The lack of immediate treatment measures can result in the loss of function, esthetics, psychological and social problems, and poor quality of life. Several factors may not enable the prompt replantation of an avulsed tooth; hence, delayed replantation has emanated as an alternative to meet the esthetic, functional, and psychological demands of the patients. Case description: This case report details the replantation of an avulsed maxillary central incisor in a 21-year-old male patient which was replanted 2 hours after the event. Extraoral root canal treatment was done before replantation and follow-up examinations were performed during 1, 3, and 6 months. The tooth remained in a stable functional position and did not reveal any pathological changes after 6 months. Both esthetic and functional outcomes were satisfactory. Conclusion: The extent of damage to the tooth and supporting structures, emergency management, and follow-up treatment play a role in the prognosis of the avulsed tooth. One of the most frequent types of procedural error is instrument separation. A separated instrument prevents complete cleaning and shaping of the root canal. Hence, every attempt must be made to retrieve the broken instrument. The clinician has to evaluate the options of attempting retrieval, and bypassing or leaving the fragment as it is. There are various instrument retrieval kits and chairside techniques available for this purpose. Case description: The present case series describes the management of a separated rotary file and a separated bur head from the root canals of mandibular second molar and maxillary canine, respectively. A 14-year-old female patient undergoing root canal treatment in relation to 47 had an accidental breakage of ProTaper gold F2 rotary file in the apical third of distal canal of 47. Attempts were made to retrieve the instrument with ultrasonically activated files under dental operating microscope and was successfully managed. A 47-year-old female patient undergoing root canal treatment in relation to 13 had an accidental blockage of the canal with 169L carbide bur in the apical third of canal. The fractured fragment was retrieved with file braiding technique. Being a field of explosive development due to new technologies, endodontics aids to provide an accurate diagnosis of the complex anatomical variations and conditions of the teeth. Radix entomolaris (RE) is the presence of an additional distolingual (DL) root in the mandibular first molars and its prevalence in the South Indian population is 13.3%. The presence of RE with middle mesial canal (MMC) in the mandibular first molar is a rare experience of the combined occurrence of two uncommon conditions requiring appropriate diagnosis and cautious management. Its management relies on the proper diagnosis and exploration of all the canals, thorough chemomechanical preparation, followed by three-dimensional (3D) obturation without hampering the original root canal anatomy. The appropriate utilization of the advanced aids of magnification and technology, such as the microscopes, loupes, and conebeam computed tomography (CBCT), coupled with the use of thermomechanically treated flexible NiTi files enhances the success in the endodontics while dealing with such rare entities. This article presents a case report of an RE with MMC which is definitely rare in occurrence and requires cautious management. Case description: A 20-year-old female patient was diagnosed with symptomatic irreversible pulpitis with apical periodontitis on the mandibular first molar. It was identified as an RE with MMC on radiographic examination using the Clark’s Tubeshift technique. After anesthetizing the tooth using inferior alveolar nerve block, rubber dam isolation was done. The access cavity was prepared and modified distolingually and the canal orifices were located. The MMC orifice was troughed along the groove joining mesiobuccal (MB) and mesiolingual (ML) canals using long shank burs and a sharp DG-16 explorer. The cleaning and shaping procedures were performed carefully to all six canals using NiTi files followed by the 3D obturation. Conclusion: This case report is a precise description of the effective management of RE with MMC which is a rare entity in our ethnic group. This case was diagnosed using the Clark’s Tubeshift technique and managed using magnifying loupe and thermomechanically treated flexible NiTi files. But the use of acidic food can lead to increased erosion after bleaching. Remineralizing treatment with a remineralizing agent is capable of reestablishing the lost enamel surface hardness after bleaching. In this study, changes in microhardness were analyzed upon the application of a remineralizing agent after subjecting the enamel surfaces to bleaching and erosive challenge. Aim: The aim of this study is to evaluate the effect of a remineralizing agent on bleached enamel surfaces subjected to erosive challenge. Materials used: (1) 35% carbamide peroxide (Opalescence PF, Ultradent)—a bleaching agent; (2) Remin Pro—a remineralizing agent; (3) 1% citric acid solution—an erosive agent. Materials and methods: Samples were divided into three groups and subjected to bleaching and erosion treatment. Group I—treated with 35% carbamide peroxide then treated with 1% citric acid