Performing a CT scan may be difficult if a patient has unstable vital signs. In contrast, radiography may not always reliably diagnose nonmarked pneumothorax or extensive subcutaneous emphysema.</p><p><strong>Objectives: </strong>This study aimed to determine the agreement between chest radiography and CT findings in patients with blunt chest trauma. The study also aimed to determine the occurrence of occult pneumothorax and clarify the proportion of subcutaneous emphysema and pneumothorax detected through radiography and CT, respectively.</p><p><strong>Methods: </strong>We included patients (<i>n</i> = 1284) with chest trauma who were admitted to the emergency room of a tertiary hospital between January 2015 and June 2022. We excluded patients aged <18 years, those with stab injury, those without radiography and CT findings, and patients who required iatrogenic intervention, such as chest tube insertion, before imaging. We recorded age, sex, trauma mechanism, and Abbreviated Injury Scale score for each patient. From radiography and CT scans, we recorded the presence of rib fracture, subcutaneous emphysema, lung contusion, pneumothorax, and pneumomediastinum. The accuracy, sensitivity, specificity, and positive and negative predictive values were calculated to assess the reliability of radiography as a predictor of CT-based diagnosis.</p><p><strong>Results: </strong>Radiography exhibited a specificity of nearly 100% for all items. In most cases, findings that could not be confirmed by CT were not evident on radiographs. The incidence of occult pneumothorax was 87.3%. {"title":"Risk of Acute Respiratory Distress Syndrome in Community-Acquired Pneumonia Patients: Use of an Artificial Neural Network Model.","authors":"Jipeng Mo, Shihui Ling, Mingxia Yang, Hui Qin","doi":"10.1155/2023/2631779","DOIUrl":"https://doi.org/10.1155/2023/2631779","url":null,"abstract":"<p><p>This study aimed to explore the independent risk factors for community-acquired pneumonia (CAP) complicated with acute respiratory distress syndrome (ARDS) and to predict and evaluate the risk of ARDS in CAP patients based on artificial neural network models (ANNs). We retrospectively analyzed eligible 989 CAP patients (632 men and 357 women) who met the criteria from the comprehensive intensive care unit (ICU) and the respiratory and critical care medicine department of Changzhou Second People's Hospital, Jiangsu Provincial People's Hospital, Nanjing Military Region General Hospital, and Wuxi Fifth People's Hospital between February 2018 and February 2021. The best predictors to model the ANNs were selected from 51 variables measured within 24 h after admission. By using this model, patients were divided into a training group (<i>n</i> = 701) and a testing group (<i>n</i> = 288 patients). Results showed that in 989 CAP patients, 22 important variables were identified as risk factors. The sensitivity, specificity, and accuracy of the ANNs model training group were 88.9%, 90.1%, and 89.7%, respectively. When ANNs were used in the test group, their sensitivity, specificity, and accuracy were 85.0%, 87.3%, and 86.5%, respectively; when ANNs were used to predict ARDS, the area under the receiver operating characteristic (ROC) curve was 0.943 (95% confidence interval (0.918-0.968)). The nine most important independent variables affecting the ANNs models were lactate dehydrogenase (100%), activated partial thromboplastin time (84.6%), procalcitonin (83.8%), age (77.9%), maximum respiratory rate (76.0%), neutrophil (75.9%), source of admission (68.9%), concentration of total serum kalium (61.3%), and concentration of total serum bilirubin (50.4%) (all important >50%). The ANNs model and the logistic regression models were significantly different in predicting and evaluating ARDS in CAP patients. We hypothesized that RA is associated with all-cause mortality in critically ill older patients with AKI.</p><p><strong>Methods: </strong>We recorded demographics, laboratory tests, comorbidities, vital signs, and other clinical information from the MIMIC-III V1.4 dataset. The primary endpoint was 90-day all-cause mortality, and the secondary endpoints were 30-day mortality, one-year mortality, renal replacement treatment (RRT), duration of stay in the intensive care unit (ICU), sepsis, and septic shock. We generated Cox proportional hazards and logistic regression models to determine RA's prognostic values and subgroup analyses to determine the subgroups' mortality. We conducted a Pearson correlation analysis on RA and C-reactive protein (CRP) in the cohort of patients from the Second Affiliated Hospital of Wenzhou Medical University.</p><p><strong>Results: </strong>A total of 6,361 patients were extracted from MIMIC-III based on the inclusion and exclusion criteria. RA levels directly and linearly correlated with 90-day all-cause mortality. After controlling for ethnicity, gender, age, and other confounding variables in multivariate analysis, higher RA was significantly associated with an increased risk of 30-day, 90-day, and one-year all-cause mortality as opposed to the reduced levels of RA (tertile 3 vs. tertile 1: hazard ratios (HRs), 95% confidence intervals (CIs): 1.70, 1.43-2.01; 1.90, 1.64-2.19; and 1.95, 1.72-2.20, respectively). These results suggested that elevated levels of RA were linked to an elevated risk of 30-day, 90-day, and one-year all-cause death. There was a similar trend between RA and the use of RRT, length of stay in ICUs, sepsis, and septic shock. The subgroup analysis did not reveal any considerable interplay among strata. When areas under the curve were compared, RA was a weaker predictor than the SAPS II score but a stronger predictor than red blood cell distribution width (RDW) or albumin alone (<i>P</i> < 0.001); RA combined with SAPS II has better predictive power than SAPS II alone (<i>P</i> < 0.001). The Second Affiliated Hospital of Wenzhou Medical University cohort showed that CRP positively correlated with RA, with a coefficient of 0.2607 (<i>P</i> < 0.001).</p><p><strong>Conclusions: </strong>RA was an independent prognostic predictor in critically ill older patients with AKI, and greater RA was linked to a higher probability of death.
