Brock","doi":"10.36076/pmcr.2022.6.317","DOIUrl":"https://doi.org/10.36076/pmcr.2022.6.317","url":null,"abstract":"Background: Subacute combined degeneration (SCD) of the spinal cord with corticospinal tract involvement can impair motor functions below the level of the lesion. Case Report: A 63-year-old man developed progressive bilateral lower extremity weakness marked by difficulty standing for more than a second and the inability to ambulate, causing him to become wheelchair-bound. He was diagnosed with SCD. His vitamin B12 level was corrected and he underwent physical therapy. Despite these interventions, he experienced an overall deterioration in his strength examination. He later underwent an epidural spinal cord stimulator trial with pulsed electrical stimulation. With this, he experienced improvement in his lower extremity strength and regained the ability to ambulate independently. Permanent leads were placed with the retained faculty to ambulate with the stimulator turned on. Conclusion: Epidural spinal cord stimulation (SCS) has the potential to improve motor function and restore locomotion in individuals with spinal cord injury from SCD. Recent data indicates that 42.2% of adults in the United States are obese. Obesity can increase morbidity and mortality as a consequence of complex physiologic changes, and as a result, has many consequences for health care delivery and management of these patients. Patients who are obese often need interventional pain procedures; however, perception of higher risk can lead to less opportunity for these often very needed procedures. Case Report: The present case report reviews technical aspects, safety, and efficacy of successful kyphoplasty in a morbidly obese patient. Discussion: This case helps expand the current knowledge base of kyphoplasty in the morbidly obese. Conclusion: Morbidly obese patients, in general, should have access to kyphoplasty similar to normal body mass index patients. PDPH is normally treated conservatively with oral agents, such as nonsteroidal anti-inflammatory drugs and caffeine, or invasively with epidural blood patches (EBP). There is a paucity of evidence for the use of lumbar EBP in cases where the suspected chronic dural defect is at the cervical level. Case Report: A 47-year-old patient who underwent C4-C6 posterior extension of fusion as well as right-sided C5-C6 foraminotomy subsequently developed chronic PDPH symptoms that were refractory to conservative interventions. A lumbar EBP was performed for suspected cervical dura compromise, with near-immediate resolution of symptoms that lasted for multiple months. Conclusion: Lumbar EBP should be considered in patients with suspected PDPH from cervical dural compromise, especially in the context of a prolonged clinical course or failure of conservative means. Kyphoplasty or vertebroplasty has been used to reinstate the height of a compressed fractured vertebral body. OsteoCoolTM radiofrequency ablation (RFA) is a relatively new ablation technique and at present, current literature only supports its role for thoracic and lumbar vertebral metastases. Case Report: In the present case report, we utilized OsteoCoolTM RFA using anterolateral cervical kyphoplasty for cervical spinal metastasis with intractable pain and limited range of motion. Discussion: A successful palliative treatment was provided with a combination of kyphoplasty and OsteoCoolTM RFA. The patient has had great relief of pain and an increased range of motion of the cervical spine post procedure. Conclusion: Anterolateral balloon kyphoplasty provides stability to fractured vertebrae and when combined with OsteoCoolTM RFA, it potentially can provide effective pain relief in patients who have failed radiation and for whom surgery is contraindicated. This pilot study describes 7 oncology patients who received ultrasound-guided HA injections and their associated outcomes. Case Report: This study included 4 female and 3 male patients with radiographic evidence of avascular necrosis. Six of the patients had avascular necrosis of the hip, while one patient had avascular necrosis of the shoulder. All patients underwent single or multiple ultrasound-guided injection(s) of hyaluronic acid to the affected joint. Outcomes were tracked using a combination of subjective pain and functional improvement and/or numeric pain scale. Five patients had improvement in their symptoms, while 2 patients did not. Conclusion: Intra-articular HA may be a viable alternative to the treatment of avascular necrosis in oncology patients. Sacroiliac joint (SIJ) pain is one of the leading causes of lower-back pain; however, diagnosis of SIJ pain can sometimes be challenging. Family history is a known predictive factor for arthritis in both seropositive and seronegative arthropathies, including osteoarthritis; however, the role of family history in SIJ pain has not been studied. Study: This case-control study examined whether family history of SIJ pain is a risk factor for developing SIJ pain. Discussion: Results indicate that a high proportion of patients with SIJ pain reported positive family history of SIJ pain (58.6% vs 10.5%, P < .001). Cases were 6.5 times more likely than controls to report any relative with history of SIJ pain. Conclusion: Patients with a prior family history of SIJ pain can be identified early, monitored closely, diagnosed early, and started on aggressive physical therapy and close follow-up. Especially in the presence of a rash spreading to the distal extremity, CRPS-like symptoms are more common. Case Report: In this report, we present a 67-year-old patient who developed CRPS-like symptoms along with persistent severe pain after HZ infection, whose symptoms resolved after sympathetic ganglion blockade. Conclusions: CRPS-like symptoms are rarely to occur after HZ infection, especially in patients with HZ infection affecting the distal extremity. In these cases, early interventional procedures may be effective in resolving clinical symptoms, may affect prognosis, and may help prevent persistent pain. However, there are no known reports in the literature to date that discuss a fluoroscopy-guided single-shot approach of EBP administration immediately following a lumbar puncture (LP). Case Report: The patient is a 71-year-old man with progressive multifocal leukoencephalopathy that presented with a history of debilitating PDPH following recurrent LPs that were required for lab monitoring. The patient underwent a single-shot LP followed by an EBP via the same needle prior to removal from the skin. The patient was free of any PDPH symptoms immediately postprocedure and on follow-up. Conclusions: A single-shot fluoroscopy-guided approach of administering an EBP immediately after an LP, but prior to needle removal from the skin, may be a safe and efficient approach for preventing PDPH in applicable patient populations. Due to high infectious spread, 2 messenger RNA vaccines, Pfizer-BioNTech and Moderna, were the first SARS-CoV-2 vaccines authorized in the United States. CASE REPORT: We report a rare case of a 19-year-old woman who developed isolated sacroiliitis following the third (booster) dose of the Moderna vaccination. CONCLUSIONS: While most vaccine adverse effects are self-resolved, we describe a unique case of isolated sacroiliitis after the SARS-CoV-2 vaccination, which was then successfully treated with sacroiliac joint injection after failing medical management. Serious complications from this procedure are rare, but include dural puncture, epidural hematoma, and air embolism. Pneumorrhachis, the presence of air in the spinal canal, is hypothesized to be a common radiological finding following epidural anesthesia that is rarely symptomatic. CASE REPORT: We report the case of a 56-year-old woman who experienced right upper-extremity numbness and paralysis following interlaminar CESI, with subsequent imaging showing an air lesion in the epidural space from C6-T3. The patient’s symptoms resolved spontaneously after 12 hours of observation. CONCLUSIONS: This case demonstrates a previously unreported complication of CESI that is important for practitioners of ESI (epidural steroid injection) to be aware of.
