Razafimahatratra, Gaëtan Duval Solofomalala","doi":"10.53555/eijmhs.v6i4.146","DOIUrl":"https://doi.org/10.53555/eijmhs.v6i4.146","url":null,"abstract":"Introduction: The recognition of perioperative blood loss as a factor in patient mortality and morbidity led to the operating room patient safety checklist recommended by the World Health Organization. \u0000Methods: This was a prospective, descriptive study over a ten-month period from January to October 2019 including all the patients hospitalized in the Orthopedic Surgery and Traumatology department of the CHU Anosiala. All patients operated on for an orthopedic or trauma pathology were included in this study, patients having undergone percutaneous surgery were excluded and not included patients not operated on. \u0000Results: One hundred and twelve patients were collected. The average age of the patients was 32.9 years, there is a male predominance with a sex ratio of 2.5. Screw-retained plate osteosynthesis was the most frequent intervention (29.5%) in traumatology, in Orthopedics the installation of the total hip arthroplasty (THA) (6.3%) and sequestrectomy (11.6%) in osteo-articular infection. The mean blood loss for the whole procedure was 508 milliliters (ml) of which in orthopedics, the placement of the total knee arthroplasty (TKA) had the highest mean loss (1630 ml) followed by THA and open reduction neglected dislocations with a respective loss of and 1101 ml and 623.3 ml. In trauma, screw-retained plate osteosynthesis of the femur was the most hemorrhagic with an average blood loss of 820.3 ml and in osteoarticular infections, sequestrectomy with an average loss of 596.2 ml. \u0000Conclusion: The amount of blood loss in orthopedic and trauma surgery varies depending on the type of surgery performed. It was carried out in Madagascar since 2012 at the Joseph Ravoahangy Andrianavalona University Hospital Center (CHU-JRA) and this study is the first study concerning the UKA carried out in the country. The objective of this study is to assess the medium-term results of PUCs in Madagascar. \u0000Methods: this was a six-year prospective study from 2012 to 2017 carried out in the Department of Orthopedic Surgery and Traumatology of the CHU-JRA Antananarivo including all patients who received a UKA. \u0000Results: twelve UKA were placed in eight patients. The mean age of the patients was 73.5 years. Mechanical pain in the internal compartment of the knee was the complaint of all patients, and the diagnosis of internal tibial osteoarthritis was confirmed after the x-ray. An improvement in the IKS (International Knee Society) score was found, with the average overall score which had gone from 132.5 preoperatively to 190 at the last follow-up, an IKS knee average score increased from 65 to 95 and an IKS score function increased from 67.5 to 95. After a mean follow-up of four years, the quality of life of the patients was excellent with the KOOS (Knee Injury and Osteoarthritis Outcome Score) which went from 26/100 before the operation to 98/100. Apart from a loosened prosthesis no other complication were found. \u0000Conclusion: UKA is an ideal treatment for isolated osteoarthritis of a tibiofemoral compartment. It offers better physiological function, a less invasive surgical approach, a faster recovery time. The aim of this study was to describe epidemiology and surgical management of traumatic arterial of limbs in Antananarivo. \u0000Material and methods: This was a retrospective and descriptive study for a period of 10 years from January 2008 to December 2017 performed at the Cardiovascular Surgery Unit in JRA Teaching Hospital Antananarivo, including all patients who underwent a surgery for traumatic arterial injury of limbs. \u0000Results: Ninety-two cases of traumatic arterial of limbs has been recorded. The most victims were young (mean age=34.68), male (88.04%). The most mechanism of arterial injury was a civilian liability accident (63.04%) which 53.26% was due to stabs wounds. There were 82 cases of arterial wounds and 10 cases of blunt arterial trauma. Clinical signs were bleeding (89.12%), pulseless (46.73%) and hematoma (10.86%). Lesion were laceration (n=36), complete transection (n=32), avulsion (n=7), pseudoaneurysm (n=5), arteriovenous fistula (n=2), contusion (n=10). The most involved vessel were the brachial artery (29.34%) in the upper extremity and the femoral artery (15.21%) in lower extremity. Surgical procedure were revascularization (84.78%), ligation (9.78%) and primary amputation (5.43%). The overall limb salvage rate was 96.73% of arterial injuries successfully repaired. \u0000Conclusion: Revascularization is important in traumatic arterial injury of limbs.
