In the corpus cavernosum, phosphodiesterase type 5 inhibitor (PDE5I) increase cyclic GMP concentration and cause smooth muscle relaxation of arteries and sinusoids, thus facilitating blood flow. As a consequence, PDE5I, such as sildenafil, tadalafil and vardenafil have proved to be effective, well-tolerated drugs in the treatment of ED. Increasing the frequency of sexual intercourse increases the circulating testosterone levels, and hence PDE5I therapy may affect testicular function of ED patients. ObjectiveThe aim of this study was to evaluate and compare the effect of sildenafil and tadalafil on testosterone blood level. Patients and methodsFifty-six married male patients with ED and age range 40–70 years with low–normal total testosterone level were included. Patients were randomly allocated into two groups: group I included 28 patients with ED who received 25 mg sildenafil once daily oral dose for 2 months, and group II included 28 patients with ED who received 5 mg tadalafil once daily oral dose for 2 months. Serum testosterone and luteinizing hormone (LH) levels were evaluated before starting the treatment and after 2 months of treatment. ResultsThere was increased total testosterone level, a significant decrease in LH and increased post-treatment International Index of Erectile Function values in both groups. There was a significant increase in values of post-treatment total testosterone level and International Index of Erectile Function and a significant decrease in post-treatment LH in group II compared with group I. Ancient Chinese, Indian and Greek books contain phrases that strongly point to the occurrence of female ejaculation. The referral to this subject continued through the years until Grafenberg described it in 1950. Views denying the occurrence of female ejaculation represent a minority while views confirming its presence depend mainly on subjective evidence. Controversy about the nature of the expelled fluid shows that some workers find it consistent with urine while others find it consistent with prostatic fluid. A review of the already published articles in this context was carried out using PubMed, Medical Subject Headings (MeSH) database, Excerpta Medica database, Cochrane Central Register of Controlled Trials, Google Scholar, and Scopus engine until October 2015. The following keywords were used to assess exposure, outcome, and estimates for concerned associations: delayed puberty; gonadal dysgenesis; Kallmann’s syndrome; developmental abnormalities of the pituitary; and Turner’s syndrome. A variety of congenital and acquired conditions that affect the hypothalamus and/or the pituitary gland can lead to hypogonadotropic hypogonadism (HH). Congenital HH is mainly caused by genetic defects and is divided into two main categories based on the integrity of smell sensation: anosmic (Kallmann’s syndrome) and normosmic isolated HH. Hormonal evaluation is the key diagnostic tool for the assessment of hypogonadism and basically includes testosterone, follicle-stimulating hormone, and luteinizing hormone. In borderline cases, several stimulation tests can be used, including luteinizing-hormone-releasing hormone, human chorionic gonadotropin, and clomiphene. Other hormones or imaging tests may be needed according to the cause and clinical presentation. Hormone replacement therapy is the main target in the treatment of HH and it includes either androgen therapy or gonadotropin-releasing hormone (GnRH)/gonadotropin therapy. Androgen replacement therapy is indicated to improve the symptoms and signs of hypogonadism. To achieve conception, GnRH or gonadotropin therapies are used. The proper time to start therapy is not well defined. At least 6–36 months of uninterrupted therapy is needed to achieve conception. Assisted reproductive techniques are indicated in case of failure to achieve conception with GnRH or gonadotropin therapy, depending on the quantity and quality of sperms and the fertility potential of the female. If azoospermia persists, testicular sperm extraction combined with intracytoplasmic sperm injection provides a successful option. This study was carried out to assess quality of life and erectile dysfunction among a sample of diabetic men. MaterialsIn the current study, we included 500 diabetic men attending the Andrology outpatient clinic, Diabetes outpatient clinic, diabetic foot outpatient clinic, Assiut University Hospitals, Egypt. The participants were evaluated using the Arabic version of the International Index of Erectile Function questionnaire and the Arabic version of SF-36 questionnaire. ResultsThe mean age was 53.13 ± 11.92 years. Of the 500 participants, erectile dysfunction was severe in 112 patients (22.4%), moderate in 82 patients (16.4%), mild-to-moderate in 116 patients (23.2%), mild in 158 patients (31.6%), and 32 patients (6.4%) had no impairment. Erectile dysfunction increased with age of participants, smoking, and duration of diabetes. This article aimed to review PP in both sexes. A search for a review of published articles was carried out using PubMed, medical subject heading databases, and Scopus engine. Keywords used to accomplish these