CLINICAL CASE : A 16-year-old patient with a fast-growing suprapubic tumor, accom- panied by generalized abdominal pain, sent to the hospital with diagnosis of pelvic tumor probable uterine miomatosis, to rule out malignant tumor. Pelvic ultrasound was performed with a report of multiple disseminated hypoechoic lesions in myometrium giving appearance of metastasis, without being the characteristic images of myomas, pelvic abdominal tomography reported hepatomegaly of the left lobe, intramural leiomyomas of medium and large elements, tumor markers within normal limits. The laparotomy was performed, left intraligamentary myoma of giant elements was located; and the histopathological study reported uterine leiomyoma of conventional histological pattern. CONCLUSIONS: The clinical of leiomyomas in adolescent patients represents a chal- lenge for the gynecologist. {"title":"Hemorragia intraparto y lesión renal aguda en pacientes con finalización del embarazo mediante cesárea","authors":"J. G. Vázquez-Rodríguez","doi":"10.24245/gom.v88i4.3841","DOIUrl":"https://doi.org/10.24245/gom.v88i4.3841","url":null,"abstract":"OBJECTIVE: To quantify intrapartum hemorrhage and serum creatinine (Cr) postpartum concentration to calculate its correlation and determine the frequency of acute kidney injury in patients undergoing caesarean section. MATERIALS AND METHODS: Cross-sectional and retrospective study carried out in women who ended their pregnancy by caesarean section between January 1 and December 31, 2019 at the Hospital for Gynecology and Obstetrics 3 of the IMSS. The clinical records were consulted to calculate the amount of intrapartum hemorrhage and the value of Cr to determine its correlation and the frequency of acute kidney injury (postpartum Cr >1 mg/dL). Statistical analysis: descriptive statistics, Student's T test and Pearson's correlation coefficient (r). RESULTS: The mean age was 32.44 ± 7.11 years with gestational age 32.55 ± 4.18 weeks, intrapartum hemorrhage 558.11 ± 570.62 mL and postpartum Cr 0.77 ± 0.21 mg/dL. The correlation of intrapartum hemorrhage vs postpartum Cr was r = 0.158 and obstetric hemorrhage (> 1,000 mL) vs postpartum Cr r = 0.390. The frequency of AKI was 14.03% (16 cases) and dialysis requirements 2.42% (3 cases). CONCLUSIONS: The amount of intrapartum hemorrhage was acceptable with low frequency of obstetric hemorrhage. Postpartum Cr increased significantly. correla-partum hemorrhage vs postpartum Cr correlation was positive which increased its significance as the amount of bleeding increased. MATERIALS AND METHODS: Retrospective, cross-sectional study of relational analysis of patients who came to consult at the IECH Fertility Center between the months of May 2019 and May 2020. The Kolmogorov-Smirnov normality test was used, with means and standard deviations (SD) for normal values; medians and interquantity ranges (IQR) for non-parametric values. To show differences between the groups the paired t-test and Mann-Whitney test were applied depending, respectively, on the normality of the data and the Spearman correlation coefficient to calculate the dependence between non-parametric variables. GraphPad Prism (v 8) was used, the value of p < 0.05 was considered with statistical significance. RESULTS: A total of 106 files of patients diagnosed with infertility with a mean age of 35 4.97 and a median of 19.90 ng/mL (IQR 57.06) of 25-hydroxyvitamin D were analyzed. Vitamin D deficiency (less than 20 ng/mL) was not associated with antimul- lerian hormone values (p = 0.0525); a difference was found between vitamin D con-Mario centrations for patients younger and older than 35 years (p = 0.0423) and a correlation between vitamin D and antimullerian hormone (Spearman’s rho: -0.210, 95%CI -0.41 to -0.005, p = 0.0495). CONCLUSIONS: 50% of the infertile patients had vitamin D deficiency. MATERIALS AND METHODS : A retrospective descriptive study of a series of cases of autopsies, with anatomopathological findings of multiple congenital arthrogryposes, performed on fetuses and newborns, during the established time lapse. RESULTS: The analysis included 8 cases with an anatomopathological diagnose of multiple congenital arthrogryposes, six of them with PCR positive for ZIKA. Among the ultrasound reports, altered limb movement and abnormal central nervous system development (corpus callosum and cerebellum), were the main findings; which were confirmed by the anatomopathological study. Regarding perinatal results, 7/8 cases died during the first hour of life. CONCLUSION: In case of suspicion or confirmation of multiple congenital arthrogry- poses by ultrasound, infectious processes should be ruled out; also, a clinical genetics consult should be sought to exclude associated alterations. en el sistema nervioso central. In patients undergoing benign pathology, the technique can only be performed by removing the uterine body (subtotal or supracervical hysterec-tomy). However, the concern about the possible occurrence of a neoplastic lesion in the remaining cervical stump has led to investigations into the benefits of also performing the cervical exeresis (total hysterectomy). Our goal is to present a review on the current situation of the topic, concluding that there is still no scientific consensus on which technique is the most recommended. CLINICAL CASE : 36-year-old woman who underwent supracervical hysterectomy with laparoscopic bilateral salpinguectomy due to symptomatic myomatous uterus. After surgery, he presented a high-grade preneoplastic lesion, which indicated that cervical conization was indicated. CONCLUSION: Some experts argue that the risks of performing the removal of the cervix are notable enough to indicate subtotal technique in patients without a history of the rectovaginal septum.
