{"title":"Hyperandrogenemia and virilization with simultaneous pituitary and adrenal adenomas.","authors":"J A Jackson, R C Mellinger","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>We describe a postmenopausal woman who presented with virilizing hyperandrogenemia and was found to have an intrasellar tumor and a large left adrenal mass. Pathologic studies showed on undifferentiated hypophyseal adenoma with immunostaining for chromogranin only and a benign adrenocortical adenoma. It is important that medical personnel understand appropriate ways to communicate with the patient in order to inform him about his illness, throughout its duration. Physicians must develop an understanding of how patients communicate and ways to respond to that communication. Furthermore, the physician needs to communicate with the family of the terminally ill patient. {"title":"Vertebral compression fractures at the onset of acute lymphoblastic leukemia in a child.","authors":"M B Oliveri, C A Mautalen, C A Rodriguez Fuchs, M C Romanelli","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>A child with acute lymphoblastic leukemia, spinal osteoporosis with vertebral compression fractures, and hypercalcemia appearing early in the course of the hematologic disease was followed for two and a half years. Bone mineral density (BMD), measured by single photon absorptiometry at the radial shaft, was within normal limits for age and sex. However, x-rays of vertebrae and vertebral BMD, measured by dual photon absorptiometry, showed marked demineralization. Despite leukemic remission, the spinal osteoporosis became worse and the patient required aggressive treatment for eight months. Treatment included 50 units of calcitonin subcutaneously every other day, 1,000 mg/day of oral calcium, and 3,000 IU/day of vitamin D. The back pain disappeared quickly, and laboratory controls showed a significant diminution of bone turnover. No new compression fractures occurred. Eighteen months later, the patient continued in remission and menarche had occurred. Dual photon absorptiometry revealed a significant \"catch up\" of the lumbar spine BMD. X-ray examination showed a marked remodeling of the vertebral bodies. BMD measurements in this child indicate that bone loss affected the trabecular bone compartment or occurred only at active bone marrow sites. {"title":"New purine analogues for the treatment of chronic B-cell malignancies.","authors":"T E Gribbin","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Adenosine deaminase (ADA), a purine salvage pathway enzyme, appears to play a key role in normal lymphocyte growth, development, and differentiation. Three new purine nucleoside analogues, deoxycoformycin, fludarabine, and 2-chlorodeoxyadenosine, affect the normal function of the purine salvage pathway by inhibiting ADA or by acting as analogs of the ADA substrates. These agents show significant activity in the treatment of chronic B-cell leukemias and low-grade lymphomas. {"title":"Prehospital care of acute myocardial infarction: a review.","authors":"A R Gokli, J L Kovar, T Kowalenko, R M Nowak","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Each year more than 1 million people in the United States suffer from acute myocardial infarction (MI) with most of the deaths occurring within hours of symptom onset. Over the last 25 years, different prehospital systems have evolved throughout the world which allow early cardiac monitoring and treatment of acute MI patients. Thrombolytic therapy in acute MI has been shown to decrease mortality and preserve left ventricular function when administered early after onset of symptoms. The potential role of Emergency Medical Services or Mobile Coronary Care Units in achieving early thrombolysis is under investigation. Several studies of prehospital interventions to achieve early thrombolysis are reviewed. The use of thrombolytics by prehospital personnel has been found to be feasible, safe, and effective in reducing time delays. {"title":"Intravenous nitroglycerin for acute myocardial infarction.","authors":"S Borzak","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Intravenous nitroglycerin (IV TNG) has been increasingly used in the setting of acute myocardial infarction. The seven randomized trials comparing IV TNG with placebo, and one trial comparing IV isosorbide dinitrate with furosemide, were reviewed for evidence of beneficial clinical effects. IV TNG in low dosage is safe in the setting of acute myocardial infarction and modestly effective in relieving chest pain. Favorable hemodynamic effects are most pronounced in patients with congestive heart failure. {"title":"Measurement of left ventricular ejection fraction using gated 99mTc-sestamibi myocardial planar images: comparison to contrast ventriculography.","authors":"D A Parker, R L Lloret, F Barilla, L Douthat, M Gheorghiade","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Using the new myocardial perfusion agent 99mTc-sestamibi and multigated acquisition on a nuclear medicine gamma camera, the left ventricular ejection fraction (LVEF) was derived in 13 patients with coronary artery disease (CAD). Cross-sectional activity profiles were used to measure the left ventricle from end-diastolic and end-systolic images. Several different geometric methods were then utilized to derive ejection fractions from the nuclear data. Comparison of the resultant ejection fractions to those obtained from contrast ventriculography showed significant correlation for all geometric methods (P less than 0.01, Sy X x = 6.2 to 9.6).
