We will discuss the recent article by Firouzian et al. that tries to identify the impact of both positively and negatively regarded personal traits on conditioned pain modulation (CPM): resilience and catastrophization. Carvalho","doi":"10.3166/DEA-2021-0144","DOIUrl":"https://doi.org/10.3166/DEA-2021-0144","url":null,"abstract":"De plus en plus d’etudes indiquent que le microbiote intestinal pourrait jouer un role important sur les fonctions du systeme nerveux en modulant l’activite des cellules nerveuses. Il a ete montre que les produits derives des bacteries peuvent influencer la perception de la douleur. De plus, des perturbations du microbiote (ou dysbiose) sont souvent associees a des pathologies intestinales ou extraintestinales comme des desordres neurodegeneratifs ou des troubles developpementaux. Cette revue presente les etudes precliniques et cliniques mettant en evidence un impact du microbiote sur la perception de la douleur dans differents contextes pathologiques. Le lien entre le microbiote et l’activation des neurones est discute au travers de l’interaction directe hote–microbiote qui implique l’activation des nocicepteurs par les composes ou metabolites microbiens. Il s’agit dans cet article de questionner ce qui se passe pour nos patients qui font l’experience de la ketamine dans la prise en charge des douleurs chroniques et de le mettre en perspective avec la clinique psychopathologique du trauma et de la douleur chronique. Ongoing research aims at either finding new treatments to replace opioids or reducing its heavy adverse effects due to long-term use: opioid-induced hyperalgesia and antinociceptive tolerance. In a recent study, Doyle et al. (2020) demonstrate that the activation of sphingosine-1-phosphate receptor subtype 1 (S1PR1) in the central nervous system contributes to morphine-induced hyperalgesia and antinociceptive tolerance in a rodent model of chronic pain. Pourtant, cette fonction existe. Nous nous intéresserons à l’infirmière ressource douleur (IRD) qui, dans une fonction transversale, fait un travail d’IPA bien avant sa reconnaissance légale. Depuis 2007, le ministère de la Santé finance des projets de prise en charge de la douleur. En 2013, une équipe algologique multidisciplinaire composée d’un médecin, un/une infirmière et un psychologue a été mise en place dans chaque hôpital aigu en Belgique. Sa mission est de sensibiliser les soignants à la nécessité d’une prise en charge correcte de la douleur dans le but de prévenir l’apparition d’une chronicisation. The main entry pathway for SARS-CoV-2 into the cell is thought to be through binding to the angiotensin converting enzyme 2, but it seems another protein can induce its viral entry: neuropilin-1 receptor (NRP-1). NRP-1 is usually bound by vascular endothelial growth factor-A (VEGF-A), a - amongst other- pronociceptive factor. By binding to NRP-1, the Spike protein of SARS-CoV-2 blocks neuronal signaling, reducing the pro-nociceptive implication of VEGF-A. This analgesic role of SARS-CoV-2 gave rise to an increase in screenings for new compounds that could interfere with this pathway. Dans le domaine de la douleur chronique, il s’agit de mettre en œuvre un parcours de santé complexe et coordonné. En effet, les composantes biopsychosociales de la douleur chronique et ses impacts sur la qualité de vie des patients impliquent l’utilisation de thérapies multimodales et la participation de plusieurs professionnels aux compétences et modes d’exercice différents. Il convient donc de trouver des solutions pour coordonner le parcours afin d’obtenir une cohérence des postures diagnostiques et thérapeutiques entre les différents professionnels, pour hiérarchiser les actions àmener et pour rendre lisible à tous les actions mises en œuvre et leurs bénéfices obtenus.
