However, the detection of AMA-M2 in asymptomatic patients causes ambiguity of interpretation. Along with the assumption that such patients may have preclinical stage of PBCh, there is an opinion about the \"healthy carrier\" of AMA-M2. The article presents the clinical observation of a patient with the high AMA-M2 titer without significant clinical manifestations, the normal level of alkaline phosphatase, with changes in liver tissue characteristic of PBCh, revealed by the results of histological examination. This form of GERD is characterized by the difficulty of establishing the diagnosis and high frequency of ineffective therapy. The article describes the main endoscopic signs of damage to the esophagus and larynx, presents normal pH-impedance reflux monitoring and criteria for pathological reflux into the esophagus and the lower third of the pharynx, as well as the possibilities of additional diagnostic methods – esophageal manometry and pepsin test. Treatment of patients with extra esophageal manifestations of GERD is a difficult task; the results of randomized placebo-controlled studies are contradictory. Early detection of OEM is an important aspect of the management of IBD patients, since timely and effective therapy of combined immuno-inflammatory pathology has a positive effect on the prognosis of the disease and reduces the disability of IBD patients. The article provides information on methods of diagnostic of ophthalmic manifestations of IBD and treatment of patients with combined immuno-inflammatory pathology. The close interaction of gastroenterologists and ophthalmologists is an important link in the implementation of multidisciplinary management of patients with IBD and autoimmune uveitis. The syndrome of excessive bacterial growth in the small intestine (SIBO) is often combined with organic and functional intestinal diseases, worsening the clinical picture and worsening the quality of life of patients. Recently, there has been an increase in the number of diagnosed cases of bacterial overgrowth syndrome in the small intestine, which is primarily due to the availability of hydrogen breathing tests (VDT), which facilitate the diagnostic process. Objective. QOL analysis of patients with IBD and IBS, depending on the presence or absence of SIBO. Material and methods. The study included 152 patients with IBD and 100 patients with IBS. All patients underwent VDT for the verification of Iron the \"Gastro+\" device using lactulose. To access QOL a general questionnaire for both groups of patients (SF-36) and were used specialized questionnaires IBD (IBD) and IBS-QOL (IBS). Results. The presence of SIBО in both IBD and IBS patients is associated with a decrease in quality of life indicators reflecting both physical and mental health. The purpose. To assess the level of adipokines (leptin and adiponectin) in blood serum, the correlation of these laboratory markers with the clinical manifestations of IBS, quality of life, eating habits, and signs of anxiety and depression based on the results of using a special questionnaire in patients with IBS. Material and methods. An open cohort cross-sectional study was conducted with the inclusion of 175 participants aged 18 to 44 years (mean age 30.06 ± 6.15 years). Group 1 included 100 obese or overweight IBS patients aged 18 to 44 years (mean age 30.63 ± 6.37 years, BMI 25.88 ± 6.28 kg/m2). In group 1, subgroup 1A included 50 people with IBS associated with overweight or obesity. In subgroup 1B – 50 people suffering from IBS, with normal weight. Group 2 included 75 people without IBS, aged 19 to 43 years (mean age 29.31 ± 5.81 years, BMI 27.80 ± 6.20 kg/m2). Subgroup 2A included 50 non-IBS participants who were overweight or obese. Subgroup 2B included 25 apparently healthy participants. IBS symptoms were assessed using a 10-point scale. Using verified questionnaires, the severity of gastrointestinal symptoms, the presence of anxiety and depression, the level of specific anxiety, quality of life and eating habits were assessed. Results. IBS patients with overweight and obesity are characterized by higher levels of leptin and low levels of adiponectin than patients with normal weight (Mann – Whitney test, p < 0.05). Adipokine levels correlate with high cravings for sweet, fatty, starchy, and salty foods. A higher concentration of leptin is characteristic of patients suffering from IBS with a predominance of constipation (Mann – Whitney test, p < 0.05). High levels of leptin and low levels of adiponectin in overweight and obese IBS patients correlate with more severe symptoms of abdominal pain, flatulence, poor quality of life, and more frequent signs of anxiety and depression (Spirmen test p < 0.05, Mann – Whitney test, p < 0.05). Conclusion.
