In turn, wheeze has been associated with bacterial dysbiosis of the respiratory tract. We investigated the effects of RSV prophylaxis by palivizumab during infancy on respiratory microbiota composition at six years of age in participants of an RCT. Methods: 429 infants born between 32-35 weeks of gestation randomly received palivizumab or placebo during the RSV season of their first year of life. Children were followed for clinical symptoms until 6 years of age. At age six, nasopharyngeal swabs were collected and reversible airway obstruction was determined. Samples of 395 children were available for 16S-rRNA sequencing. Findings: We observed that RSV prophylaxis in infancy was associated with a higher abundance of Haemophilus and lower abundance of Moraxella and Neisseria, at age six years. Stratified analysis comparing microbiota at age six of children with PCR-confirmed RSV infection to children without proven RSV infection in infancy showed similar microbiota associations compared to those found with RSV prophylaxis. PCR-confirmed rhinovirus infection in infancy partly negated the impact of PCR-confirmed RSV infection on respiratory microbiota at age six. Reversible airway obstruction at age six was positively associated with Haemophilus and Streptococcus abundance and negatively associated with the abundance of health-associated spp., e.g. Moraxella, Neisseria, Corynebacterium and Dolosigranulum. Aim: To assess the impact of knowledge, attitude and perceptions about asthma, on medication adherence in children with asthma. Methods: In this prospective study, children aged 7-18 years with asthma had comprehensive clinical assessment and spirometry. A 31 item questionnaire to assess the knowledge, attitude and perceptions (KAP questionnaire) about asthma was administered. Medication adherence was assessed by a combination of self reporting, pharmacy records review, inhaler check, and tablet counting, as appropriate; and was classified into 3 groups (good, average and poor). Asthma control was assessed as controlled, partly controlled or uncontrolled based on the GINA criteria. Results: 180 children (mean [SD] age=11.4 [2.8] years, 59.4% males) were studied. Adherence was good in 101 (56.1%), average in 56 (31.1%) and poor in 23 (12.8) children. The proportion (mean [95% CI]) of appropriate responses on the KAP questionnaire in those with poor adherence (47.7 [41.2-54.2]%) was significantly lower compared to those with good (57.2 [53.4-60.9]%) and average (52.5 [49.1-55.8]%) adherence (p=0.0433). Asthma control was good in 47.5% with good adherence, compared to 33.8% with average or poor adherence (p=0.0031). Significant bronchodilator reversibility in FEV1 (≥12%) was observed in 23.7% with good adherence, compared to 39.2% with average or poor adherence (p=0.0338). Conclusions: Our data suggest that knowledge, attitudes, and perceptions about asthma affect medication adherence and asthma control. Detailed multidisciplinary team (MDT) assessment is key to phenotype children for identification of modifiable factors and consideration of licensed biological therapies (Omalizumab, Mepolizumab). We reviewed eligibility for biologics in a regional paediatric severe asthma centre in the UK. Methods: 67 patients on Steps 4-5 of GINA guideline therapies underwent protocol-based hospital, school and home MDT asthma assessment between 2015-2018. Medication adherence was assessed by electronic monitoring device (EMD). Spirometry, fractional exhaled nitric oxide (FeNO), blood tests for full blood count, IgE and aeroallergen sensitisation were performed. Results: Median age at assessment was 10.9 yr (range 6.0-15.8 yr). Male:Female - 40:27. Median inhaled steroid dose was 1000mcg Beclomethasone equivalent (range 400-2000mcg). 44 (66%) were on long-term oral steroids. Median FEV1 was 81% predicted (range 45-115%). Median FeNO was 37ppb (range 7-210ppb). 42 (63%) had adherence assessed by EMD. 35 (52%) were eligible for Omalizumab; 54 (80%) were eligible for Mepolizumab; 27 (40%) were eligible for both. There was no correlation between FeNO and peripheral eosinophil count in patients eligible for Mepolizumab (p=0.23). Conclusions: Surprisingly 80% children in our cohort fulfilled the criteria for Mepolizumab. The effect of interventions including improved medication adherence on peripheral eosinophil count is unknown. Aims: To carry out a systematic review and meta-analysis of cohort studies to understand if breastfeeding (BF) is inversely associated with physician diagnosed asthma in children. Methods: A comprehensive search was conducted on cohort studies published until June 2018 in any of MEDLINE, EMBASE, Cochrane, CINAHL and ProQuest Nursing and Allied Health Source databases. A manual search of references was conducted for additional resources. Data extraction were restricted to cohort studies or randomized controlled trials that reported physician diagnosed asthma in children aged Results: Out of 46 articles, the meta-analysis of 26 studies indicated that BF compared to no BF was associated with a 21% lower risk of asthma (Pooled effect size, 95% CI=0.79, 0.76-0.83) on asthma and BF. Similarly, children who had been exclusively BF had 28% lower risk (0.72, 0.64-0.80) of asthma and those who had partially breastfed had 17% lower risk (0.83, 0.76-0.90). The heterogeneity among studies that reported any, exclusive and partial BF were 46%, 30% and 45% respectively. There is uncertainty in what merits a clinically-relevant change in FeNO. Methods: Individual participant data were obtained from seven randomised controlled trials where FeNO was used to guide asthma treatment. The FeNO values at the start and end of a period of stable asthma were analysed. The median absolute and percentage change in paired FeNO measurements at the start and end of a stable period were determined for individuals with a baseline FeNO of Results: Data were available in 665 children (mean age 13.1, 59%male). In Africa, few studies have evaluated allergic sensitisation and in Angola, none. The aim of this study was to evaluate the profile of allergic sensitisation in children with asthma and other allergic diseases. Method: This was a cross sectional study, using the methodology of the International Study of Asthma and Allergic Diseases in Children (ISAAC), conducted from September to November 2017 in 5 to 14 year-old children in the Bengo Province, Angola. Out of total of 33 public schools, 5 (15%) were randomly selected, 3 in urban and 2 in rural areas. Allergic sensitisation was defined by positive skin prick tests (SPT) and/or positive inhalant Phadiatop. Data were analysed with SPSS Statistics v25.0. Results: The sample consisted of 1023 children, 48% girls, 58% 10-14 year-old, 61% living in urban areas. Of these children, 8% had positive SPT with most frequent sensitisation being to mites (B.tropicalis, D.farinae, D.pteronyssinus) and cockroach mix, without statistically significant differences between sexes, age and urban or rural area residents. About 56% children were monosensitised, 44% were polysensitized, and most sensitized children (55%) were asymptomatic. No significant differences in allergen sensitisation patterns were observed among atopic children who were asymptomatic, had asthma, rhinitis or eczema.
