The aim of this study is to investigate the effect of local epinephrine administration alone in reducing the frequency and severity of post-endoscopic retrograde cholangiopancreatography pancreatitis. Materials and Methods: The data of 979 patients who underwent endoscopic retrograde cholangiopancreatography were evaluated retrospectively. Age, gender, indications for endoscopic retrograde cholangiopancreatography procedure, technique, conditions that increase the risk of post-endoscopic retrograde cholangiopancreatography pancreatitis, presence of post-endoscopic retrograde cholangiopancreatography pancreatitis and it’s severity, serum amylase, leukocyte and C-reactive protein levels (before and after endoscopic retrograde cholangiopancreatography) were recorded. The data were compared between two groups as 473 patients who received only local epinephrine prophylaxis and 506 patients that did not. Results: Post-endoscopic retrograde cholangiopancreatography pancreatitis rate was 6.8% in all patients and 13.6% in patients with high risk. Post-endoscopic retrograde cholangiopancreatography pancreatitis was observed less frequently in the group that received local epinephrine prophylaxis when compared to the group that did not (9.1% vs 4.4%; p = 0.004). Post-endoscopic retrograde cholangiopancreatography amylase, leukocyte and C-reactive protein levels were significantly lower in the epinephrine group when compared to the null group (p = 0.001, p = 0.004, p = 0.001). Less severe and moderate pancreatitis was observed in the epinephrine group (p = 0.003). Local epinephrine irrigation was observed to reduce the rate of post-endoscopic retrograde cholangiopancreatography pancreatitis in patients with high risk (18.9% vs 7.9%; p = 0.002). Conclusion: Epinephrine irrigation with direct spraying method to major papillae is an easy-to-apply, safe and promising method in prevention of post- endoscopic retrograde cholangiopancreatography pancreatitis. Infection of the necrotic component of walled-off pancreatic necrosis can result in life-threatening complications. Half of the patients may develop symptoms related to pressure, infection, gastric outlet, and biliary obstruction. Moreover, organized necrotic material should be drained from symptomatic patients. In the early years, while open surgery is the standard treatment for walled-off pancreatic necrosis, it is associated with a rate of high morbidity and mortality. As a result, the use of open surgery has declined as less invasive methods, such as radiological and endoscopic procedures, have become more common. In recent years, endoscopic techniques and accessories, especially endoscopic ultrasonography, have revolutionized the endoscopic treatment of peripancreatic fluid collections. Endoscopic methods are favored over other methods because they are associated with lower morbidity, mortality, and hospital stay. They occur in patients with altered gastrointestinal motility or anatomy (previous gastricsurgery, diabetes mellitus, neurological disorders). They are rare, with an estimated %0.3 on upper endoscopy. The most common symptoms include abdominal pain, nausea, vomiting, early satiety, anorexia, and weight loss. We present a patient who diagnosed with 8-9 cm phytobezoar, and treated by endoscopic method. An 87 years old woman presented to our emergency room with upper abdominal pain, nausea, and postprandial emesis for 3 days. Emergency esophagogastroduodenoscopy revealed prepyloric antrum, gastric phytobezoar (8-9 cm) and ulcers (10-12 mm). We used an endoscopic snare to cut the bezoar into several pieces. After endoscopy, all bezoars disappeared. There was no recurrence during 1-year follow-up. In conclusion, endoscopic treatment may be a safe and viable option for the extraction of gastric bezoars presenting with gastric outlet obstruction. In our study, we aimed to evaluate the frequency of duodenal scattered white spot lesions, their pathological findings and their relationship with Helicobacter pylori. Materials and Methods: A total of 445 patients admitted to Department of Internal Medicine and Gastroenterology and underwent gastroscopy by the same endoscopist who have dyspeptic complaints. The endoscopy findings of all patients were evaluated retrospectively. Antrum and duodenal biopsies were taken of patients with endoscopic findings of duodenal scattered white spot lesions and histologically examined. Results: Two-thirds of the patients were female (60%) and the mean age was 47.1 years. The examined endoscopic reports revealed that 39 (8.8%) patients had duodenal scattered white spot lesions. The biopsies of the patients with duodenal scattered white spot lesions revealed intestinal lymphangiectasia in 10 patients (26.3%), chronic nonspecific duodenitis in 21 patients (55.2%), and Giardia infection in 7 patients (18.5%). There were 19 (n = 19) patients with Helicobacter pylori was found to be positive (p = 0.695). The frequency of Helicobacter pylori was also not found to be statistically different in the pathologically intestinal lymphangiectasia group. Conclusion: The frequency of duodenal scattered white spot lesions in gastroscopies of patients with dyspeptic complaints was found to be 8.8%. However, the confirmation of intestinal lymphangiectasia with pathology is observed in only a quarter of these patients. Gereç ve Yöntem: Haziran 2014 - Aralık 2020 tarihleri arasında tek bir merkezde perkütan olarak yapılmış olan karaciğer biyopsileri retrospektif olarak taranmıştır. Bulgular: Toplam 1783 hastaya karaciğer biyopsisi yapılmıştır. Bu hastaların yaş ortalaması 48.7 ± 14.4 olup ortanca yaş 50’dir. Hastaların 996’sı kadın (%55.9); 787’si erkektir (%44.1). En fazla karaciğer biyopsisi yapılan hastalık hepatit B virüs enfeksiyonu (n: 400) olurken bunu non-alkolik yağlı karaciğer hastalığı (n: 298) izlemektedir. Karaciğer biyopsisi yapılan tüm biyopsi hastalarının; 2014 yılında %31.8’ini hepatit B virüsü; %11.1’ini non-alkolik yağlı karaciğer hastalığı; %5’ini hepatit C virüsü; %18.4’ünü etiyolojisi net olarak ortaya konulamayan hastalıklar; %3.1’ini otoimmün hepatit oluştururken, 2020 yılında bu oranlar sırasıyla hepatit B virüsü için %17.1; non-alkolik yağlı karaciğer hastalığı için %23.7; hepatit C virüsü için %0; etiyolojisi bilinmeyen bilinmeyen hastalıklar için %6.6; otoimmün hepatit için %11.8 bulunmuştur. Pylori) infection was held responsible for changes in complete blood count and inflammatory parameters. But changes in these parameters after eradication treatment was not entirely evaluated. This study was designed to evaluate the effect of eradication threatment for H. pylori infection on inflammatory markers and complete blood count parameters. Materials and method: This single center observational study enrolled participants who came to internal medicine outpatient clinics because of dyspeptic complaints and who were diagnosed to have H. pylori infection after endoscopic evaluation, between July 2021 and January 2022. Participants who were tested negative for H. pylori stool antigen test eight weeks after eradication treatment were included. Complete blood count and infammatory parameters before and after eradication treatment were compared. Results: Mean participant age was 43.84±12.59 years. There were significant decreases in C-reactive protein (CRP), platelet to lymphocyte ratio (PLR) and neutrophile to lymphocyte ratio (NLR) after eradication. Changes in platelet counts and mean platelet volumes (MPV) were insignificant. Conclusion: CRP, PLR and NLR were significantly lower after eradication for H. pylori.
