In this treatment, clients will learn to identify specific triggers of episodes of eating symptoms, anxiety, depression, and other distressing emotional experiences. They will also learn to identify objective, specific aspects of their eating and emotions, including behaviors, physical feelings, and thoughts. Emotions such as fear, sadness, anxiety, anger, guilt, and disgust are discussed. Clients begin by reviewing the important takeaways from this treatment program, such as (1) all emotions provide information that can motivate us to take action in helpful ways; (2) staying present in the moment and taking a nonjudgmental view of our emotions can help to prevent emotions from increasing in intensity; (3) the way we think about a situation influences how we feel, and how we feel affects the way we interpret a situation; and (4) although avoiding uncomfortable emotion experiences can work well in the short term, it isn’t an effective long-term coping strategy. There can be more than one interpretation of a situation, and cognitive flexibility involves being able to consider various interpretations. The homework for this session is for the client to prepare to substitute opposite actions in place of the problematic EDBs. There are opportunities in this session to use mild imaginal exposures. The purpose of these exercises is to get in touch with urges, in order to examine the types of behaviors that happen in response to strong emotions, as well as the antecedents, consequences, and the way that moment felt to that client. High motivation can lead to treatment engagement and treatment commitment. Next, clients will set specific treatment goals they hope to achieve during treatment as well as develop manageable steps to reach treatment goals. Regular eating—loosely defined as three meals and two or three snacks per day, spaced at regular intervals throughout the day—is an important goal to include early in the treatment of eating disorders. Some clients may have treatment teams in which a different team member (e.g., nutritionist/dietitian) is responsible for addressing the goal of regular eating. If not, regular eating can be addressed as one of the goals within this treatment.
