significant (p <0.001).