solution; group II —treated with 35% carbamide peroxide and a remineralizing agent followed by 1% citric acid solution; group III—without bleaching with 1% citric acid solution. The sample size calculated using the statistical package G*power (3.1.5) with a level of significance of 0.05. The sample size obtained is n = 15, 5 samples per group. GI, GII, and GIII were subjected to the Vickers microhardness analysis. Loss of surface hardness loss (% SHL) was analyzed. Result: The lowest % SHL is in group II—24.718 when compared with group I—35.048. Work in the United States is evolving rapidly with demographic trends, technological advances, and international competition. To help workers succeed in the midst of change and economic uncertainty, the Department must anticipate new trends and adopt effective strategies. With that in mind, we set out to modernize the Department – to transform DOL into a more effective and efficient government agency that delivers benefits for its customers and stakeholders alike. During my tenure as Secretary of Labor, the Department responded in many new and innovative ways to the challenges of the 21 st Century. Four examples of progress stand out. First, the Department successfully updated the 50‐year‐old white‐collar overtime regulations under the Fair Labor Standards Act to provide millions of low‐wage vulnerable workers with strengthened overtime protections. Second, DOL achieved the first major update of union financial disclosure regulations in more than 40 years, giving union members enhanced information on how their hard‐earned dues are spent. Third, the Pension Protection Act of 2006 complements the Department's record enforcement of the Employee Retirement Income Security Act (ERISA) and achieves the President's goals of stronger funding and greater transparency of our pension system, including important reforms to expand retirement security and protect workers in defined contribution plans. And fourth, the Department has been privileged to assist our returning veterans, who have fought so bravely to defend our freedom at home and throughout the world. Among our actions, the Department published the first‐ever regulations implementing the Uniformed Services Employment and Reemployment Rights Act, launched ReaLifelines to provide one‐on‐one services to seriously wounded service members, initiated the HireVetsFirst campaign, and, this past August, inaugurated America's Heroes at Work, which focuses on the challenges of returning service members with Traumatic Brain Injury and Post‐Traumatic Stress Disorder. The Program Performance Overview in the Management's Discussion and Analysis section of this report includes many more examples that highlight the impact DOL programs have had on individuals over the last seven years. This report documents the latest evidence of the Department's transformation with program and financial performance results. While the Department did not meet all of its 2008 goals, we are continuing to focus on improving program performance. Microbial elimination from chronic lesions makes treatment all the more difficult. Apexification and pulp revascularization are the treatment options available. Apexification with calcium hydroxide has certain disadvantages and mineral trioxide aggregate (MTA) may be considered as a promising alternative. Case description: This case report describes the management of a symptomatic maxillary left central incisor with immature apex and a chronic periapical lesion. Sodium hypochlorite of 3% and EDTA were used as irrigants for microbial elimination. Repeated intracanal dressings with calcium hydroxide were given for better periapical healing. MTA was used for apical barrier formation. After confirming the hard set of MTA, obturation was done with thermoplasticized gutta-percha and access cavity sealed with composite resin restoration. Conclusion: In this case, MTA helped to form an apical barrier in the immature necrotic maxillary central incisor and showed a good clinical and radiographic success. Treatment of crown-root fractures often requires a multidisciplinary approach. Gingivectomy of the affected tooth can violate gingival contour of the tooth so orthodontic extrusion is a conservative procedure that allows extrusion of a tooth without any bone loss. Case description: This case report details the multidisciplinary management of the traumatized anterior tooth including endodontic treatment, orthodontic extrusion, and post-core-crown procedure to restore a right maxillary central incisor to achieve a satisfactory cosmetic result. The fractured fragment was removed and pulp tissue extirpation was done in 11. Obturation was completed at next appointment. The fracture line was extended subgingivally, so orthodontic extrusion was planned. After 8 weeks, the tooth had extruded around 2 mm of palatal tooth structure sufficient to provide a ferrule of 1 mm. A cast post and core was prepared, and a definitive metal-ceramic crown was placed over 11. The patient was reviewed for 6 months and 1 year. Conclusion: Management of cervical third fracture has various treatment options. Orthodontic extrusion is a conservative procedure that allows retention of a tooth without any bone loss. Cast post and core can adapt to the canal and strengthen the tooth.
