Movement is the result of activities of all the structures that make up a joint (i.e., ligaments, tendons, muscles, fascicles, blood vessels, nerves, etc.) and of the control actions of the nervous system on them. Therefore, many pathological conditions can affect the Neu-ro-Myo-Arthro-Kinetic System (NMAK). Osteoarthritis (OA) is the degenerative form of arthritis with a high incidence and a pro-longed course that affects articular and periarticular tissues such as articular cartilage, subchondral bone, and synovium, a degen-erative consequence. Instead, Rheumatoid arthritis (RA) is an immune-mediated synovial disease caused by a complex interaction between genetic and environmental factors. This review aims to compare Osteoar-thritis (OA) and Rheumatoid Arthritis (RA) in terms of pathogenesis and microenvironment and determine the main changes in a joint microenvironment regarding immunological defense elements and bioenergetics which can explain the pathological development with new thera-peutical opportunities. \u0000Keywords: Osteoarthritis (OA); Rheumatoid arthritis (RA); Homeostatic imbalances; Microenvironment

It accounts for about 80%-90% of all motor neu-ron diseases, and is characterized by a marked variability in terms of clinical forms, gene-tics, survival rate and diagnostic particularities. A diagnosis of ALS or one of the variants comes with a great burden for the patient and patient’s family because of the high morbidi-ty and mortality rate of this disorder. As a consequence, it is mandatory to optimize the ac-curacy of the diagnostic process of ALS spectrum for providing the best clinical manage-ment and quality of life for patients and avoiding diagnostic mistakes. In this study we aimed to analyze the efficacy of a 24-weeks physical therapy programme based on a particular methodology consisting of the reprogramming of the specific proprioceptive neuromuscular facil-itation techniques added to the classical physical therapy and by introducing modern interactive therapies and technologies: Capacitive Resistive Electric Transference, Instrument Assisted Soft Tissue Mobilization, kinesiological tapes and PRAMA system, compared with classical physical therapy. Our study included 26 patients, aged between 18 and 55 years, with proximal humerus fracture, who complete the 24-weeks rehabilitation programme. We assessed pain, shoulder range of motion, muscle strength and the ability to perform activities of daily living. The statistical analysis was performed using IBM SPSS and Excel 2021. The results showed statistically significant im-provement in all shoulder motion, increased muscle strength, decreased pain, and a better ability to perform daily activities. The physical therapy programme based on the proposed particular methodology has proven to be more effective than classical physical therapy, both regarding the improvement of the movement parameters compared to the physiological values, as well as the symmetry of both shoulders.

Osteoid osteoma represents about 3% of all primary bone tumors and 11% of all benign bone tumors. Data from the literature suggest that a neuromuscular rehabilita-tion program after osteoid osteoma surgery is very beneficial and improves the general quality of life.\u0000Material and methods. A 30-year-old male patient with intermittent right shoulder pain radiating to the right hand, and recurrent myalgias in the past year presented to our neuro-logical department. The neurological examination highlighted limitation of the abduction of the right upper limb. The paraclinical investigations included plain radiography of the right upper limb and electroneurography, which were normal, and native cervical MRI which revealed discrete C5 disc overflow, without visible signs of compression. The patient presented limited initial response to NSAIDs, so his treatment was changed to corticoster-oid therapy. Further, the patient was guided to undergo a rheumatological examination where a musculoskeletal ultrasound was performed, showing no any specific modifica-tion. Additionally, we indicated a native right shoulder MRI, which revealed a signal mod-ification of the proximal humeral diaphysis. We further indicated an MRI scan with con-trast of the upper right limb, which revealed a nidus at the top one-third of the humerus. Additionally, a CT scan with contrast of the same region displayed images that were high-ly suggestive of osteoma. The patient was referred to the orthopedics department, where a complete resection of the tumor was performed, and the pathology report confirmed the fi-nal diagnosis of osteoid osteoma. \u0000Conclusions. The aim is to study the effect of haloaerosoltherapy (HAT) and complex rehabilitation treatment based on it on functional recovery of COPD patients. Materials and methods: 133 patients with COPD (GOLD II-III) beyond acute period were examined. Bronchial obstruction, intensity of clinical manifestations, disturbances of cardiovascular system were investigated. The 6-minute walk test was conducted. Results: Before treatment, it has been shown that exercise tolerance significantly depended on severity of bronchial obstruction, intensity of clinical manifestations and degree of cardiovascular risk. Treatment was carried out according to three treatment complexes (TC). TC-1 was based on HAT usage with certain concentration and dispersion of aerosol, 18-20 seances per course. In TC-2, singlet oxygen therapy was included, in TC-3 mineral water was administered as drinking use and inhalation. After course of treatment, walking distance covered in 6 minutes has been increased in patients of all groups. When using complex treatment, these changes were more pronounced. Con-clusions: HAT with appropriate dispersion and concentration of haloaerosol promotes in-creasing tolerance to physical activity and may be used as a stage of physical rehabilitation with possible subsequent therapeutic physical exercises. Notably pronounced positive dy-namics of studied data was noted in COPD patients convalescents after COVID-19.

Tissue oxygenation refers to delivering oxygen to cells throughout the body. Microcirculation brings blood through small sanguine vessels to maintain the cells' supply of nutrients and oxygen. Optimal tissue ox-ygenation and microcirculation are essential for maintaining healthy tissue. Conversely, poor oxygenation can cause tissue damage, impair wound healing, and increase infection risk. Several factors can affect tissue oxygenation and microcirculation, including age, lifestyle factors (such as smoking and stress), and underlying medical conditions (such as diabetes and high blood pres-sure). To improve tissue oxygenation and microcirculation, individuals can engage in healthy lifestyle habits such as regular exercise, healthy eating, stress management, and avoiding smoking. Tissue oxygenation can also be looked therapeutic, given that topical and cosmetic treatments such as massage, pelotherapy, hydrotherapy, moisturizers, and certain skin care products can help promote healthy microcirculation at the somatic level. In the scientific literature, the focus is on hypoxia rather than tissue oxygenation. This article proposes a paradigm shift and emphasizes the homeostatic importance of microcirculation and tissue oxygenation in pathophysiological and therapeutic circumstances. The systematic review of the data from the last 2 years (2021-2022) and the meta-analysis performed on tissue oxygenation will contribute to the practical approach to the pathology circumscribed to tissue oxygenation.\u0000Keywords: Tissue oxygenation; Hypoxia; Microcirculation; Homeostasis; Microenvironment

Thermal/spa facilities often receive older patients with osteo-articular and musculoskeletal conditions. To assess the grade of the functionality, quality of life, depression, nutritional risk, cognitive state, insomnia, and the risk of falls, specific questionnaires were used. Out of a total of 43 participants, 30 were women (69.7%), and 22 older than 70 y (51.1%). There were 2 frail (4.6%) and 20 prefrail (46.5%). Conclusions: more than half of the people older than 60 who visit the Cofrentes spa present a functionality deficit condition. Women present a lower quality of life than men.