concerned associations were puberty, PP, adolescence, adrenarche, menarche, pubarche, and thelarche. Etiologically, PP is divided into gonadotropin-releasing hormone (GnRH)-dependent and GnRH-independent causes. GnRH-dependent PP [central precocious puberty (CPP)] is based on hypothalamic–pituitary–gonadal axis activation associated with progressive pubertal development, accelerated growth rate, and advanced skeletal age. CPP is one of the common forms of PP resembling the normal route of puberty at an age less than 8 and 9 years for girls and boys, respectively. Peripheral precocious puberty is related to sex steroid exposure independent of hypothalamic–pituitary–gonadal axis activation. Therapy is indicated in children with CPP with accelerated bone age, height progress, or psychosocial stress to halt puberty succession to a socially satisfactory age, allowing the child to achieve optimal height potential. MethodsSixty women participated in the current study. They were investigated by using hysterosalpingography (HSG), followed by the CAT and laparoscopy. ResultsThe diagnostic accuracy of HSG was 70%, with sensitivity of 85%, specificity of 59%, positive predictive value of 61%, and negative predictive value of 83%. The CAT showed a diagnostic accuracy of 88%, sensitivity of 96%, specificity of 82%, positive predictive value of 81%, and negative predictive value of 97%. There was a substantial agreement between the results of CAT and HSG in the assessment of tubal disease with a &kgr; value of 0.631. As regards the type of infertility, CAT was found to be more positive with secondary than primary infertility. It is often quite concerning to patients and can affect psychosocial well-being of patients. This review addresses developments in the diagnosis and treatment of male DP, highlighting some clinical tips and tricks in a question (Q) and answer (A) format. MethodsThe author searched electronic databases from 1966 to November 2015, including PubMed, MEDLINE, EMBASE, EBCSO Academic Search Complete, Cochrane Systematic Reviews Database, and Google Scholar using the following keywords: male, boys, adolescent, delayed puberty, diagnosis, and treatment. ResultsClinically, DP can be diagnosed in boys who lack testicular enlargement or secondary sexual development by 14 years or who do not progress to Tanner stage V 4.5 years after the onset of puberty. Constitutional delay of growth and puberty (CDGP) is the most common cause of DP. Initial workup should include a thorough history, physical examination, and targeted investigations. No single test is considered foolproof for differentiation between CDGP and hypogonadotropic hypogonadism (HH). Klinefelter’s syndrome may present with a lack of pubertal progress rather than delayed onset. Spontaneous puberty may start in patients with CDGP; otherwise HH should be reconsidered. Reversal and relapse may occur with HH. Induction of spermatogenesis may be an issue in some cases. ConclusionA range of options now exists for the management of DP. Current therapies have room for innovation in both pharmacological and assisted reproduction. PurposeThe aim of the present study was to assess sexual satisfaction in ED patients on penile prosthesis and on oral PDE-5 inhibitors. Materials and methodsThis comparative, cross-sectional study included two groups of ED patients recruited from the Andrology Outpatient Clinic of Suez Canal University Hospital, Ismailia, Egypt: group 1, in which patients were treated with penile prosthesis implantation (29), and group 2, which included patients on PDE-5 inhibitors treatment (35). Patients were asked to answer a validated questionnaire (Erectile Dysfunction Inventory of Treatment Satisfaction). ResultsThe overall score of treatment satisfaction was significantly higher in the group treated with penile prosthesis surgery than that in the group treated with oral drugs: 93.55±7.34 versus 75.06±17.22. Libido and the frequency of intercourse increased with PPI. This type of pornography can motivate sexual behaviors that can affect health either positively or negatively. ObjectiveThe aim of this study was to evaluate the effect of soft-core pornography on female sexuality and to assess its effect on female sexual desire, orgasm, and vaginal lubrication. Participants and methodsThis is a cross-sectional study in which 200 sexually active married women were administered a self-filling questionnaire covering different aspects of female sexuality. All participants were free from any disease known to affect sexual function. ResultsIn total 52% of the participants and 59.5% of their husbands were positive watchers. An overall 51.6% of participants who were aware that their husbands were positive watchers reported experiencing negative emotions (depression, jealous), whereas 77% reported changes in their husbands’ attitude. Negative watchers were more satisfied with their sexual life compared with their counterparts. Although watching soft-core pornography had a statistically significant effect on sexual desire, vaginal lubrication, ability to reach orgasm, and masturbation, it had no statistically significant effect on coital frequency.
